AFFIDAVIT OF LOSS

I, JOEL C. MENDEZ, Filipino, of legal age, with principal office address at 31 Roces
Avenue, Brgy. Paligsahan, District IV, Quezon City, Philippines, after being duly sworn in
accordance with law, depose and state:

1. That I am the Managing Director of Weigh less Center., a ownership duly

organized and existing under and by virtue of the laws of the Republic of the
Philippines with principal office at 31 Roces Avenue Quezon City;

2. That being registered with the Bureau of Internal Revenue, Joel Mendez. with Tax
Identification No. 120-644-683-008 has been issued a BIR Certificate of
Registration for its Fairview Branch located at the 2 nd Floor SM City Fairview,
Greater Lagro, Quezon City;

3. On or about June 2009, the original copy of the said BIR Certificate of
Registration could not be found and that despite of diligent search and efforts, the
same could no longer be searched, which I believe is now lost beyond recovery.

4. As such, I am executing this Affidavit of Loss to attest to the truth of the

foregoing and for whatever legal purposes that this may serve.
