AFFIDAVIT

I, JOVENTINO P. MARIBBAY, of legal age, Filipino, married and a resident of

23-D Arao Street Ugac Sur, Tuguegarao City, after being sworn to in accordance with
law, do hereby depose and state:

1. That I am the sole proprietor of a retail store which I operate under the
business name J.P. Maribbay Store;

2. That due to my advanced age, I can no longer manage the aforementioned

store and as such, I am now transferring the ownership and management of
the store to my son, JOVEN A. MARIBBAY;

3. That the transfer of the ownership and management of the store to my son is
not to defraud creditors nor to escape any liability;

4. That in view of the foregoing, I hereby request the Department of Trade and
Industry and the Business Permits and Licensing Office to cancel the
Certificate of Business Name Registration and Mayor’s Permit, respectively,
issued in my name and to issue another in the name of the new owner
thereof;

5. That I am executing this affidavit to attest to the veracity of the foregoing

and for whatever legal purposes it may serve.
