KEPALA UPTD PUSKESMAS SINGOJURUH

Menimbang : a. bahwa dalam upaya untuk menjaga kualitas pelayanan

dan pelaksanaan program di UPTD Puskesmas Singojuruh;
b. bahwa untuk mewujudkan hal tersebut, dapat dicapai
dengan mengadakan layanan Klinis di UPTD Puskesmas
Singojuruh;
c. bahwa perlu ditetapkan Layanan klinis di UPTD Puskesmas
Singojuruh.

Mengingat : 1. UU RI Nomer 29 Tahun 2004 Tentang Praktik Kedokteran;

2. UU RI Nomer 38 Tahun 2014 Tentang Keperawatan;

Menetapkan : KEPUTUSANKEPALA UPTD PUSKESMAS SINGOJURUH

TENTANG LAYANAN KLINIS DI UPTD PUSKESMAS
SINGOJURUH.
KESATU : Menetapkandaftarpenyakit non spesialistik yang dapatditangani di
puskesmasSingojuruh.
KEDUA : Menetapkan daftar sepuluh besar penyakit di Poli Umum, Unit
Gawat Darurat dan rawat inap,daftarpelayanan yang
bisaditangani di Poli Gigi, sebagaimana tercantum dalam
lampiran merupakan bagian yang tidak terpisah dari surat
keputusan ini.
KETIGA : Surat Keputusan ini berlaku sejak tanggal ditetapkan dengan
ketentuan apabila dikemudian hari terdapat perubahan dalam
penetapan ini akan diadakan perbaikan sebagaimana mestinya.
