Neurapraxia: A nerve lesion which causes paralysis without peripheral degeneration.
Axonotmesis: A nerve lesion in which nerve fibers are so severely injured that peripheral degeneration occurs.

SENSORY LEVELS
1. Local Sensory Levels: เช่นมี Radicular pain ไปที่ shoulder = C4,5 Root compression
Hyperesthesia = Increased sensation
Hypoesthesia = Decreased sensation
Paresthesia = Abnormal sensation
Anesthesia = Loss of sensation
2. Long tract Sensory Levels:
2.1 Proprioception & Vibration: เช่น Loss of proprioception (Pallesthesia) of lower limb = Thoracic cord lesion
2.2 Pain and Temperature: เช่น Loss of pain & temp from level of umbilicus Lt foot = T9 lesion
Hypalgesia = decreased pain sensation
Analgesia = Loss of pain sensation
MOTOR LEVELS
1. LOCAL MOTOR LEVEL: เช่น Atrophy of small muscle of hand = C8, T1 lesion
2. LONG TRACT MOTOR LEVEL: เช่น Paralysis of arm and leg ข้ างเดียวกัน = Cervical cord lesion;
Paralysis of leg = Thoracic or lumbar cord lesion
ANS LEVELS
1. Local ANS Levels: เช่น Horner’s syndrome = C8, T1, T2 lesion;
ความผิดปกติของ urination & defecation = sacral cord lesion
2. Long Tract ANS Level: เช่น Loss of sweating, change of BP, Abnormal urination, Defeaction, Sex and reflex

2. PATHOGENESIS:
2.1 Morphological damage: ไขสันหลังถูกทาลายทันทีหลังบาดเจ็บ เช่น ถูกแทงที่คอ, ถูกยิงทะลุไขสันหลัง ทาให้ มี anatomical
damage อย่างถาวร
2.2 Vascular damage: เส้ นเลือดของไขสันหลังถูกตัดขาด ทาให้ ไขสันหลังขาดเลือดไปเลี ้ยง
2.3 Pathological and Biochemical changes after cord injury:
จากการทดลองในสัตว์ เช่น ลิง exposed cord
ใช้ น ้าหนักกด 300 g/cm2 กด cord จะเกิด paraplegia ทิ ้งไว้ 6 สัปดาห์จะคืนปกติ
ใช้ น ้าหนักกด 500 g/cm2 กด cord จะเกิด permanent damage เมื่อดู cross-section ของ cord ในระยะเวลา
ต่างๆกัน จะเป็ นดังนี ้
5 นาที = ไม่มีการเปลีย่ นแปลง
30 นาที = มี petechial hemorrhage รอบๆ grey matter
3 ชม. = มี hemorrhagic necrosis
Norepinephrine ทาให้ เกิด vasospasm แล้ วเกิด hypoxic ischemia ตามมา และมี necrosis ของไขสันหลัง
2.2 Incomplete cord lesion ไขสันหลังเสียหน้ าที่บางส่วน เช่น ยังเหลือ perineal sense ผู้ป่วยอาจมีโอกาสฟื น้ ได้
เช่น hemisection spinal cord injury

3. X-ray spine: Cervical Fx = 67% Thoracic Fx = 8%
Thoracolumbar Fx = 17% Lumbrosacral Fx = 8%

5. PROGNOSIS:
1. Motor เสียหมด แต่ perineal sense ยังดี โอกาสฟื น้ (Recovery) มี 50%
2. Motor เหลืออยู่ เช่น กระดิกเท้ าหรื อนิ ้วเท้ าได้ มีโอกาสฟื น้
6. MANAGEMENT:
1. First Aid: Stabilization ของ spine และ cord เวลานาผู้ป่วยส่งโรงพยาบาล
2. Neurologic Function: เช่น ทา traction, spinal fusion
3. Prevention of Complication:
3.1 Paraplegia, Quadriplegia, Sphincter เสีย เกิด urinary retention infection/stone
3.2 Bed sore – พลิกตัว ทุก 6 ชัว่ โมง
3.3 Pneumonia ในรายที่มี high cord injury
3.4 Paralytic ileus
3.5 Psychotherapy
CORD COMPRESSION
1. SEVERITY
ขึ ้นกับแรงและระยะเวลาที่กดทับ และระยะเวลาการเป็ นอัมพาต
2. SYMPTOMS & SIGNS
1) Pain: Local pain
Radicular pain – กด nerve roots
Funicular pain – ปวดซูซ่ า่ ตามแนวกลางของกระดูกสันหลัง มักเกิดจาก Intramedullary tumor
2) Motor deficit:
LMN lesion – at level of lesion
UMN lesion – below level of lesion
**ถ้ าเป็ นที่ C1 – C4 ผู้ป่วยจะเสียชีวติ เร็ ว เพราะมีอมั พาตของกล้ ามเนื ้อหายใจและกระบังลม
3) Sensory deficit:
Hypalgesia
Analgesia
Paresthesia
4) ANS: Horner’s syndrome
Loss of sweating ขนไม่ลกุ อุณหภูมิผิวหนังลดลง
L1 – L3 เสีย ejaculation
S3 – S5 Autonomic bladder, No erection
