AFFIDAVIT OF AUTHORIZATION

I, ALEJANDRO J. CASTILLO, of legal age, married, Filipino citizen, and a

resident of 191 Macapagal Village, Mabalacat, Pampanga, after having been duly sworn
to in accordance with law do hereby depose and state – THAT:

1. I am the eldest of the three (3) children of deceased Vicente Castillo and
Lilia Julao Castillo;

2. One of my siblings is ARNOLD J. CASTILLO who was born on October

16, 1981;

3. When our parents died, my brother ARNOLD J. CASTILLO, who was then
of legal age, decided to stay with his friend, MILTON SALUNGA LAGUNA,
at their house in Barangay Mawaque, Mabalacat, Pampanga;

4. I am fully aware that the family of MILTON SALUNGA LAGUNA

considered and treated my brother as if he is their own blood and even
provided support for his education;

5. Recently, my brother was lifted and rushed to Stavanger University

Hospital in Norway due to a medical condition and his condition remains
critical;

6. Thus, considering that MILTON SALUNGA LAGUNA has served as his

personal confidante in basically all matters respecting his employment as
a seafarer, if not every aspect of his life, it would be in accord with my
brother’s desire to see his friend, MILTON SALUNGA LAGUNA, visit him
and make important decisions as regards his medical condition;

7. Accordingly, I for and in behalf of our sibling, hereby authorize and give
my consent to the intended visit of MILTON SALUNGA LAGUNA to
Stavanger University Hospital in Norway as requested;

8. I therefore executed this Affidavit to attest to the truth I mentioned herein

and for whatever legal purposes it may serve.
