WAIVER OF RIGHTS

KNOW ALL MEN BY THESE PRESENTS:

We, CHERRY SOBREDILLA-CAMAGO, ROAMIL SOBREDILLA, CLAIRE
SOBREDILLA-JUAREZ, all of legal age, married, Filipino and residents of Sta. Barbara,
Iloilo, Philippines, and MELENIV SOBREDILLA, of legal age, single, Filipino and residents
of Sta. Barbara, Iloilo, Philippines, after having been sworn to in accordance with law, hereby
declare that:
1. We are the surviving children and heirs of JUAN PENUELA SOBREDILLA, who died
on October 23, 2002, in Sta. Barbara, Iloilo.

2. The said Juan Penuela Sobredilla was insured at AFP PJMC, Fort Andres Bonifacio,
Metro Manila;

3. We do hereby WAIVE, TRANSFER, and CONVEY all the rights, interests and
obligations over his differential benefits at AFP PGMC in favor of our mother,
CAROLINA OLITRES SOBREDILLA, legal age, widow, Filipino and a resident of
Sta. Barbara, Iloilo, Philippines. Who is able and willing to assume all the rights,
interests, and obligations thereto.
