JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, KRISTINE A. DELA CRUZ , of legal age, Filipino, married and

presently residing at Gundaway, Cabarroguis, Quirino and FAYE MARIE A.
SACALAMITAO , of legal age, Filipino, married and presently residing at Ricarte
Norte, Diffun, Quirino both after being duly sworn to in accordance with law, depose
and state that:

1. We personally know RHODETTE IVY B. ANCHETA of Aurora East,

Diffun, Quirino, being acquaintance for a long period of time;
2. We declare, confirm and affirm that she was born at Emmanuel
Community Hospital, Inc., located at 2449 Jose Abad Santos, Tondo
Manila, Philippines, which is within the jurisdiction of Barangay 216,
Zone 20, District II;
3. That as per certification of Brgy Chairman, ALFREDO SAMSON OF
Brgy 216-zone-20 DISTRICT II, that said hospital was well known,
however due to circumstances unknown to the Barangay Officials, said
hospital has already closed, replaced by MMDA GWAPOTEL and is also
an abandoned building up to this time, which copy of said certification is
hereto attached as Annex “A”;
4. However, she has been using Karuhatan Valenzuela, Metro Manila as her
Place of Birth because she was made to believe by her mother, that it is the
correct address of EMMANUEL COMMUNITY HOSPITAL, INC. as
indicated in her birth certificate, which hereto attached as Annex “B”;
5. As such, she has been using Karuhatan Valenzuela, Metro Manila in all
her official documents, such as reflected in her Official Transcript of
Records (OTR), which hereto attached as Annex “C”;
6. That as per records, the correct Place of Birth of RHODETTE IVY B.
ANCHETA is in JOSE ABAD SANTOS, TONDO, MANILA;
7. We are not related by affinity or consanguinity to the above mentioned
person;
8. We are executing this affidavit to confirm and affirm the foregoing facts
and for whatever legal purpose it may serve.
 IN WITNESS WHEREOF, WE have hereunto set our hands this 10th day of
January at Diffun, Qurino, Philippines.
