AFFIDAVIT
I, RACHEL A. TEE, of legal age, Filipino, widow and a resident of Punta
Bato, Barangay Tangub, Bacolod City, Negros Occidental, Philippines, after
having been duly sworn in accordance with law, hereby depose and say:

1. That I am the widow of the late ANTONIO TEE who died on May 21, 1990
at the Doctor’s Hospital Inc., North Drive, Bacolod City, (a copy of his Death
Certificate is hereto attached as Annex “A”);

2. That my late husband had an odd and peculiar habit of using varied
middle names in filling-up official documents and government records;

3. That in his Naturalization Papers, our Marriage Contract, the Certificate

of Live Birth of our children and the Alien Certification of one of our
children, he filled-up varied middle names and appellations thereto;

4. That in our Marriage Contract, he used ANTONIO TEE ENG (a copy of our
Marriage Contract is hereto attached as ANNEX “B”);

5. That in his Certificate of Naturalization, he used the names ANTONIO TEE

ENG, ANTONIO TEE JAO and ANTONIO TEE to refer to his person (copies
of his Certificate of Naturalization and the Certification of the Solicitor
General are hereto attached as ANNEX “C-1” and “C-2”, respectively)

6. That in the Certificate of Live Birth of our son NELSON A. TEE, he used
ANTONIO TEE as his name (a copy of the Certificate of Live Birth of NELSON
A. TEE is hereto attached as ANNEX “D”);

7. That in the Certificate of Live Birth of our son ARNEL TEE, he used
ANTONIO TEE, SR. as his name (a copy of the Certificate of Live Birth of
ARNEL TEE is hereto attached as ANNEX “E”);

8. That in the Certificate of Live Birth of our son ANTONIO A. TEE JR., he
used ANTONIO TIU TEE SR. as his name (a copy of the Certificate of Live
Birth of ANTONIO A. TEE JR. is hereto attached as ANNEX “F”);

9. That in the Certificate of Live Birth of our daughter NANCY A. TEE, he

used ANTONIO YOUNG TEE as his name (a copy of the Certificate of Live
Birth of NANCY A. TEE is hereto attached as ANNEX “G”);

10. That in the Alien Certificate of Registration and Certificate of Live Birth of
our daughter SANDRA TEE, he used ANTONIO TEE ENG and ANTONIO
YOUNG TEE, respectively, as his name (a copy of the Alien Certificate of
Registration and Certificate of Live Birth of SANDRA TEE are hereto
attached as ANNEX “H” and “I”);
 11. That when my late husband died in 1990, in our moment of sorrow, I
wrote “YOUNG” as his middle name in his Death Certificate to conform to
his Middle Name as appearing in our children’s Certificate of Live Birth”’

12. That when I recently filed a Declaration of Heirship, I used “ENG” as his
middle name to conform to his name as appearing in our Marriage
Contract;

13. That as his wife, advanced in years, I too was confused, was and am
constantly in quandary as to what name should I refer to in ascertaining
the identity of my late husband;

14. That the names ANTONIO TEE, ANTONIO TEE ENG, ANTONIO YOUNG
TEE and ANTONIO TIU TEE refers to one and the same person, that is,
my late husband ANTONIO TEE, the latter being his true and correct
name;

15. That the discrepancy, insofar as his middle name and appellation is
concerned as appearing in our official documents and government
records, were probably brought about by excusable neglect without any
intention to prejudice or defraud any individual or institution, whether
public or private;

16. That I declare further that my daughter SANDRA TEE died on May 27,
1965 when she was just four (4) years of age;

17. That I declare further that my son NELSON TEE died on October 21,
2017, leaving NELARMI P. TEE as his sole and compulsory heir;

18. That I am executing this affidavit to attest to the truth of the foregoing
statement and for whatever legal purpose it may serve.
