JOINT AFFIDAVIT OF UNDERTAKING

WE, MARLON R. SILARAN and MARITA RIVERA, both of legal age, Filipinos
and residents of Barangay Faustino and Cabaruan, Cauayan City, Isabela, respectively,
after being sworn to in accordance with law, do hereby depose and state:

1. That I, Marlon Silaran, is the authorized driver of a 10-wheeler truck bearing

Plate Number ACN 5552 which is registered in the name of MARITA RIVERA,
the registered owner of EDN TRUCKING located at Cauayan City, Isabela;

2. That at about 11:00 o’clock in the morning of June 27, 2018, I, Marlon
Silaran, parked the aforementioned truck along Luna Street Buntun Highway
infront of CB Machineries to load a generator set. My companion initially
opened the wing door of the truck and when we were loading the generator
set, I saw that it will not fit so I fully opened the wing door of truck;

3. That upon lifting higher the door wing of the truck, a traffic enforcer told me
that I hit a Closed Circuit Television Camera (CCTV);

4. That I pulled down the wing door and saw that indeed a CCTV Camera was
installed at the post and the clamp which supports the CCTV Camera was
bent. Likewise the CCTV Camera sustained damage and it is due for
replacement;

5. That in view of the foregoing, we hereby undertake and bind ourselves to be

solidarily liable for any damage being caused to the CCTV Camera and to
shoulder all the expenses for its replacement;

6. That we are executing this affidavit freely and voluntarily without being
forced, coerced or intimidated and to attest to the veracity of the foregoing.

IN WITNESS WHEREOF, we have hereunto set our hands this _____ day of
