(U-2 Hydrogen Escape Issue During Emergency MCC Checking dated 17/11/2017)

Present Condition:-
 Unit-1& 3 CPR MIV is in closed condition. Unit-4 CPR MIV is in open condition and after
closing its MIV, no change observed in CPR position (close), so again MIV kept open.
 Unit-2 CPR MIV was open. After closing the MIV, seal oil pressure decreased from 10.8 to
9.24 & pressure got normal to 10.8. so CPR MIV closed (CPR passing). Now CPR MIV
closed.

Sequence of Event:-

1) DC SOP started before checking of Emergency MCC.

AC SOP-2,DC SOP&AOP-2 were running.
Parameters are:-
Seal Oil DP:-1.39 &1.42 kgsc
Seal Oil Pump Dish Press. :- 11.042/11.045 kgsc.
Lube Oil Press.:- 6.57(HDR) 2.59 kgsc.
H2 Press. :-3.21 kgsc.

2) 10:36:07hrs: After TRIPPING OF EMCC was tripped, AC SOP-2 and AOP-1 got tripped and
DC EOP started in AUTO but due to CPR passing DC SOP dich. Pr. reduced from 11.02 to
5.04 kgsc.
Parameters are:
DC SOP discharge pressure:- 5.044 Kgsc
Seal Oil DP:- 0.40/0.45 Kgsc
Lube Oil Press:- 0.77 Kgsc
H2 Press.: 3.22 Kgsc

3) 10:36:27hrs: AC SOP-2 started after EMCC charged through DG.DC SOP dich. pr. increased
from 5.04 to 6.75 Kgsc. DC SOP and DC EOP were running. Parameters are:
DC SOP discharge pressure:- 6.75 Kgsc
Seal Oil DP:- 1.40/1.42 Kgsc
Lube Oil Press:- 0.72 Kgsc
H2 Press.:- 3.22 Kgsc

4) 10:36:37hrs: AOP-1 started but it did not contribute so pump stopped/tripped. AC SOP-2 also
stopped. AC SOP-1 and DC EOP were running. Parameters are:
DC SOP discharge pressure:- 4.56 Kgsc
Seal Oil DP:- 0.28/0.42 Kgsc
Lube Oil Press:- 0.68 Kgsc
H2 Press.:- 3.22 Kgsc

5) 10:38:54hrs: AOP-1 and AC SOP-2 started. DC SOP and DC EOP were running. Situation
normalised. Parameters are:
DC SOP discharge pressure:- 9.21 Kgsc
Seal Oil DP:- 3.96/3.85 Kgsc
Lube Oil Press:- 2.84 Kgsc
H2 Press.:- 2.99 Kgsc

6) 03:38hrs: In night shift, CPR passing suspected so CPR MIV closed for checking same. After
closing MIV, CPR position changed from CLOSE to OPEN. SO dich. pr. and DP fluctuation
observed during this.

OTHER OBSERVATIONS:
1. CPR passing observed.
2. H2 pressure dropped from 3.22Kgsc to 2.99Kgsc in 3 minutes.
3. ALL SOPs (AC & DC) NRV passing checked by pump changeover. Found normal.
