LABORATORY INCIDENT REPORT

Location of Incident: Room G16, CHEMPRO Analytical Services Laboratories, Inc., 2nd
Floor MS Building San Roque St. Barangay Kapitolyo, Pasig City

Details of Incident: An employee was performing the first catalyst calcining since the lab
they worked in had been moved to a new location. The particular reaction carried out during
the incident was a “nickel nitrate decomposition.” The lab is located on the “High Bay
Platform” of room G16. This is an interior lab not exposed to the outside elements. The off-
gas of the calcining process was bubbled into a beaker of water set on the bench top beside
the reaction taking place. This bubbling process was used to capture the NOx that is released
from the nickel nitrate decomposition so that it was not released to the immediate breathing
zone of the employee. Typical protocol, provided to the employee via “word of mouth” prior
to the experiment, calls for the beaker to be placed in a fume hood while the catalyst
calcining takes place. However, on the day of the incident the beaker was on the lab bench
during the experiment. While calcining the catalyst, the employee noticed a distinct metallic
smell and realized he was being exposed to the NOx generated from the experiment. At this
time he noticed the beaker used to scrub the off gas had been turned over on the lab bench,
thus creating a direct release of NOx to the immediate environment. The employee then
verbally notified the other two employees working in the immediate area and they all
evacuated simultaneously.

What action was taken: The employee immediately notified his supervisor of the exposure.
The safety representative was notified the next day. A call was then placed to the health
services and the employee that detected odor was taken for medical evaluation and admitted
to hospital overnight for observation.

Causation: No written procedures.


Lack of training and understanding of basic safety precautions for the
procedure.

REFLECTION PAPER

A serious workplace injury or death changes lives forever – for families, friends,
communities, and coworkers too. Human loss and suffering is immeasurable. Occupational
injuries and illnesses can provoke major crises for the families in which they occur. In
addition to major financial burdens, they can impose substantial time demands on uninjured
family members.

Safety Management has taught me to value safety, welfare, health, and well-being. It
should be instilled in our minds that safety is a responsibility of every person who works for
the company to take reasonable care for the safety of himself and other human beings and the
surroundings which may be affected by his acts or omissions. It is important that we know
that every working environment poses danger that could potentially harm us.

This course made me realize that when I start working, being armed with the
knowledge you learned such as the simple precautionary measure, planning, and trainings
would make ne steps ahead of my coworkers. A safe attitude is essential because it will help
to guard against complacency, and create a productive working environment for all, so it is
crucial to be aware of the risks to health and safety before an accident happens, and to take
steps to remove or control those risks. Being prepared to deal calmly with unexpected or
unusual situation and reporting any potentially dangerous work situations are a basic
requirement of safety.

It is important to constantly look at the safety processes to make sure they are the best
they can be. Businesses that achieve lower injury rates have a working health and safety
program in place. They have made safe operation a business objective and these businesses
have also a more profitable bottom-line. It is very important that all employers provide safe
and healthy conditions in which their employees can work effectively without any hazard or
risk. Also, employers and employees should remember that observation, learning, and
experience are the keys to recognizing potential safety hazards in the workplace.

Studying safety management is a great way to branch out your career and take another
step toward a more successful future. Time and again, companies that put safety first turn out
higher quality products. In some cases, that’s because a safe workplace tends to be a more
efficient one, free of debris and tangles of cords. In other cases, it’s a matter of focus. By
working in a clean, efficient environment, workers are able to reduce distractions and truly
focus on the quality of what they do. The results? Better products that create customer
loyalty, bigger margins and increased sales. In these ways and others workplace safety is
about much more than legislation. It’s about creating the kind of productive, efficient, happy
and inspiring workplace we all want to be part of. It’s about creating a highly profitable
company. And that’s why it’s important.
