1. INTRODUCTION
1.1. Description
This Code of Practice (CoP) is written to set policy and the minimum standards required for data
entry into OpenWells and PROFILE (the Group replacement for DIMS). As with DIMS,
OpenWells is a Landmark Graphics product and it is part of an integrated suite of applications
for Well Engineering which are supported by the common Engineer’s Data Model (EDM)
database platform.
This CoP will explain the strategy, the logic behind the hierarchy attributes, the operations and
NPT coding systems.
1.2. Applicability
This CoP applies to all employees within Wells. This includes all Well Engineering and Well
Services employees.
1.3. Language
The words shall, must and will indicate mandatory requirements and will require an approved
variance for deviation.

2.1. Strategy
• To create a Group common standard for Well Operations operations reporting.
• To have common well, wellbore and event attributes to allow classification that
will ensure ‘like’ well operations performance can be compared on a regional and
global basis.
• To create a reporting structure that reduces the chances of error in data entry and
provides data that can be used with confidence for performance monitoring and
to predict future projects times and costs.
• To facilitate Shell Group internal reporting and benchmarking of well operations
performance.
• To align well operation phases with the desired detail level required to produce
consistent ‘fit-for-purpose’ well activity durations and costs.
• To create an activity reporting structure that enables the DTL work process.
• To enhance Well Services reporting.
• To facilitate capture of all necessary mechanical well data in order to be able to
produce a Well Status Diagram that satisfies the requirements of future well
entries and all Safety Critical Integrity management requirements throughout the
well life-cycle.
2.2. Aim
• The aim of this CoP is to describe policy and standards for the application and
use of OpenWells, Profile and Data Analyser applications within the Shell Wells
organisation.
• All applicable Shell and Landmark training materials shall support this CoP.
• The CoP for operational reporting is a set of Shell Group agreed minimum
standards that operational reporting shall adhere to.
• A description of how the EDM data hierarchy is structured is included.
• Non Productive Time (NPT) and Lateral Learning principles are explained.
• A description of typical roles and responsibilities for a Region or Company are
laid out in matrix format (Appendix 1).
Considerable enhancements have been made to the system in order to help with consistent
reporting. These are:
(1) An improved classification of wells and operations executed on the well.
(2) A simplification of the phase coding system to mark milestones in a wellwork
program, which will ensure no bouncing in and out of phases.
(3) The introduction of a primarily two level recording system with a third level
assigned exclusively to Well Servicing, including wireline and well intervention
operations.
(4) The capture of all well services operations
(5) A common NPT and Non Conformance reporting and closeout process.
(6) The potential for the automatic extraction of data for well planning, business
improvement (DTL Analysis) and NPT and Non-Conformance reduction. This
also recognizes that there will be a heavy reliance on Phase, Operations and NPT
data to be entered accurately.
(7) A Lateral Learning functionality with the capability to record learnings at all stages
of the Well/Life Cycle.
NOTE: Persons responsible for the recording of operational data in the OpenWells
database must be familiar with the new Phase, Operations and NPT categorisations
2.5. OpenWells
OpenWells is Landmark's latest Drilling and Completions Optimisation reporting application. It
is a fully comprehensive well operations reporting, communications and data analysis system
integrated into the Engineer's Desktop.

This application is used to capture well operations information for operators or service
companies that can be used for performance analysis or analysed using engineering applications
on the Engineer's Data Model (EDM) platform. OpenWells shares a common database with
CasingSeat, StressCheck, COMPASS, and WELLPLAN.
OpenWells provides drilling, completions and well servicing information within a single
application, supports multiple systems of measurement and can be easily configured to support
multiple languages. Whether exploring, developing, or managing your asset portfolio, you can
quickly procure a complete operations history for any well through OpenWells.
NOTE: All NPT, Lateral Learnings and Root Cause Failure Analysis (RCFA)
reporting MUST be done in OpenWells.
3. CODING STRUCTURE
3.1.1. Database
This is the database name that you will select when logging on to OpenWells.
A set of regional databases has been created, residing in Amsterdam, or locally in the
Region as follows:-

Each Regional database is prefixed EPE_, EPW_, EPM_, EPG_ and EPA_ as
appropriate. The second part of the database name will then be EDM_P1 where P
(indicates the operational or Production database).
3.1.2. Company
• A Company may have several Projects
• The term company may represent an operating group within an exploration and
production company

3.1.3. Project
• A field, asset or group of sites
• All Sites within a Project are aligned to either Grid North or True North
• Project is the data level directly above the Site level
• Each project must have a unique name within the Company
• One system datum is used for each project (e.g. mean sea level that is used to
define 0 TVD for the Project)
• Depths can be referenced to the Project level system datum or to additional rig
depth reference elevations specified at the Well level
Examples of projects from EPE would be Brent, Cormorant etc. EPW project examples are
Auger, Brutus etc.
3.1.4. Site
• A collection of one or more Wells referenced from a local coordinate system
centred on the Site location
• May be an offshore platform, single land well, single subsea
Exploration/Appraisal well, or cluster of subsea wells e.g. on a template
• Data level directly above the Well level
• Must have a unique name within the Project
Examples of EPE sites would be Brent Alpha, North Cormorant etc.
3.1.5. Well
• A Well is a surface or sub sea wellhead location on which Rig or work units
conduct operations down the Well and may have one or more Wellbores
• Data level directly above the Wellbore and Event levels
• Must have a unique name within the Site
• Wellbores may include the original Wellbore with one or more Sidetracks tied on
at different kick-off depths
3.1.6. Completion
• A system of isolated wellbore openings, wellbore conduits, and equipment, which
provides a means to produce or inject fluid (gas or liquid) from or into producing
zones.

• Single Completion may provide the conduit for fluid flowing via one or more
Effective Wellbore Opening Intervals:
• Effective Wellbore Opening Intervals servicing one Completion are contained in
the same Wellbore
3.1.7. Wellbore
• Describes path of a Well from a top depth to a bottom depth
• May be a planned (or unplanned) sidetrack or lateral in a multi-lateral completion
• Data level at the bottom (same level as Event) of the data hierarchy for drilling
located beneath the Well
• Must have a unique name within the Well
• Original Hole is entered as a Wellbore and would start from surface.
• Wellbores may start some distance along an existing Wellbore - a "Sidetrack" i.e.
Wellbore 2 etc.
A well commences with the original wellbore, which is referred to as Wellbore 01. Subsequent
wellbores are added as the original wellbore is sidetracked i.e. wellbore 02 etc.
3.1.8. Event
• A time slice or period marking a phase of major operations occurring on a Well,

3.1.9. Report
A Report is a subset of an Event where operations and mechanical well details are recorded.
As can be seen the data hierarchy is such that a large development can be categorised in order to
keep data separate. This may not be suitable for single well exploration ventures. In such cases it
is quiet all right to have duplication of naming within the levels.

4. ACTIVITY DESCRIPTION
There is now a Group wide agreed coding that has the following features.
Rig Move/Rig Down Describes the event where the primary purpose is to move the rig
(RIG) on or off location. This activity may often be associated with a
“Utility Well” where such activity is booked as the work is not
specific to an individual well or wellbore. Note: Where a rig
move is part of a wellwork activity on an individual well, then the
rig move would be part of another Event Type.
Drill Only The original drilling of a well/wellbore. This would be the event
(DRO) type for most exploration and appraisal wells for instance Note:
testing activities on an exploration/appraisal well would also come
within this Drilling event. Since a new wellbore in an existing well
is a sidetrack, this event type also covers sidetrack drilling.
Drilling and Used in the past for a lot of development wells (particularly
Completion offshore) where it was the intention to drill and (permanently)
(DRC) complete the well/wellbore at one visit. Given current work
practices, it is strongly suggested that this Event type IS NOT
USED. It will continue to exist to satisfy the requirement of
historical data. Separate Drill Only and Completion events
should be chosen for such an activity in the future, and Output
reports will be configured to allow the viewing of multiple Events
in a single report e.g. Drilling and Completion.
Completion Only Describes a visit to an existing wellbore for the purposes of
(COM) installing a completion including associated wellhead/tree
components, and potentially perforating. This would be the case
both for rig installation and where for instance a Production Hoist
is used to install the completion after a Drilling Unit has left the
location.

Clarification Notes
(1) Activity on multiple wellbores in a well (e.g. a multi-lateral well) will each form a
single Event, and the reports associated with these activities will be associated
with the individual multi-lateral legs (wellbores). The first activity within the
Event would cover drilling of the parent wellbore, together with all activities up to
the point where new formation is drilled in a second wellbore (i.e. on completion
of POOH the window milling assembly). Subsequent operations (i.e. picking up
and running the BHA for drilling) would be recorded against the new wellbore
and new event.
(2) Sidetracking of an existing well will consist of 2 Events. The first covers
abandonment operations on the existing wellbore and preparation for
sidetracking, including e.g. whipstock setting and casing milling up to the point
where a BHA is to be picked up, run and new formation drilled. The BHA
operations would constitute the first operations on the new wellbore and would
therefore be a separate drill only Event.
(3) Unforeseen activities that interfere with the progression of work (e.g. security and
weather related delays for instance) can be captured within the Event that the
activity occurs on, or alternatively for extended delays, may be recorded under a

separate Event against the well or Utility Well for that installation. These
activities will mostly likely constitute NPT. The choice of whether to capture
against the well or dummy well, will depend on local budget handling of such
delays.
(4) Batch conductor setting (drilled and set), drilling or completion must be handled
by separate Events against each well.
(8) A clear differentiation has been made in these Event Types between Workover
(tubing removal) and Well Intervention (work within the existing tubing).
4.3. Phases
There are now 21 Phase Types and 51 Phases. These are listed below (Full definitions of the
phases are contained in Appendix 2).
(2) Core, Evaluate and Secure Hole have been added to ensure well planning and
performance measurement match.
(3) A Pilot Hole Operation has been introduced to cater for both surface operations
and reservoir tagging operations.
(4) Start and end points are clearly defined. These signify milestones in the well
activity and do not correlate exactly with previous definitions but will reduce error
in data entry.
(5) There is a much reduced chance of reverting to a previous phase once the next
phase has commenced.
4.4. Operations
The “Reason” field has now been removed and two levels of operation reporting are now
available. Simplified examples are shown below.
Drilling Operations will mainly work on Phase and Operation.
There are Well Services codes in Operations (Level 1) and additional, detailed activity codes
exclusive to Well Services in Operations (Level 2) to enable performance measurement.

Well Service performance is predominantly being measured in two levels, as there are far fewer
phases associated with Well Services Operations. A short example showing how this works is
shown below.
Appendix 4 details the allowable Operation Codes per phase. The system is configured such
that only these reduced sets of operations codes are available for selection when operating in
the various phases. The intention is to make it easier to select appropriate operations codes. If
during the course of operations additional operations codes are found necessary then this must
be requested via your local EDM Focal Point.

5. NPT REPORTING
Global NPT Picklist for DTL Benchmarking is listed below: Full details of the NPT codes and
their descriptions is contained in Appendix 5.
The expanded list to assist in ‘drill-down’ of the Global NPT List is listed below. This list shall be
the prime list in the NPT entry screen and the Global NPT code shall be a default selection after
the appropriate code below is entered.
Response Matrix for Well Trouble Events below provides guidance for the appropriate
team makeup.
The Rig Superintendent and Engineering Team Leader should determine the severity of the event
considering both actual / potential consequences of the event or inability to achieve defined well
objectives and the frequency in which the event has occurred in industry, in EP Wells and within
a specific location. As an example, location could be Gulf of Mexico operations or a specific field
/ rig.
For consequence, use the highest impact either actual or potential. For frequency ask how often a
similar event has occurred. It is important to consider the potential of events so that escalating
factors can be understood and the appropriate level of response identified and implemented.
For events with a “severity level” of A, B or C, refer to ‘Severity Matrix for Well Trouble’ below.
The Rig Superintendent and Engineering Team Leader have authority to implement immediate
action.
For events with “severity level” D or E the appropriate Level 4 or Level 3 manager
input to and concurrence with the TOR is required before initiating the response.
Depending on the nature of the problem and practical limitations, the RCFA can be done in a
formal session or the RCFA Team Leader can collect information and base the analysis on the
obtained information.
As the threshold for low severity events (i.e. certain A or B events) is low, such that there may be
many of these events to report, the decision may be made by the rig superintendent and the
engineering team leader not to follow up on these events. Good judgment on the part of the
RCFA accountable party (see Response Matrix below) is required here. Particular attention needs
to be given to the re-occurrence and frequency of a particular A or B event. If the frequency /
chance of re-occurrence is high and the cumulative impact of multiple low-severity events is
significant, it is prudent to follow through with a thorough investigation.
Recommended tools for events with severity level C, D or E (refer to Severity Matrix for Well
Trouble and Response Matrix for Well Trouble Events below) are TapRoot and Tripod Beta but
equivalent tools can be used. Both TapRoot and Tripod are also used in the HSE incident
management process. TapRoot should be used in conjunction with EquiFactor in the case of
equipment failures. HSE staff may be available to help out with utilising TapRoot and Tripod
Beta tools.
In conducting the investigation, attention should be given both to the action(s) that led to the
event and the condition(s) that made it possible for the event to happen. Ask yourself 5 times the
question “why”, looking for the underlying reasons. If an error was made, look for what led to it.
After the root-cause has been established, the investigation team should recommend remedial
actions as appropriate. Consider cost/benefit for recommended actions and apply ALARP
principles. The recommendations will be evaluated by a Regional Technical Expert (RTE,
formerly named SME = Subject Matter Expert) before being implemented. For events with
severity level C, D or E (refer to Severity Matrix for Well Trouble and Response Matrix for Well
Trouble Events below) it is recommended to include the RTE as a regular member of the RCFA
team.
As indicated above, events that are considered low severity (i.e. certain A or B events) may not
warrant a full RTE review. Again, good judgment by the RCFA accountable party is required
here. In case there is no full review, the RCFA accountable party and RTE may have a brief
conversation (e.g. by e-mail or phone) followed by efficient closeout of the RCFA report (see
Documentation and Communication Process below).
The Response Matrix describes the levels of documentation required. At a minimum, each event
along with appropriate recommendations and follow-up should be summarised in ‘OpenWells’
using the NPT Report. The process for initiating the report, tracking progress, implementing
recommendations and closeout are outlined in the five-step process below.

STEP 1 – The RCFA responsible party (or delegate; note that delegation applies to reporting
only; the RCFA responsible party remains the key responsible/accountable party during all the
steps of the process) initiates the NPT report in OpenWells. General, NPT and Equipment
Failures Sections are completed as follows:
• RCFA Reporter (i.e. the person filling out the RCFA report)
• Classification within response matrix (Refer to Figure 5.2 below) determining this
classification
• Time lost in hours
• Trouble / learning code, according to OpenWells NPT codes
• Learning and Completion/Review date
• Cost impact (in USD)
• Summary and Incident detail, including the TOR
• Classification of the NPT event includes:
o Source of learning (e.g. wellsite)
o Equipment type
o HDI main & sub categories for characterizing event / incident
o Potential “worst case scenario” severity matrix classification
o Owner/Manufacturer, hole size and depth

STEP 2 – Detailed root cause analysis of the failure is carried out by the team assembled by the
RCFA responsible party (or delegate). The appropriate regional technical expert (RTE) is
identified. It is recommended to include the RTE as a member of the RCFA team for more
severe events (C, D & E). NOTE: in the case where the rig superintendent and engineering team

RTE and arrange for an efficient close-out of the NPT report. Note that initiation and
completion of an NPT report is still required even when events are not analyzed using RCFA.

STEP 5 – After checking with the RCFA responsible party, the RTE completes the relevant
section of the NPT report. Either the RCFA responsible party or the RTE closes out the report
by checking the “completed” flag on the Close out Comments section report. This will remove
the NPT report from the list of active / outstanding report tracked by the NPT coordinator.
It is expected that actions and recommendations made in the NPT report are addressed /
implemented in a timely fashion, and are discussed in Drilling & Completions Network meetings.
NOTE: for more significant events (i.e. C, D & E events, see Appendix 1) Taproot and
Tripod Beta reports may be required (Refer to Figure below). These reports are to be
attached to the NPT report in OpenWells.

6. DATA MIGRATION
The move from a three tiered to a two tiered approach will have an impact on the data being
migrated and the ability to analyse historical performance. The ability to analyse will be enhanced
but the quantity of the historical data shall be reduced.
The new coding system is essentially building for the future, particularly at the two operations
levels.
Phase durations will be marginally affected but new phases should be within 10% accuracy
compared with manual conversion. The error will arise due to slight changes in phase definitions
and automated transfers where NEV codes have been used in EPIDORIS databases. A script
will have to be written to work around this issue.
The migration of the operations data shall be predominantly from ‘Operation’ to ‘Operation
Level 1’. ‘Reason’ instead of ‘Operation’ have been used for critical ‘Reasons’ such as “Well
Control”, BOP Testing” etc.
The Global NPT codes should be capable of being created without any problems from the
existing NPT data and the Global detailed code should migrate well in the majority of cases.
There may be an issue where a service and an equipment failure have not been well enough
defined in the historical data.

DCM Drill / Mill Shoe Track To make planned progress through shoe track equipment or

These modes of use can be alternated within one run, e.g. three
singles rotary, one single steering using the down hole motor
only. If that is the case than this should be reflected in the way
the activities are coded. It is possible to record every change in
mode, but in most cases this will be impractical. Instead, one
could divide the time during which the drilling was done
alternating from one mode to the other into two parts, coding
one part with DRR and the other with DRM. Example When
drilling for 10 hours alternating between "rotary" and “down hole
motor" mode, where the “down hole motor" mode takes up 40%
of the time, this could be reported as an activity of 4 hours
coded with DRR and 6 hours coded with DRM. If the codes are
used in this way, meaningful statistics for the performance

DRS Drill with Rotary Steerable Making progress through formation and/or cement making use
of a rotary steerable assembly. Includes: connection times,
routine flow checking after a drilling break, drill pilot hole as part
of drill/drive conductor, CT drilling operations. N.B. See the

REP Repair/Replace Replacing, repairing or modifying any surface or downhole failed
equipment. if this is done while the Work Unit is not involved in

ROV ROV Operations Includes: All ROV operations when they are the primary well
work activity on the rig. Excludes: ROV operations where all
well work activities proceed unaffected.
RPI Retrievable Packer - Install and Operations associated with running a retrievable packer.
Remove via DP/Tubing Includes: Making up running/retrieval string, RIH/POOH, Setting
packer, unseating packer and pulling same. Excludes: Pressure
testing (PRE).
RUD Rig-Up/Down On-site equipment and materials preparation for well servicing
operations. Includes: Moving well services equipment around
the rig, clearing rig floor, R/U & R/D chicksans and other lines,
Nippling up & down of BOP's / diverters / risers/shooting nipples
etc, Initial pressure testing, M/U wireline tools, rig up sheaves

SAF Safety Related Covers non-routine safety related activities that are not
specifically covered by another Operations code. Includes: rig
shut down waiting on Accident Investigation, lifeboat drills, non-
standard safety briefings, Time out for Safety. Excludes:
Routine Toolbox Talk, JSA or STOP intervention. This code
shall only be used at this level if wireline or tubing operations
