residence and postal address at #67 Horizon Street, SSS Village, Marikina City ,
after having been sworn to in accordance with law hereby deposes and states
that:

1. I am at present an incoming 4th year Nursing student at Trinity

University of Asia, Quezon City (Trinity);

2. I am here in the Philippines purposely for my college studies as

mentioned above;

3. I AM A BONA FIDE AMERICAN CITIZEN having been born to

American Spouses Raymond and Josephine Camacho of Texas, USA;

4. I am a holder of an American passport with passport no.

443776128. A copy is attached to this Affidavit for reference as Annex “A”;

5. I UNEQUIVOCALLY STATE THAT I DO NOT HAVE A DUAL

CITIZEN STATUS OR NEITHER WAS I A HOLDER OF DUAL
CITIZENSHIP or has been or ever had the occasion to be under a status of
dual citizen as a Filipino or as any other national except for my US Citizenship
which I presently enjoy; and

6. I am executing this affidavit to attest to the truth of all the

foregoing and for whatever legal intents and purposes this may serve.

Affiant further sayeth naught.
