That I, RUEL B. MEOT, of legal age, Filipino, married, and a resident of barangay San
Miguel, Bacong, Negros Oriental, under oath, do hereby depose and manifest:

That I was a Member of the Social Security System since 2008 when I was working as a
Security Guard with NEMA Security Agency located at Bantayan, Dumaguete City, Negros
Oriental;

That in 2010 I transferred employment to CORDIAL SECURITY AGENCY, located at

the Boulevard, Dumaguete City, Negros Oriental until 2011;

That sometime in May 2011 again I transferred employment to 719 SECURITY

AGENCY, located at Real Street, Dumaguete City, Negros Oriental until the present;

That I was never been employed by AARON N. BENTER/ ASIATIC SECURITY ever
since, hence I am surprised why said agency paid for my premium with the SSS because the
aforementioned employers were the ones who paid for my premiums with the Social Security
System;

That I executed this affidavit freely and voluntarily not to defraud any private or
government agency of the Republic of the Philippines; but just to attest the truthfulness and
veracity of my statements herein
