NUR4948L Preceptorship Evaluation

As the semester comes to an end we value your input as the clinical preceptor and will use
your comments in the student’s final semester evaluation. If you can take a minute to write
a brief narrative addressing these headings we will incorporate into the final evaluation.

Motivation to Learn

Hannah is very eager to learn all that she can from every case that she can. Hannah
always asks pertinent questions and appropriately uses critical thinking to learn much more than
just the basic nursing care for a patient’s condition. Hannah has always been very open to
criticism and promptly incorporates new education into her nursing care.

Patient Assessment

Hannah has gown and refined her skills in patient assessment over the past 180 clinical
hours. Hannah is able to quickly and accurately assess patients’ conditions and formulate care
based on clinical assessments. We have had many discussions about the skillful art of patient
assessments in the neuro world and Hannah has listened and implemented every bit of teaching
to hone her assessment skills.

Communication

From hour one till hour one hundred and eighty, Hannah has also improved her
communication skills as a nurse. Hannah is constantly improving her ability to communicate
patient condition to physicians, communicate personal needs to her preceptor, and communicate
patient needs to the entire care team.

Skills

Like any nursing student, hands-on skills cannot be perfected in the short time that is
nursing school. Hannah does have great attention to detail and takes great attention to ensure that
all skills are completed to the best of her ability. Hannah has been very receptive to learning
about nursing skills and making adjustments to her skills when needed. All her nursing skills are
of the upmost percentile for that of any nursing student.

Safety

Hannah has shown that she always has safety in mind. I have never had any doubt or
concerns about Hannah concern for patient safety or personal safety. Hannah is always taking the
extra steps to ensure patient safety and personal safety.
Documentation

Hannah has also learned very quickly how to perfect her documentation. Hannah has also
shown that she will seek out additional ways to document a patients’ assessment if Cerner does
not have the appropriate documentation available. Hannah’s documentation skills are on par with
that of an active ICU RN.

Professionalism

Hannah has had consistent professionalism with everyone during this preceptorship.
Once again, Hannah’s professionalism has never been a topic of concern for me or any member
of the unit.
