AFFIDAVIT OF COHABITATION
I, EDEN BANDOJO VENTURA, of legal age, Filipino, single and a
resident of B17 L10 Ph.2 Peace Village Brgy. San Luis, Antipolo City, after having been
duly sworn to in accordance with law, depose and state that:

1. I and my common law wife had cohabited for twenty one (21) years in
Brgy. San Luis Antipolo;

2. Unfortunately, my common law husband Ronald E. Sacasan died last

January 31, 2013;

3. It appears in MERALCO record that my late live-in partner is the

sebraber in the meter installation for our residence;

4. Only lately the electric meter got burned resulting to its destruction,
hence I applied for a new meter from MERALCO.

5. I am executing this affidavit to proved that I am the common law wife

of the deceased Ronald Sacasan and to attest the veracity herewith I
farther for the new meter application for MERALCO.

AFFIANT FURTHER SAYETH NAUGHT.
