PENATALAKSANAAN HOLISTIK DAN KOMPREHENSIF PADA PASIEN LAKI-LAKI

DENGAN GOUT ARTHRITIS DAN HEMIPARESIS DEKSTRA PASKA-STROKE

Abstrak

Latar Belakang: Gout arthritis adalah penyakit dengan manifestasi bengkak dan nyeri pada salah
satu sendi yang merupakan sebab dari terdeposisinya kristal monosodium urat di jaringan.
Hipertensi merupakan salah satu faktor yang berperan dalam penyakit tersebut. Hipertensi perlu
mendapatkan terapi yang tepat untuk menghindari komplikasi stroke. Pasien stroke sebagian besar
berakhir dengan cacat. Kecacatan yang terjadi dapat diterapi dengan rehabilitasi. Proses rehabilitasi
terbaik adalah dengan latihan fisik mandiri dan rutin di rumah.

Tujuan: Penerapan prinsip pelayanan dokter keluarga pada pasien dengan mengidentifikasi faktor
risiko, masalah klinis, serta penatalaksanaan pasien dengan pendekatan patient centered dan family
focused, dan community oriented.

Metode: Analisis studi adalah laporan kasus. Data primer diperoleh melalui anamnesis,
pemeriksaan fisik dan kunjungan rumah. Penilaian dilakukan berdasarkan diagnosis holistik yang
dilakukan secara kuantitatif dan kualitatif.

Hasil: Pasien berada pada derajat fungsional 3 dengan gout arthritis dan hemiparesis dekstra paska-
stroke, memiliki faktor internal yaitu jenis kelamin laki-laki, usia 42 tahun, darah tinggi, kurang
pengetahuan tentang penyakit, stressor sebagai kepala keluarga dan pola pengobatan kuratif.
Dilakukan penatalaksanaan holistik terhadap pasien dan keluarganya tentang upaya melakukan
latihan fisik mandiri di rumah, mengonsumsi obat dan kontrol secara teratur serta cara
mengendalikan stres bagi pasien. Dilakukan evaluasi untuk menilai perubahan yang terjadi dari
sebelum dilakukan intervensi. Pada evaluasi didapatkan keluhan telah membaik.

Kesimpulan: Peran keluarga amat penting dalam perawatan dan pengobatan anggota keluarga yang
sakit. Dalam melakukan intervensi terhadap pasien tidak hanya memandang dalam hal klinis tetapi
juga terhadap psikososial dan lingkungan, sehingga diperlukan pemeriksaan dan penanganan yang
holistik, komperhensif dan berkesinambungan.

 HOLISTIC AND COMPREHENSIVE MANAGEMENT TO A MAN WITH GOUT
ARTHRITIS AND HEMIPARESE DEXTRA POST-STROKE

Abstract

Background: Gout arthritis is a disease that manifests by swelling and pain in unilateral joint that
caused by the deposition of monosodium urate crystals in the tissues. Hypertension is one of the
factors that have a role in gout arthritis. Hypertension needs to be treated well to prevent
complications like stroke. Mostly, stroke patients will end up with disabilities. Disabilities that
occurs can be reduce with rehabilitation therapy. The best rehabilitation process is independent
physical exercise at home.

Objective: To applicate the principle of family doctor to patient by identifying risk factors, clinical
problems, and managing patients with a patient centered and family focused approach, and
community oriented principles.

Method: This study is a case report. Primary data obtained through history taking, physical
examination and home visits. Assessment is based on a quantitatively and qualitatively holistic
diagnosis.

Results: Patient is in 3rd functional level with gout arthritis and right hemiparesis post-stroke, having
internal factors such as gender is men, 42 years of age, high blood pressure, lack of knowledge about
the disease, stressors as a head of family and health behavior is curative seeking. Holistic
management to the patient and his families were an efforts to do independent physical exercise at
home, to take medicine and control regularly and educate how to do stress control for patient. An
evaluation was conducted to assess changes that occur from before intervention. In the evaluation,
complaints were improved.

Conclusion: The role of family is very important in treatment of sick family members. To give a
best therapy for patients we have to see it not only in terms of clinical but also psychosocial and
environmental aspects, so we can give a holistic comprehensive and continuous examination and
treatment.
