INTRODUCTION
Stem Cell is the latest methodology in medical treatment that can not be easily
solved by the conventional method. Generally, the stem cell has a beneficial
treatment for cancer, auto immune and degenerative disease. Therefore, this
conference is important to be discussed.
Over the years, the number of cancer patients is rapidly increased. We need to
overcome this accurately. Faculty of Medicine, Universitas Sumatera Utara
collaborated PUI Stem Cell USU holds an International conference. The
conference will discuss the basic stem cell science and its application. Apart
from that, it will also discuss the basic oncology, General medical science,
diagnosis, treatment and prognosis as well as all the psychological and social
aspects of oncology.
The conference will present the speakers who are expert in their field of health.
All manuscript will be selected by CRC Press (Scopus Indexed) for proceeding
and also in Sumatera Medical Journal (SUMEJ).
The conference will be held continuously every year. It will be equipped by the
workshop. We provide a live streaming show during the conference. We expect
the number of journal from lecturers, students, and participants will bring
benefit for academics and practitioners in the field of health in Indonesia.

The risk of Albuminuria to the ischemic acute patient with or
without metabolic syndrome

Abstract
Stroke is not only a major cause of death, but also a leading cause of disability worldwide.
Metabolic syndrome is one of the predictor of stroke since it has been linked to the
development of atherosclerosis that will lead to cerebrovascular event. Albuminuria is known
to be an indicator of endothelial dysfunction that is a major cause of cerebrovascular disease.
In this study, 50 subjects of acute ischemic stroke consist of 25 subjects of acute ischemic
stroke with metabolic syndrome and 25 subjects of acute ischemic stroke without metabolic
syndrome. The ischemic stroke with metabolic syndrome population is significantly
associated with increased risk of albuminuria 16 times higher than the ischemic stroke
without metabolic syndrome (OR = 16, IK95% 1.855 – 137.97, p=0,002). Subjects of
ischemic stroke with metabolic syndrome is significantly associated with increased risk of
albuminuria.

In advent of dynamic and better resolution of camera systems, endoscopic approach is more
acceptably popular than conventional methods. Thus, surgical intervention on maxillary
pathology is shifted intoless invasive and low morbidity with better disease surveillance.

Abstract
The aim of this study is to evaluate the response of cervical cancer stage IB – IIB after
neoadjuvant chemotherapy based on VEGF. Material and method: data were collected from
51 paraffin blocks patient cervical cancer stage IB – IIA who received chemotherapy
ifosfamide – ciplatin prior hysterectomy radical at Adam Malik Hospital Medan. VEGF
expression evaluated from cervical biopsy tissue, and response therapy evaluated base on
tumor size clinically. 20 out of 51 with complete clinical response, while the rest are the
partial response. 18 out of 51 complete clinically response with negative or weak VEGF
expression, and 31 out of 51 partial response with moderate or strong VEGF expression. 23
cases with tumor size > 4 cm and 23 cases stage IIA expressed VEGF moderately or strongly.
Chemotherapy neoadjuvant ifosfamide- cisplatin has the good response for cervical cancer
with tumor size < 4 cm and stage IB where less expressed VEGF.

Correlation of SGA With RMR And FFM Measured By BIA
In Non- Hodgkin Lymphoma

Abstract
SGA is used to assess malnutrition in simple way without a complete body composition
analysis. Increased metabolism characterized by increased RMR is often found in patients
with cancer. The aim are to know the correlation between SGA with RMR and FFM in
patients with NHL. Correlation between RMR and FFM measured by BIA and SGA then
assessed and analyzed. 27 NHL patients, consisting of 59.3% men and 40.7% . Well
nourished were found in 7.4% of the subjects; 63% were found to be malnourished and
29.6% were severely malnourished. The mean RMR and FFM were 1346.3 ± 145.7 kcal; and
44.5 ± 6.8 kg consecutively. Only 3% patient was found to have RMR above normal. No
correlation was found between SGA with RMR (p = 0.275) and FFM (p = 0.850). There was
no correlation between SGA and RMR nor FFM measured by BIA in NHL.

The Effect of Poguntano on Superoxide Dismutase in Diabetic

Abstract
Diabetes mellitus is one of the most common non communicable diseases worldwide.
Diabetic chronic complications arising from macro and micro vascular changes. Decreased
activity of antioxidant such as superoxide dismutase may increase the susceptibility of
diabetic patients. This is an experiment study using alloxan diabetic rats and extract ethanol
of Poguntano. Superoxide dismutase showed statistical significant between group with
normal rats and diabetic control rats (p<0.001), diabetic control with group giving insulin
(p<0.00) and diabetic control with group giving Poguntano 200 mg (p<0.00), but did not
showed significant results in the other groups. Extract ethanol of Poguntano showed
promising results in lowering blood glucose in alloxan induced diabetic rats, and do prevent
the stress oxidatives.

Correlation of BDNF and Cognitive Function in Smoking

Abstract
Schizophrenia is a complex neurodevelopmental disorder with impaired cognitive function as
a major part. BDNF may be a marker of abnormal neuronal development and
neurotransmission in schizophrenia. Cognitive often correlates significantly with awareness
of pain, social skills and the presence of delayed return to normal life. The purpose of this
study was to look at the association of serum levels of BDNF and cognitive function in Batak
schizophrenic patients with smoking male using a history of risperidone treatment. This study
was a cross-sectional correlative numerical study. To assess cognitive function using the
Montreal Assessment Cognitive Instrument Indonesia version. BDNF serum levels were
analyzed using The quantitative sandwich enzyme immunoassay.There was a significant
correlation between Moca-Ina Score and serum BDNF level with correlation value of 0.441
showed a positive correlation with moderate correlation strength (r = 0.4 - <0.6).

Abstract
Nasopharyngeal carcinoma (NPC) is one of the most common malignancy in Indonesia with
over 13.000 new cases reported yearly, especially in male. TNF-α serum is reported as a new
biomarker to predict bone inva-sion, post-therapy distant metastasis, and poor life
sustainability in NPC. To identify the expression of TNF-α in regards to clinical stage of
NPC. This is an analytic study with 126 NPC samples of patients in Haji Adam Malik
General Hospital. NPC mostly found in age group 41-60 years (57.1%), male (71.4%),
non-keratinizing SCC (79.4%), and stage IV group (54.8%). In immunohistochemistry
evaluation, most of TNF-α overex-pressed in non-keratinizing SCC (68.0%), T3-T4 (50.8%),
N2-N3 (62.7%) and clinical stage III-IV (69.9%). Spearman’s test for categorical corelation
yield p value of <0,001. There is a significant correlation between TNF-α expression and the
stage of NPC.

Abstract
To determine the expression of microvessel density in primary tumor size (T), lymph node
(N), and clinical stage in nasopharyngeal carcinoma. This descriptive study involved 30
samples in Haji Adam Malik General Hospital Medan. Nasopharyngeal carcinoma mostly
found in male (73,3%); 41-60 years old (60,0%); T4 primary tumor (36,7%), N3 lymph
node(43,3%), and non-keratinizing squamous cell carcinoma (53,3%). High
microvesseldensity (>45 MV/LP) was found in 56.7%; non-keratinizing squamous cell
carcinoma (58.8%), T4 primary tumor (35.3%), N3 lymph node of (47,1%); and stage 4
(76,4%). Microvessel density most overexpressed in non keratinizing squamous cell
carcinoma, primary tumor size (T4), lymph node size (N3) and clinical stage 4.

Abstract
Nasopharyngeal carcinoma is one of the most dominant cancer in Southern China and
SouthEast Asia. The prevalence of NPC in Indonesia is 3.9 per 100,000 population every
year. Cyclooxygenase (COX) is an en-zyme in biosynthetic pathways of prostaglandins (PG),
thromboxane and prostacyclin of arachidonic acid. Expression of COX-2 increase in early
stages of carcinogenesis, during development and invasive growth of tumors. To know the
expression of COX-2 in nasopharyngeal carcinoma patients. This is a descriptive study with
30 NPC samples of patients in Haji Adam Malik General Hospital Medan. NPC mostly found
in group 41-60 years (60.0%), males (73.3%), and non-keratinizing squamous cell carcinoma
(53.3%). Positive COX-2 expression in non-keratinizing squamous cell carcinoma (54.5%),
T3-T4 primary tumor size (68.2%), neck en-largement of N2-N3 (68.3%) and clinical stage
of III-IV (90.9%). There is increasing of COX-2 expression in advance stage of
nasopharyngeal carcinoma.

Transfusion of blood components in a stem cell transplant

Abstract
Heavily treated patient in oncology need transfusions of cellular blood components on a
regular basis. Plate-lets as well as units of erythrocytes need to be ABO compatible to the
recipient. In platelet transfusions, RhD compatibility can be discussed. It is preferred that all
cellular blood components are leukoreduced, and for pa-tients in a stem cell transplant
programme also gamma irradiated to prevent for transfusion associated graft versus host
disease.

The Effect of andaliman fruit extract to blood glucose levels

Abstract

Diabetes mellitus (DM) is a disease which treatment is expensive, therefore many researcher
try to find a cheap and effective treatment. The aims of this study is to evaluate the effect of
andaliman fruit extract to blood glucose level of mice with type 1 diabetes. The study was
conducted on 25 male mice. The mice were divided into 5 goups. Every group were injected
with alloxan intraperitoneally and treated with aquades, an-daliman extract and metformin for
15 days straight. The result showed that administration of andaliman ex-tract with the dose of
100, 200 and 300 mg/kg decreased the blood glucose levels significantly compared to the
negative control group (p≤0,05). Andaliman extract of 100 and 200 mg/kg were not
significantly different with the positive control group (p≥0,05). Blood glucose levels of mice
with diabetes mellitus type 1 were de-creased in the giving of andaliman extract.

Correlation of Plasma Cystatin C Levels and Renal
Dysfunction in Preeclampsia

Abstract
To determine correlation of plasma Cystatin C levels and the incidence of renal dysfunction
in preeclampsia. Mean urea levels in the group with renal dysfunction were 34,13 ± 21,29 mg
/ dL higher than the group without renal dysfunction by 27,55 ± 12,48 mg / dL, was no
significant significant difference (p > 0,05). Mean Cystatin C levels in the group with renal
dysfunction were 1,52 ± 0,37 mg / L higher than the group without renal dysfunction of 1,17
± 0,12 mg / L, was a significant difference (p < 0,05). Cystatin C levels in preeclampsia with
renal dysfunction were higher than those without renal dysfunction and were statistically
significant (p value = 0,003) and mean urea levels were not statistically significant (p value =
0,37).

Association Birth Weight and Body Mass Index with
Cognitive Function at Singkuang

Abstract
Children with low birth weight (LBW) is generally increased incidence of neurological
deficits or cognitive impairment during childhood compared with a normal birth weight.
Otherwise, that can affect cognitive function is a body mass index (BMI). The purpose of this
study was to determine the relationship between birth weight and BMI and cognitive function
in children in primary school at Singkuang. A cross-sectional study was conducted in primary
school at Singkuang. The assessment of a child's cognitive function with the Mini Mental
State Examination (MMSE). The results of this study indicate significant association between
birth weight and cognitive function (P = 0.0001). There is no relationship between BMI and
cognitive function in children in primary school at Singkuang (P = 0.356). There was a
significant association between birth weight and cognitive function, and no association
between BMI and cognitive function in children.

Comparison Of Thyromental Height Test And Acromio-

Abstract
Thyromental Height Test (TMHT) is a new examination method which is recommended and
has been reported to show a high success rate in predicting the difficult airway compared
with AASI. To compare the accuration of TMHT with AASI predict difficulty in intubation
in patients undergoing general anesthesia. Cross-Sectional diagnostic test were performed,
101 patients were assessed for TMHT and AASI. Cormack-Lehane classification were
analysed based on TMHT and AASI. TMHT (95,05%) and AASI of (86,14%) are easy to
intubate. TMHT : AASI analysis using ROC obtained that the (AUC) is 96% : 87.0%,
sensitivity 99% : 99%, specificity 96% : 87%, accuracy 96,04% : 87.1% .(95% CI: 99,2% -
99,8%) : (95% CI: 80, 4% - 93.6%). Based on AUC, TMHT scoring system is more accurate
as predictor for difficult intubation than AASI.

Equianalgesic Oxycodone, Continuous Intravenous Morphine
in Post- Long Bone Surgery

Abstract
Oxycodone and morphine are powerful analgesic drugs used for post-operative pain
management. There is no studies have been conducted to assess the dose equivalency
between the two drugs. To get an equianalgesic dose of Oxycodone and Morphine. This study
was using a double-blind randomized clinical trials, 48 samples, ages 21-60 years, PS ASA
I-II, which will get an elective orthopedic surgery using general anesthesia tech-niques.
Oxycodone (group A) : Morphine (group B) (Initial 5mg than continuous 1 mg/hour : initial
dose 4mg than continuous 0,5 mg/hour). Average drug dose for group that takes morphine
and oxycodone (3.90 ± 0.46 mg : 1.46 ± 0.51 mg). No significant difference between the
average dose (p <0.05). Equianalgesic dosein group Oxycodone and Morphine is 1.4 mg:
3.9mg.

Abstract
To determine the difference in total serum IgE level among subjects with and without ICM
AR on computed tomography (CT) scan examination. To observe difference in total serum
IgE levels in subjects undergoing ICM adverse reaction based on the degree of severity. 104
subjects undergoing CT scans with ICM, examined IgE levels before and after ICM was
administered, then measured the severity of adverse reactions of contrast media. In this
study, subjects undergoing adverse reactions, prior to administration Iopamidol had a higher
mean IgE 1270.79 mg / dl compared with those who having AR 1174.90 mg / dl and after
administration of Iopamidol a mean of Ig E is still higher in subjects having AR 1507.96
mg/dl compared with those having negative AR that is 1325.88 mg/dl, p = 0.696, statistically,
not significant. Increase in total IgE levels in subjects with positive adverse reactions are
independent (p=0.696).

Abstract
Triple negative breast cancers are types of breast cancer with lack or no expression of
estrogen receptor, pro-gesterone receptor, and human epidermal growth factor receptor 2.
This crosssectional study was performed on patients with breast cancer. The aim is
determine clinicopathology features of TNBC through immuno-histochemistry staining. 60
TNBC samples with range of age 27- 73 years old. Most of them were in the range of age 51-
60 years old (19 cases, 31.66%) and pre-menopause age(34 cases, 57%). Tumour size T3 and
T4 with staging IIIA and IIIB, histology sub- type IC-NOS and ILC with grade 2 and grade 3
were common in TNBC. Due to its characteristics found in all ages, particularly in pre-
menopause, tumour size >5cm with me-tastases to lymph nodes, histopatology diversity
accompanied with high grade of histology, TNBC needs spe cial attention in order to predict
the prognosis and further therapy.

Relationship Between Procalcitonin and High Density
Lipoprotein in Bacterial Sepsis

Abstract
Sepsis can decrease the anti-inflammation and immunomodulator effects of High-Density
Lipoprotein (HDL). Low HDL levels inversely correlate with the severity of sepsis and
associate with an exaggerated systemic inflammatory response. Bacterial infection is a strong
stimulus for PCT. To determine the correlation between PCT and HDL level in bacterial
sepsis. An observational study with case control design in 30 sepsis and 30 non-sepsis
infection patients.Data collected to fill out PCT and HDL level on the 1st,3rd and 5th day and
then analyzed to assess their relationship. Statistically, shows a significant difference in PCT
and HDL level between sepsis and non-sepsis group.There was a negative correlation
between HDL and PCT, and a negative correlation between HDL level and the severity of
sepsis, which show a medium strength correlation.Sepsis can be estimated when the HDL
value less than 17,50mg/dl. There was a significant relationship between PCT and HDL level
in bacterial sepsis.

The comparison of ERα expression between endometriosis
with no endometriosis

Abstract
Estrogen receptors (ER) play an important role in mediating action. The ER α has a higher
affinity for estrogen and the dominant form of the normal endometrium. A cross-sectional
study from an ectopic tissue of endometriosis and normal was examined for
immunohistochemistry. This research was conducted from November 2015 until the sample
complete. The analysis was performed using Fisher Exact test, p <0.05 was considered to any
difference of estrogen alpha receptor expression between endometriosis patients with no
endometriosis. REα is more dominant in normal endometrium, whereas in the case of
endometriosis in the presence of many negative expression of REα then the assumption that
REβ is a receptor that many encountered. There is a difference in the expression of REα
between endometriosis with no endometriosis.

Abstract
This study aims to test the association between family support and coping strategies and
anxiety at Dr. Pirngadi General Hospital Medan. The samples are 102 cancer patients
undergoing chemotherapy were assessed and the results of the bivariate analysis showed a
significant positive association (p=0.001<0.05) and strong correlation (r=0.612) between
family support and Problem Focused Coping (PFC) strategy, while there was a significant
negative association (p=0.001) and moderate correlation (r=-0.462) with Emotion Focused
Coping (EFC) strategy. A significant negative association (p=0.001) and strong correlation
(r=-0.646) was found between family support and anxiety. The multivariate analysis showed
a dominant association (p = 0.001) between family support and PFC strategy (OR = 12.2),
EFC (OR = 0.142), and anxiety (OR = 0.039). The conclusions was that there is an
association between good family support and effective coping strategies and lower anxiety
levels in cancer patients undergoing chemotherapy.

Abstract
Previous studies have not been able to certainly show the affect of vitamin D supplementation
in tuberculosis patients. The objective of this study determined whether vitamin D
supplementation to patients with tuberculosis could influence 25(OH)D and calcium serum
level. Results: after 28 days, the vitamin D supplementation showed significant increased of
25(OH)D serum level at the end point (p=0.001), but not with calcium serum level (p=0.3).
Conclusions: supplementation with 1000 IU vitamin D per day increased 25(OH)D serum
level but no association with calcium serum level. These results suggest a.

Abstract
Cryptosporidium sp. is a common intestinal parasite infected immunocompromised patients.
This organism causes chronic diarrhea. Infection related to ingestion oocyst-contaminated
drinking water or food. For immunocompromised individuals, this infection could have a
higher mortality and vary in clinical manifestation. We report three patients suffered from
chronic diarrhea caused by Cryptosporidium sp. aged 33-40 years. These patients were
diagnosed as AIDS with very low CD4 count (< 5%). The inadequacy of anti-retroviral cause
progressively decrease CD4 count according to host factors, it explains Cryptosporidium sp.
is still a burden for AIDS patient, especially with very low CD4 count. There was no
medication given to this patient including paromomycin or nitazoxanide, continuing anti-
retroviral therapy was the only medication given to patients. Diarrhea frequency decreased
after 8-10 days administration to hospital and discharge was done to these patients.

Abstract
Intestinal parasitic infections has been admitted as a disease associated with low
socioeconomic status. Eosinophil has mostly known involved in the immune system. This
study aimed to determine the prevalence of intestinal parasite infection and its correlation to
eosinophil levels among Primary School Children. A cross-sectional was conducted in
Amplas Primary School Medan and Hamparan Perak, Deli Serdang. We carried out a
population of 132 children. Direct fecal examination method was used to determine the
species of infection and eosinophil level by diftel count. Finally, the data were analyzed by
using chi-square test. The overall prevalence of IPIs in this school was 26,5%, Protozoa
infection was a significant cause of disease in this school, (18,9%.) The most prevalent
species is Entamoeba coli (6,1%) followed by Giardia lamblia (4,5%), and Ascaris
lumbricoides (3%). There was a significant correlation between Hypereosinophilia and
parasite intestinal infection prevalence (p=0,021), especially protozoa infections (p=0,005).

Correlation of Depression Symptoms and IL-6 In CKD With
Regular Hemodialysis Patients

Abstract

Depression is common in patients with chronic kidney disease (CKD) undergoing


hemodialysis (HD). Associ-ated with chronic inflammation, that characterized by elevated
levels of plasma cytokines, especially interleu-kin (IL)-6. Investigate the correlation of
depressive symptoms with IL-6 levels in patients with CKD who un-derwent regular HD.
Total of 47 CKD patients with depression were enrolled in this cross-sectional study. Level
of IL-6 was then correlated with Beck Depression Inventory (BDI) II score using Spearman
test. The mean BDI score was 19,32 ± 6,18 indicating the level of mild depression symptoms,
and the mean IL-6 level (8,697± 6,947 pg/mL) was still within normal range. There was a
moderate positive correlation (r= 0.417) be-tween the level of depressive symptoms and IL-6
levels. There was a moderate positive correlation (r = 0.417) between depressive symptoms
and IL-6 levels in the patient with regular HD.

Olfactory Functions Difference on Gas Station Workers

Abstract
Chronic exposure of volatile substances can cause the impaired olfactory function. To
identify the difference of olfactory function at Gas Station workers in Medan city. Cross-
sectional analitic study. A total of 80 per-sons consisting of 40 gas station operators and 40
non-operators who fulfilled the inclusion criteria. The ap-plied statistical analysis was Mann
Whitney, Fisher's Exact, Chi Square, T independent, Spearman with 95% confidence level.
Proportion of the impaired olfactory function in the gas station operator group by 77.5% and
non-operator group by 0%. Significant correlation was obtained between age, duration of
work with the im-paired olfactory function (p = 0,047, p = 0,001). The gas station operators
will suffer the impaired olfactory function if they works for ≥ 3.89 years. There were
differences of olfactory functions on the gas station work-ers by using Sniffin 'Sticks Test.

Apoptotic index fails to predict chemoresponse in breast
cancer

Abstract
The responsiveness of neoadjuvant chemotherapy in triple negative breast cancer (TNBC)
needs determination to prevent overtreatment with chemoresistance regimen. A total of 60
consented patients from Haji Adam Malik and Bunda Thamrin Hospital were included in this
cohort prospectie study. Patients with heart, kidney, liver disease, history of surgery,
chemoterapy, or hormonal therapy were excluded. Apoptotic indexes were taken from all
subjects before and after neoadjuvant chemotherapy. After 3 cycles of neoadjuvant
chemother-apy, 31 subjects (51.7%) did not show clinical response while 29 subjects (48.3%)
had clinical response. There was no significant difference of apoptotic index before and after
neoadjuvant chemotherapy (5.47+1.38 vs 5.52+1.08 pg/mL). There was also no significant
relationship between apoptosis index and clinical response (p=0.993). This study showed that
apoptotic index fails to be neoadjuvant chemotherapy response predictor in triple negative
breast cancer. Further research with larger samples is needed to confirm this result.

Abstract

Cigarette smoke contains free radicals. The condition can be decreasing of antioxidants. This
study aims to determine the effect of cigarette smoking on superoxide dismutase antioxidant
(SOD) levels in diabetes mellitus (DM) with pulmonary tuberculosis (TB) patients.The study
was conducted on 30 male subjects DM with pulmonary TB patients. Smoking data was
obtained by questionnaire. SOD level was determined according to ELISA technique. The
mean value of SOD were analyzed by SPSS programme. There was no significantly (p =
0.56) in SOD levels of DM with pulmonary TB patients on nonsmoker groups compared to
smoker groups. Level of SOD has significant differences (p = 0.000) in smoker subjets by
Brikman index classificated. Cigarette smoking can effect on SOD level in DM with
pulmonary TB patients depend on length of time and the number of cigarettes per day that is
taken by a person

Abstract

Physical activity and sedentary behaviorplays an important role in preventing the progression
of obesity in adolescence to adulthood. This study was conducted to determine association of
physical activity and sitting time (sedentary behavior) with body mass index (BMI) in students
at the Faculty of Medicine, University of Sumatera Utara. One hundred and thirty students as
respondents in this study, selected based on inclusion and exclusion criteria, completed
questionnaires of IPAQ. BMI measured using digital weight scales and microtoise. All data
were analyzed using SPSS program. Based on one-way ANOVA test analysis revealed that
there was a significant association (p=0.007) of physical activity toward the mean of BMI but
no relationship the sedentary behavior with BMI (p=0.546). The results suggest that BMI are
related with physical activity but no with sedentary behavior.

Pattern of fungal infections in chronic suppurative otitis media
(CSOM)

Abstract
Organisms that can be found in CSOM can be either aerobic, anaerobic, or a combination of
both or fungi. Chronic secretions and the use of irrational and excessive otic topical
antibiotics encourages the growth of fungal infections in CSOM. Aspergillus and Candida sp
were commonly fungi that found in CSOM. To know the pattern of fungal infections of the
patients with CSOM at the ORL-HNS Department, Faculty of Medicine, Universitas
Sumatera Utara / Haji Adam Malik General Hospital Medan. A descriptive study with case
series approach. positive fungal cultures as many as 4 specimens (13.33%) were found from
30 aural discharges. The type of fungi were Aspergillus sp. (75%) and Candida sp. (25%).
From the results conducted a positive fungal culture, so the fungal infection is still considered
on CSOM especially from the patients who do not respond to treatment.

Association between Hypertension and Health-Related
Quality of Life in Adolescent

Abstract
Hypertension prevalence in children, particularly school age, is increasing and is related to a
change in quality of life. To assess the relationship between hypertension and quality of life
in adolescence.A cross sectional study was conducted in 13 – 18 years old adolescents in
Singkuang, North Sumatera. Anthropometry and blood pressure measurements were done.
Quality of life was assessedusing the PedsQL questionnaire. From a total of 79 samples,
19 (24,1%) children were hypertensive and 60 (75.9%)were not. There were 67 (84.8%)
children with good and 12 (15.2%) with poor quality of life. There were 14 (17.7%)
hypertensive children with good and 5 (6.3%) with poor quality of life. A Chi-square test was
done with a p=0.121 (PR = 2.256 and CI 95% = 0.809-6.287) There is no relationship
between hypertension and quality of life in adolescents.

Far Lateral Approach for Foramen Magnum Meningioma

Abstract
Among the meningioma of the posterior fossa, foramen magnum (FM) meningioma deserves
special consideration because of their characteristics in symptomatology, intriguing surgical
anatomy, unique operative requirements, and outcome. When all meningioma are considered,
FM meningioma has the worst outcome in terms surgical results and operative morbidity. We
report a 53 years old female who diagnosed had foramen magnum meningioma and had
performed tumor removal surgery using far lateral approach. Finally, detailed technic
operation and classification system from vary literature will also explained.

The Difference of MMSE and MoCA-Ina scores in Brain

Abstract
The Mini Mental State Examination (MMSE) is the most commonly used screening test for
brain tumor (BT) patients but relatively insensitive compared to Montreal Cognitive
Assessment (MoCA). The aim of this study was to compare the proportion of the MMSE and
MoCA-Ina scores in BT patients. A cross sectional study was conducted in 33 patients with
BT. All of the subjects were assessed with MMSE and MoCA-Ina. To assess the difference in
the propotion of brain tumor patients was used chi-square test or Fisher exact test. In BT
patients with group of education 7 – 9 years presented a significantly higher percentage of
abnormal MMSE and MoCA-Ina score than other group. We noticed that over 90.9% was
impairment of attention and calculation using MMSE, while delayed recall have been
affected on more than 90.9% using MoCA-Ina.

Exclusive Breastfeeding For Infant Growth and Development

Abstract
Breast milk is one of the most perfect nutrition to ensure the growth and development of
infants in the first 6 months. Exclusive breastfeeding alleged effect on infant growth and
development. The purpose of this study was to analyze the influence of exclusive
breastfeeding on infant growth and development. This study is a case control study in Sering
Primary Health Care Medan in 2014. Anthropometric parameter of subjecs’ chil-dren were
measured and plotted in the WHO graph. Then, subjects were told to undergo a 5 minute
interview to analyze the pattern of breastfeeding history. In this study, both infant growth,
development, and growth and development were associated with history exclusive
breastfeeding (p<0.001). Exclusively breastfed in-fants have good growth and optimal, in
contrast to infants who are not breastfed exclusively have poor growth. It was concluded that
there was an influence of breastfeeding with infant growth and development.

Seizure & Mild Cognitive Impairment in Tuberous Sclerosis
Complex (TSC) : A Case Report

Abstract
Tuberous Sclerosis Complex (TSC) is syndrome characterized by hamartomal growth on
multiple organ system.Caused by genetic disturbance especially on TSC1 gene producing
hamartin and TSC2 gene producing tuberin, most neurological manifestation of TSC usually
seizure caused by either intracranial tubers, subependymal nodules (SEN) and subependymal
giant cell tumors (SEGA). We report a case of Ms. MS, 18th years old with main complaint
of seizure since 5 years ago. Electroencephalography (EEG) show abnormal epileptiform
activity of 4 Hz-spike-wave complex predominantly on left temporal region while head CT
scan showed multiple calcification. Neurobehavior assessment revealed mild cognitive
impairment on memory, language and visuospatial domain. Chromosom analysis was done
with result of there is no major structural disorders. Seizure is now controlled with valproic
acid.

Abstract
Smell perception is one of the most important aspects of life, odor perception leads to the
introduction of different types of food, detection of possible harmful elements and overall
quality of life. In chronic kidney disease (CKD), kidneys are disturbed to filter blood so that
the body's remaining metabolic substances such as urea, uric acid and creatinine cannot be
excreted. Urea retention may result from a smell disturbance by uremic binding OBPs and
lipocalins mechanism, thereby saturating olfactory signaling pathways Cross-sectional
observational study. A total of 45 patients CKD Grade 5 were included in the study.
Olfactory function was assessed for odor threshold, discrimination and identification
using Sniffin Sticks Test. The mean of TDI is 26.3 (SD = 4.9). Impaired olfactory detection
was found 64.4% of patients with 27 subjects suffering from hyposmia and two subjects
suffering from anosmia.

Abstract
Sinonasal squamous cell carcinomas are a malignant tumor occurring in the nasal cavity and
paranasal sinuses. The p53 protein is a tumor suppressor gene, functioning in processes of
cellular apoptosis, differentiation, aging, and angiogenesis.The purpose of this study was to
observe the expression of the p53 mutant protein in sinonasal squamous cell carcinoma.This
was an observational study in Haji Adam Malik General Hospital where we performed the
immunohistochemical examination on paraffin blocks from 30 patients with sinonasal
carcinoma underwent surgery. Out of 30 samples of sinonasal carcinoma blocks, we found 16
samples with overexpression of p53 mutant protein (53,3%)

The Relationship Polymorphism of Gene RFC1 A80G and
NSCLP in Sumatera Utara, Indonesia

Abstract
The etiology of non-syndromic cleft lip and palate (NSCLP) has not yet been defined. Some
studies have investigated the involvement of genetic and environmental factors. Some genes
involved in the folate metabolism have been recently examined to discover the genetic factors
in the cleft lip etiology. This research intends to identify the involvement of polymorphism in
the Reduced Folate Carrier 1 A80G with NSCLP in the population of Sumatera Utara. In this
case-control study, 62 patients NSCLP and 61 controls underwent DNA isolation and
genotyping process using PCR-RFLP. The distribution of genotype and allele variants
between the case and control were not significantly different (p=0.271, p=0.809). In the
subgroup analysis, the polymorphism of RFC1 A80G showed a significant relationship in the
group of CLO+CPO. This research obtained the data that the RFC1 A80G mutant variant had
a relationship with NSCLP.

The effect of hypertonic saline irrigation vs baby shampoo in
chronic rhinosinusitis

Abstract
Nasal irrigation is a treatment of choice to relieve symptoms in chronic rhinosinusitis (CRS).
Adding a surfac-tant to the solution may increase its efficacy, but the effects on mucociliary
function are unknown. To deter-mine the effect of nasal irrigation with surfactant on
symptoms and mucociliary transport time (MTT) in CRS. Forty patients in hypertonic saline
and the baby shampoo group were evaluated for symptoms and MTT, be-fore and after nasal
irrigation for two weeks. The difference of average SNOT-20 score in the hypertonic group
was statistically significant (p= 0.01), but not in the baby shampoo group. The average VAS
score in both groups was significantly different (p= 0,01 and p = 0,05). Neither hypertonic
nor baby shampoo solution affects MTT. Nasal irrigation with baby shampoo solution has not
affected the quality of life and MTT in CRS, but may affect the severity of the disease.

The Effect of Oral Methylprednisolone on the Expression of
Interleukin 5 Nasal Polyp

Abstract
Nasal polyp is an inflammatory chronic disease which tends to recurrence. IL-5 interleukin is
a selective cytokine in eosinophil and basophil. Corticosteroid is a selected therapy which is
effective. The aim of this study is to investigate the effect of oral methylprednisolone on the
IL-5 nasal polyp expression. The research design is quasi experimental. The finding shows
that there is a meaningful decrease of the nasal polyp stages before and after oral
methylprednisolone therapy. There is a meaningful decrease of the IL-5 expression before
and after the oral methylprednisolone therapy. The distribution of effective oral
methylprednisolone decreases the size of nasal polyp and decrease the IL-5 expression.

The accuracy of pediatric appendicitis score based on age
group of acute appendicitis

Abstract
Acute appendicitis in children is one of the leading causes of abdominal emergencies that
require immediate surgery. The author conducted research aiming to know the accuracy of
Pediatric Appendicitis Score based on age group in acute appendicitis patient. Data were
analyzed by bivariate analysis with appropriate hypothesis testing. The results were then
reanalyzed by dividing the PAS value according to the age group then the sensi-tivity of the
PAS to the histopathological outcome of the appendix tissue was calculated to assess the
accura-cy of the PAS in each age group. PAS did not have a statistically significant
relationship in the <5 years age group. Overall, PAS accuracy was higher at age 13-18 years,
at 77.6%.There is no statistically significant rela-tionship between PAS value and
appendicitis diagnosis at <5 years age group.

Collection of stem cells in (autologous) donors by apheresis

Abstract
Leukapheresis procedures are performed to collect hematopoietic progenitor cells (HPCs)
from autologous and allogeneic donors for stem cell therapies. To collect sufficient HPCs, the
cells must be mobilized from the bone marrow into the peripheral circulation with G-CSF,
chemotherapy, and or plerixafor. Transfusion of specific number of HPCs are needed to
guarantee sufficiently fast and reliable recovery of the bone marrow function after the
myelosuppressive or myeloablative treatment in the patient. To assure safety for donor as
well as patient in these kind of therapy, solid quality in the complete chain is needed.
Therefore, in many countries, by law JACIE/FACT accreditation is required.

Correlation of Irritant Exposure to Nasal Mucociliary
Trammel at Gas Station in Medan

Abstract
The exposure of irritant substances such as benzene, toluene, xylene, and ethylbenzene in the
air can cause of disruption the transportation time of nasal mucociliary. The aim of this study
was to find out the correlation of the exposure of irritant substance towards the
Transportation Time of Nasal Mucociliary disruption at gas station officer in Medan city. The
type of research which is conducted was analytical research with cross-sectional study
design.There are 42 samples of subjects according to the inclusion and exclusion criteria
where 31 subjects went through a deceleration of nasal mucociliary transport (NMT) time.
The location of exposure has a significant correlation between the nasal NMT disorder that is
as many as 31 people (100%) on direct exposure.The nasal mucociliary transport time slows
down on the gas station officers who are exposed to di-rect irritant exposure.

Sum of Precordial ST Depression Predict High Grade LAD in
AIMI

Abstract
There were many studies proved that precordial ST depression in Inferior STEMI show LAD
disease, but there were also many who unproved. Sum of precordial ST depression is one of
ECG criteria that can prove it. We analyzed 60 patients inferior STEMI with precordial ST
depression. Patients were divided into four groups based on a sum of precordial ST
depression. Bivariate analysis was made to see the correlation be-tween a sum of precordial
ST depression with LAD disease and severity of LAD. There is the strong correla-tion
between a sum of precordial ST depression with LAD stenosis and severity (p=0.01).
Analysis ROC curve sum of precordial ST depression to LAD disease cut off ≥5.15 mm with
sensitivity 70% and specificity 76.2%(AUC0,752,95%CI:0,620–0,883p<0,001) Sum of
precordial ST depression correlate with high-grade LAD disease.

Correlation of Leukocyte Subtypes, NLR & Functional
Outcome in Brain Metastasis

Abstract
As most cause of death in patients with solid extracranial malignancy, brain metastasis (BM)
nowadays being studied extensively especially on how to find a reliable laboratory marker
that can correlate with its clinical outcome. Leukocyte subtypes, primarily neutrophils and
lymphocytes and its ratio known as neutrophils-to-lymphocyte ratio (NLR) has been known
before its relationship with proggresivity of BM from other solid tumors. We conduct a mean
differences and correlational leukocytes and its subsets analysis of 72 BM patients resulting
on significant positive correlation on lymphocyte percentage (r=0.383, p=0.001) and
lymphocyte absolute (r=0.265, p=0.024), also significant negative correlation on neutrophils
(r=-0.240, p=0.042) and NLR (r=-0.432, p<0.001) with Karnofsky Performance Scale (KPS).

Ferritin Level After Ferrous Fumarate Supplementation At
The 2nd Trimester Pregnancy

Abstract
This study is to determine the comparison of ferritin level after ferrous fumarate
supplementation at second trimester of pregnancy. This study was an experimental study with
pre-post test design. All subjects were compared with ferritin serum after one-month ferrous
fumarate consumption.The study began in April 2015. Blood sampling was conducted to all
mothers who entered the inclusion criteria, were given ferrous fumarate for one month then
they were re-examined. The mean rate of Ferritin in maternal serum after iron supplemen-
tation measured after one month was 28.7689±20.77 ng/ml higher than before treatment, ie,
17.5±19.09 ng/ml. Wilcoxon test got P-value <0,05. There is a significant increase in serum
levels of pregnant women’s ferritin after iron supplementation.

Abstract
To determine the characteristic of the patient with benign ovarian cyst, histopathologic
distribution of benign ovarian cyst, HE4 expression in various benign ovarian cyst tissue and
normal ovaries and to determine HE4 expression base from histopathologic type. This
analytic observational study was performed with the case-control design. In this study, HE4
immunohystochemistry staining was performed in the various benign ovar-ian cyst (n=20)
and normal ovaries (n=20) and interpretation was performed by the pathologist using Allred
score. There is a statistically significant relationship between benign ovarian cyst with HE4
expression. Hence there is no significant difference between histopathologic subtype of the
benign ovarian cyst with HE4 ex-pression.There is a significant relationship between the
benign ovarian cyst and HE4 expression.

Red Blood Cell Indices and CD4 Count In HIV-Positive

Abstract
Introduction Red blood cell indices, hemoglobin, and hematocrite reflect rapidity of HIV
disease progression. Objective This study aim to determine red blood cell indices and CD4
count in HIV-positive reproductive women. Methods This study was a cross sectional study
conducted at AIDS outpatient clinic at Haji Adam Malik General Hospital, Medan Indonesia.
All seropositive reproductive women within antiretroviral therapy were consented for blood
count and CD4 examination. Data was collected and analyzed with SPSS 19. Results In
subjects with CD4≤350 mm3, mean hemoglobin was 10.95 ± 2.01, hematocrite was 31.83 ±
5.04%, MCV was 84.17 ± 11.41, MCH was 25.98 ± 2.65, and MCHC was 32.18 ± 2.17.
Mean hemoglobin, hematocrite, and MCH value was significantly lower in subjects with
CD4 ≤350 mm3 (p=0.014; p=0.001; p=0.01; respectively). Conclusion Lower Hb, Ht, and
MCH were associated with lower CD4 count.

IHC Expression Relationships MMP7 and VEGF with
Normal Ovaries and Ovarian Pathologies

Abstract
To investigate the association of MMP7 and VEGF expression with ovarian cancer, benign
ovarian cysts, and normal ovaries. This study was analytical research with case control
design. In the study, IHC expression of MMP7 and VEGF was carried out on paraffin block
of ovarian cancer tissue, benign ovarian cyst, and normal ovary. In 40 subjects with ovarian
tumors, 17 patients were found with positive MMP7 expression. In the con-trol group, no
subjects were found with positive MMP7 expression. There was a significant relationship be-
tween MMP7 expression and ovarian tumor incidence. Meanwhile, in 40 subjects with
ovarian tumors, 21 pa-tients were found with positive VEGF expression. In the control group,
no subjects with positive VEGF ex-pression were found. There was a significant relationship
between VEGF expression and ovarian tumor inci-dence. There was a significant relationship
between MMP7 and VEGF expression with ovarian tumor inci-dence.

Trigeminal cardiac reflex in post-operative spheno orbital
menigioma

Abstract
The trigeminal cardiac reflex (TCR) is a phenomenon that composed of bradycardia, arterial
hypotension, ap-noea, and gastric hypermobility. It most occur during skull base surgery at or
around structures that are inner-vated by any sensory branch of the trigeminal nerve. We
report a 48-year-old female who was diagnosed of having left spheno orbital meningioma and
performed cranio-orbitozigomatic approach to remove the tumor. Six hours post-operative,
she had sudden decreased in heart rate 31% decreased from base line, and also mean arterial
blood pressure decreased 35% from base line. Laboratory finding was normal, oxygen
saturation was 100% and no hypercarbia occurred. She was suspected to have trigeminal
cardiac reflex. Intravenous admin-istration of atropine abolished the reflex, heart rate and
MABP slowly increased to normal.

Cancer Stem Cells in Colorectal Carcinogenesis

Abstract
Colorectal cancer (CRC) is the third most commonly cancers worldwide. Indonesia with
more than 250 mil-lion population, is a country in South East Asia with the incidence rates
for CRC per 100,000 population were 15.2 for males and 10.2 for females, should have
63,500 cases per year. According to the stochastic, any cells are capable of initiating and
promoting cancer development, whereas the cancer stem cell (CSC) model sug-gests that
tumors are hierarchically organized, only CSCs possess cancer-promoting potential. The
Wingless, Notch, Hedgehog pathway, has shown to be dysregulated (uncontrolled) in
colorectal CSC. Many new mole-cules are now being studied to block theses pathways. Some
of the molecules block the self-renewal and in-duction of apoptosis in CSCs. The design of
CSC-targeted interventions is a rational target and reduce local recurrence and metastasis.
This review aims to summarize the issue on CSCs.

Relationship Knee Osteoarthritis Grade and Serum
Cholesterol Level in North Sumatera

Abstract
Several studies have shown a clear association with obesity, age, gender, and osteoarthritis,
however, the role of cholesterol is unclear. The aim of this study was to determine whether
serum cholesterol level was associat-ed with the grade of knee osteoarthritis. Plasma levels of
total cholesterol, triglycerides, low-density lipopro-tein, high-density lipoprotein were
measured in 47 knee osteoarthritis. Knee osteoarthritis was defined using Kellgren-Lawrence
grading scale. There were 40 females (85.1%) and seven males (14.9%) with a mean of age
61.38 + 10.43 years respectively. Subject with osteoarthritis correlated with gender
male/female, grade I 4(0/10), grade II 11(14.3/25), grade III 14(28.6/30), grade IV
18(57.2/35) averaged 2.8+0.98. There was no significant correlation between the level of
total serum cholesterol, triglycerides, low-density lipoprotein, high-density lipoprotein and
knee osteoarthritis grade. In conclusion, all the lipid and lipoprotein that observed in knee
osteoarthritis radiograph have no relationship with its grade.

The Correlation between Smoking and Olfactory Function
Using Sniffin’ Sticks Test

Abstract
To determine the correlation between smoking and olfactory function. This was an analytical
observational study with the cross-sectional design. The population in this study is all
employees, staff, residents, the family of patients residing in ENT Department Haji Adam
Malik Hospital in September 2016. All subjects must have been between 25-50 years old,
currently smoking cigarettes and did not meet the following exclusion criteria: the medical
history of recent viral infection, nasal/sinus surgery, nasal/brain tumors, head trauma,
radiotherapy, chronic rhinitides in exacerbation or tracheostomy. Out of 45 smokers, the
proportion of olfactory dysfunction was 37.8% while 62.2% did not suffer from olfactory
dysfunction. The TDI score average was 30.80 with the lowest score average of 22.12 and
the highest of 40. The results of this study indicate a significant correla-tion (p <0.05)
between the number of sticks per day and the number of years smoking with olfactory
dysfunc-tion.

The Outcome of Tibial Fracture Who Underwent ORIF
Compared to MIPO

Abstract
MIPO is a new technique of internal fixation that is aimed to fix the weaknessesnof
conventional techniques. In this study, we researched the outcome of patients with tibial
fracture who underwent ORIF compared to MIPO. We found 30 patients, whom 15 were in
the group of conventional internal fixation surgery and 15 were in the group of internal
fixation surgery minimally invasive. For the length of stay, from the chi square test resulted p
value = 0,043 (p<0,05), showed there was a difference in length of stay between the ORIP
group and MIPO group. From all the post operative complications studied, the chi square test
resulted in p>0,05 which meant there were no difference in the variables between the ORIF
group and MIPO group. Meanwhile for post operative complications, there were no
differences between the ORIF group and MIPO group.

The relationship of HER2 overexpression to histopathologic
grading of breast cancer

Abstract
Human Epidermal Growth Factor Receptor 2 (HER2) has a close relationship with the
histopathologic grading of breast cancer that is thought to be associated with prognosis and
recurrence. The data was taken from 2011-2014 tissue histopathology, grading, and HER2
medical records data. Immunohistochemistry examina-tion for HER2 with values of +1, and
+2 is said to be negative (unexpressed), and +3 is said to be overex-pressed, the grades were
grouped into low grade (grade 1 and 2) and high grade (grade 3). From this study, we
obtained prevalence ratio of 7.667. Thus it can be said that HER2 expression was eight times
more likely to have high grading when compared with negative HER2 results. There was a
significant association between HER2 (Neu overexpression) with the histopathologic grading
of breast cancer.

The Relationship between Neck Circumference and BMI with

Abstract
Hypertension is still becoming a global health problem. Body Mass Index (BMI) and Neck
circumference (NC) are an anthropometric measurement to assess obesity and also reported
to have a correlation with the risk of hypertension. to determine the relationship between NC,
BMI and hypertension in children. A cross-sectional study was conducted in North Sumatera,
Indonesia. Samples were children aged 12-15 years old. Chi Square test was used to see the
relationship between NC, BMI and hypertension. Logistic regression was used to compare
which one has a bigger influence on the increase in blood pressure. P<0.05 was significant.
264 children were included. Hypertension occurred in 9.8% of children with neck
circumference above the 90th percentile and 11.4% in children with overweight/obese BMI
status. Children with overweight/obese BMI status showed a higher risk of hypertension. NC
and BMI has been correlated with hypertension in chil-dren.

Abstract
Post-Stroke Depression (PSD) characterized by mood abnormality, self-blame, and grief
diagnosed in 40-72% of stroke patients, that can inhibit the healing of stroke patients and
increase mortality rate. The aim to investigate the difference of CRP between depression
level in ischemic stroke patients. 45 consecutive ischemic stroke patients at Adam Malik and
affiliation were assessed the CRP value, then on the 15th day, the Beck Depression Inventory
(BDI) questionnaire used to classify depression levels. 51.1% PSD patients were men with
60.71 ± 10.3 years old. 64.4% were smokers and high school, graduates. Mean CRP level in
post-stroke mild depression was 0.7 mg/dl, moderate depression 3 mg/dl and major
depression 9.4 mg/dl. There were significant differences in CRP value between depression
levels in ischemic stroke patients (p=0.001). The highest CRP level occurs in severe
depression, followed by moderate depression and mild depression.

Apoptotic Effect of Gentamicin in Cochlea Ototoxic Model
(Pre Eliminary Study)

Abstract
This study aimed to show the potency of gentamicin to increase apoptotic index in cochlea
lateral wall in oto-toxic rat models. Eight Rattus norvegicus were divided into 2 groups, 1:
control, 2: gentamicin (+). The rats were terminated to measure apoptotic index with
TUNEL assay in the fibroblasts of the cochlea lateral walls. Data were analysed by using
ANOVA and p < 0.05 was used as the cut-off for statistical significance. Ad-ministration of
gentamicin showed increased apoptotic index which statistically significant (p< 0.05) in.
This study showed the potency and ability of gentamicin as pro apoptotic.

Increased Expression of TGF-β in Cochlear Fibroblast in
Diabetic Model Rats

Abstract
Cochlea as a microvascular organ is very susceptible to conditions that result in disruption of
vasculariza-tion.TGF-β plays a role in diabetic complication process especially in
microangiopathy due to its role in thick-ening basal membrane of the vessel. This study aims
to see the TGF-β expression in cochlear fibroblasts dia-betic model rats. Eight male Rattus
norvegicus divided into 2 group, group 1: control, rats were injected only with the single dose
of sodium citrate intraperitoneally and then terminated on day 5. Group 2: rats were in-jected
with intraperitoneal single-dose streptozotocin (STZ) and terminated on day 5.Samples were
terminated and necropsied for the removal of their cochlear tissue and counted for its TGF-β
expression by immunohisto-chemical examination. There was the significant difference
(p<0.05) in TGF-β expression in group 1 compared to group 2.

Abstract
Children glioma is a life-threatening condition with a survival rate of less than 1 year.
Nimotuzumab, a unique and affinity differentiated anti-EGFR antibody for the past 10 years
had been used in combination with radi-otherapy or radiochemotherapy. The aim of these
studies was to evaluate nimotuzumab in combination with radiotherapy or radiochemotherapy
in newly diagnosed children with DIPG. Patients received i.v infusions of 150 mg/m2 of
nimotuzumab in combination with 54 Gy radiotherapy or radiochemotherapy for DIPG
patients or 150 mg/m2 of nimotuzumab as monotherapy on high grade glioma, weekly, for 6-
8 weeks, and then every 2-3 weeks until disease progression or unacceptable toxicity. A total
of 87 newly diagnosed children with DIPG (35 male/52 female), median age 7.6 yr old were
recruited Use of nimotuzumab had been efficacious in newly diagnosis children with DIPG
and in recurrent or refractory children with high grade glioma.

Comparative Time Achieved VAS ≤ 3 Oxycodone And
Fentanyl Post-Operative Analgesia

Abstract
Fentanyl has a potency ratio 100 times stronger than morphine. Oxycodone is a receptor
agonist of mu and kappa opioid, structurally similar to codeine, but pharmacodynamically
similar to morphine. To find out the comparison of time achievement of VAS ≤ 3 on
oxycodone. This study was using double-blind, randomized clinical trials, collected 48
samples, ages 21-60 years, PS ASA I-II. Oxycodone (A): Fentanyl (B) (initial bolus 5 mg
than continuous infusion dose of 1 mg/hour: initial bolus 5 cc of NaCl 0.9% than continuous
infusion dose of 0.5 mcg/hour) with significance level 95% (p <0,05). Average length of
surgery Fentanyl: Oxycodone (218.96 ± 53.38 minutes: 191.87 ± 80.10, p> 0.05). Oxycodone
is faster in achieving VAS≤3 compared with Fentanyl intravenous infusion.

Abstract
Currently, adult stem cells can be obtained not only from the spinal cord and peripheral
vessels but also from fat tissues of the human body, where it can be isolated as adherent stem
cells (mesenchymal stem cells). Consideration of fat tissue as the source of mesenchymal
stem cells (MSCs) for autologous tissue engineering is because they are readily available in
abundant quantities through minimally invasive procedures, as well as easily cultured and
propagated. Besides, the fat tissue is a type of adult stem cells (ASCs) that capable of
metamorphosis/ transdifferentiation into various strains due to the nature of high plasticity.
Therefore it is possible to proliferate in the desired direction of the network. The purpose of
this research was to the investigated effect of different medium on adipose-derived stem cells
(ADSCs) proliferation from the human subcutaneous adipose tissue.

Low Survival Rate in High-Grade Osteosarcoma:
A Retrospective Study at A Single Institution in Indonesia

Abstract
Osteosarcoma is a very rare malignant bone tumour, it is an aggressive bone neoplasm.
Combination of surgi-cal removal and systemic multidrug chemotherapy is the current
strategy of treatment. The study aims to eval-uate the prognostic factors influencing the
survival rate in our centre. A retrospective cohort study of patients diagnosed with
osteosarcoma between January 2012 and December 2014. A total of 58 osteosarcoma patients
were included in this study, including 36 men (62%) and 22 women (38%), with a mean age
at diagnosis of 15 years. The median survival of this group was 12 months. Patients with no
pulmonary metastases after treatment had better survival rates of 18 and 90% at two years.
The overall survival of osteosarcoma patients in this study is low. Patients who do not
comply with treatment had worse survival. Patients with pulmonary metastases have a
significantly increased risk of death.

Factors Influencing the Decision of Parents to LP in
Pediatric with CNS Infection

Abstract
Lumbal Puncture (LP) is a procedure that can help to diagnosed CNS infection in children.
The procedure of LP is a common invasive procedure, but sometimes it had refused by the
parents. Furthermore, it can cause a delayed in the diagnosis and affected the prognosis. The
subject was the parents of pediatric patients with CNS infection. We have a questionnaire and
using Chi-square and Fisher exact test to analyze factors that in-fluenced the parent's decision
to LP procedure. From the parents of 50 children with CNS infection, 19 par-ents (38%) were
refused the LP procedure. Father’s education was related to the refusal of LP procedure with
PR 6.64 (95%CI; 8.95-788.08). Mother’s education was related to the refusal of LP
procedure with PR 7.69 (95%CI; 3.19-16.24). There was the significant result of the parent's
education with the decision of LP proce-dure.

Abstract
Blood glucose and white blood cell count at admission have shown a prognostic significance
in patients with myocardial infarction. The aim of this study was to analyze the prognostic
value of Leuco-Glycemic Index (LGI) to predict major adverse cardiovascular events
(MACE) in patients with acute ST segment elevation myocardial infarction (STEMI) without
diabetes. A retrospective study was conducted in 81 patients STEMI without diabetes. The
cutoff point of LGI value is derived from the ROC curve and compared the value of the LGI
cutoff point to MACE during hospitalization. The value of the LGI cutoff point is 1420 with
a sensi-tivity of 81.5% and a specificity of 72.2%. In a multivariate analysis, LGI ≥ 1420 (OR
12.672, p=0.003) was an independent predictor of MACE during treatment in STEMI
patients without diabetes.

The Relation of Allergic Rhinitis Classification with IL-5 on

Abstract
IL-5 is an important role cytokine on the RA. IL-5 has an important role on eosinophils.
ARIA-WHO made classification of RA based on how long the clinical symptoms and the
impact on quality of life. The aim of this study was to know the classification of RA with IL-
5 on RA study. This study used a cross-sectional method with 39 samples. The examination
of IL-5 used ELISA. The highest classification of RA was medi-um-severe persistent of
43.58% with the mean value IL-5 was 56.25 pg/ml. Based on the test of Kruskal Wal-lis, it
was obtained p-value = 0.664. Conclusion: There was no significant relation between
classification RA and IL-5.

Correlation Between Breastfeeding Duration and Motor
Development in Children

Abstract
WHO recommends to continue breastfeeding until two years of age. There are few studies
assessing the correlation between breastfeeding duration and motor development. A cross-
sectional study was conducted in April 2016 in Muara Batang Gadis. The sample were
children 3–6 years of age. Breastfeeding duration was obtained by interview, and motor
development was assessed using Denver Developmental Screening Test II. Chi-square test
was used to evaluate the correlation between breastfeeding duration and motor development,
with P-value <0.05 and 95% Confidence Interval (CI). Among 204 children, 5.4% had the
fine motor delay and 1% had the gross motor delay. Children with breastfeeding for at least
two years had better fine motor development compared to breastfeeding for less than two
years (P<0.001, PR=1.139, 95% CI=1.055 – 1.231) and there is no difference in gross motor
development (P=0.193, PR=1.023, 95% CI=0.991 – 1.055).

Association of Anal Position Index (API) with Constipation

Abstract
Constipation is one of the most common chronic disorders of childhood. Anterior
displacement of the anus is one of the organic causes of constipation. Reisner et al. proposed
an objective method to determine the normal position anus using anal position index. Anal
position index less than 0.46 in boys and less than 0.36 in girls in-dicate anterior
displacement anus.A cross-sectional study of 66 children was conducted. Subjects were
chosen using consecutive sampling. Study data were collected by conducting demographic
data and anal position in-dex.Anal position index, which is the ratio of anus-scrotum distance
in boys and anus-fourchette in girls be-tween coccyx and fourchette/scrotum was calculated
in this study.Girls were more likely developing constipa-tion than boys. There was no
relationship between anal position index and constipation(PR 0.825; 95%CI 0.201 to 3.394).

Relationship Between CHA2DS2-VASC Score With CIN For
AMI Patients After PCI

Abstract
The CHA2DS2-VASC score has been reported recently to predict adverse clinical outcomes
so is CIN in patients with AMI regardless of having AF. We investigated the relationship
between CHA2DS2-VASC score with CIN in patients who underwent PCI strategies. This is
a study of 40 patients with and underwent PCI. The CHA2DS2-VASC score was calculated
for each patient. From this study 16 cases(18.82%) of CIN were diagnosed. CIN was defined
as rising in serum creatinine>0,5mg/dL or>25% increase in baseline within 24h after PCI. In
the ROC curve analysis,the cut-off value of CHA2DS2-VASC score in the prediction of CIN
was>4 (sensitivity:56.25%,Specificity:87.5%) (AUC0.698,95%:CI1.460-6.163,p=0.003) and
has a significant association with CIN(R20.485). We also identified Hb level<12mg/dL as an
independent predictor of CIN with (RR3.44,95%:CI1.816-6.532,p<0.001). The CHA2DS2-
VASC score was positively associated with CIN. Therefore, it can be used as a simple and
reliable tools to predict CIN in AMI patients who underwent PCI.

Abstract
The diagnostic tests C-reactive protein and haematological scoring system provide results in a
shorter time than cultures, and have been used to help to determine the diagnosis of neonatal
sepsis. Objective. To evaluate the diagnostic value of C-reactive protein and hematologic
scoring systems towards positive blood culture result. The C-reactive protein showed
sensitivity and specificity of 92.8% and 62%, respectively. The haematological scoring
system showed a sensitivity of 100% and specificity 82.7%. Both CRP and haematological
scoring system were sensitive to predict positive blood culture. However, haematological
scoring system showed better specificity than C-reactive protein. Both tools can be used to
assist the diagnosis of neonatal sepsis in resource-limited settings.

Abstract
Tumor Necrosis Factor (TNF-α) is one of the pro-inflammatory cytokines that play an
important role in the pathogenesis of nasal polyps. Fluticasone furoate (FF) is the most potent
inhibitor of NF-kB activation and TNF release. Determine the effect of nasal spray FF on
TNF-α expression on nasal polyps. This study was ana-lytic with an experimental quasi-study
design, performed on 16 cases of nasal polyp obtained with non-probability consecutive
sampling. 16 samples, TNF-α expression in nasal polyps before treatment with FF was
mostly overexpression (87.5%) while post-treatment was found to be negative (81.3%). Data
analysis showed the significant decrease in TNF-α expression after therapy (p = 0.001). There
was no significant correlation be-tween nasal polyp decline and decreased expression of
TNF-α in nasal polyp after FF therapy (p = 0.136). Fluticasone furoate has been shown to
decrease TNF-α expression in nasal polyps and significantly reduce pol-yp stages.

Profile of teenage pregnancy in H. Adam Malik Hospital

Abstract
Delivery at an early age was associated with a greater health risk for the mother. To assess
the profile of teenage pregnancy in H. Adam Malik Hospital Medan year 2011-2015. This
study was conducted with de-scriptive study design using secondary data from medical
records. There were 128 patients who became preg-nant in their teens. Then the data were
tabulated and presented in the form of frequency distribution table and were analyzed
descriptively. Most pregnant women came to the hospital for parturition (60.1%) followed by
preeclampsia/ eclampsia (14.8%). The way of delivery of pregnant adolescent women was
vaginal (52.3%) fol-lowed by cesarean (36.7%). There was no significant relationship
between adolescent age of pregnant and newborn weight. There was no significant
relationship between the age of the adolescent and method of de-livery. Most teenage
pregnancies occurred between 17 – 19 years. Most pregnant teenagers delivered vaginal-ly,
and most of the newborns were normoweight.

Relationship between House Environmental Factors and
Tuberculin Skin Test in children

Abstract
Tuberculosis is a major cause of death in the world. The increasing TB cases can be affected
by population density, temperature, ventilation, lighting, and humidity. Tuberculin Skin Test
(TST) is a tuberculosis diagnos-tic test with sensitivity and specificity of ≥ 90% and can be
used to detect children who infected with tuber-culosis. A cross sectional study was
conducted among elementary school students in Singkuang Village, North Sumatera on April
2016. Univariate analysis showed that the relationship between ventilation, residen-tial
density, temperature and TST was statistically significant (P=0.006, 0.001, 0.005
respectively). Poor venti-lation was associated with higher risk in positive TST of PR=4.6
(95% CI= 1.43-14.70), while poor tempera-ture of PR=8.8 (95% CI= 3.34-22.85), and
residential density of PR= 9.4 (95% CI =1.33-66.49). There is a re-lationship between house
environmental factors and TST among elementary school students.

Abstract
It is common to believe that depression can lead to more severe disease. Breast cancer is the
most studied cancer in terms of its psychosocial effects, and not surprisingly, many studies of
the prevalence of depression in cancer are studies of women with breast cancer. Aim: To
determine the level of depressive syndrome in breast cancer patients by using BDI
questionnaire. Method: The respondent amounted to 66 breast cancer patients who come to
the Clinic of Surgical Oncology and General Surgery at Adam Malik Hospital, and Hospital
Haji Medan. Results: It was found that depressive syndrome was most prevalent in patients
with breast cancer (42.4%), followed by severe depressive syndrome (25.8%), mild
depressive syndrome (19.7%), and minimal (12.1% ).

Diagnostic accuracy of Gene Xpert MTB/RIF for TBC Pleural
Effusion Compared to ADA

Abstract
Indonesia is one of the few countries with high burden of tuberculosis (TB). Pleural TB is the
most common form of extrapulmonary TB. However, diagnosis of tubercular pleural effusion
remains a common clinical challenge. From May 2017 through September 2017 a total of 42
patients with exudative lymphocytic pleural effusions suspected of pleural tuberculosis were
enrolled in this study. Adenosine deaminase (ADA) and Gene Xpert were examined from
pleural fluid. 64,3% had ADA in positive level (>40 IU/L), and 31% had Gene Xpert positive
Mycobacterium Tb. The Gene Xpert sensitivity was 40.7% with specificity 86.6%, PPV and
NPV were 44.8% and 57.1%. Kappa Compatibility study shows that Gene Xpert point was
0.227 (p 0.066). The usefulness of Gene Xpert to diagnose pleural TB is limited by its poor
sensitivity. Gene Xpert can not be used as single diagnostic tool for tuberculous pleural
effusion.

Sensitivity and Specificity of Urine Nitrite Test and Gram
Staining of UTI in Children

Abstract
Urine culture is a standard gold diagnosis of urinary tract infections (UTI). More simple tests
are often used instead of urine cultures. To compare sensitivity and specificity of urine nitrite
testing and gram staining to urine culture. A cross-sectional study was conducted at RSUP H
Adam Malik, February to July 2017, recruited with consecutive sampling. Data collected
were sample characteristics, clinical manifestation, causal organism, nitrite test, gram
staining, and urine culture results. Data were analyzed with Fisher exact test with p <0.05. A
total of 60 samples participated. The organism encountered was E.Coli. The clinical
manifestation that was often was the fever of 37 children with UTI. The sensitivity and
specificity, of nitrate test, were 64.8% and 86.9%, sensitivity and specificity of gram staining
were 94.5% and 100%. Gram staining is a good alternative test for diagnosis of UTI but for
nitrite testing still needs further research.

Abstract
Hyperglycemia causes a phagocytic injection of polymorphonuclear leukocyte (PMN), which
is associated with an increased frequency of infection. Impaired function of PMN
phagocytosis is associated with an increased risk of infection that stimulates the formation of
proinflammatory cytokines. In chronic worm infections, Treg cell activation results in IL-10,
which plays a role in class switching antibody response in which IgE-producing cells switch
to produce IgG4 which inhibits effector effranulation. This study aims to examine the
correlation between Soil-Transmitted Helminths (STH) Infection and IL-10 enzyme genes
polymorphism among Diabetes Mellitus (DM) and Tuberculosis (Tb) patients. The results
showed that negative significant correlation between STH infection and IL-10 genes enzym
polymorphism among DM and Tb patients.

Cardiomyopathy occurrence in breast carcinoma with three
and six cycles of doxorubicin

Abstract
Objective: Incidence of breast carcinoma continues to increase, more than 170,000 cases are
found per year, where incidents vary by country. One of the treatments for breast carcinoma
is chemotherapy. The most com-monly used regimen is doxorubicin and one of its
complications is cardiomyopathy. Materials and Methods: All breast cancer patients were
histopathologically confirmed. Echocardiography is performed in patients who have
undergone 3 and six cycles of doxorubicin. Ejection Fraction was recorded for
cardiomyopathy assess-ment. Results: Of the 42 cases of 3 cycles, seven people had
cardiomyopathy, all of which were mild (45% - 55%). Of the 48 cases undergoing six cycles,
six people had cardiomyopathy, 5 of which were mild, and the other person was in a severe
category (<35%). Conclusion: Cardiomyopathy may be considered starting after three cycles
of doxorubicin administration, where the highest incidence occurs at the age of 40 years.

Abstract
An inflammation agent such as TNF-α can induce EC dysfunction via releasing protein
endoplasmic reticu-lum stress (ERS) pathway. Targeting ERS was the new approach in
vascular biology to inhibit EC dysfunc-tion. One of its biomarkers is activating transcription
factor 6 (ATF6). However, ERS in endothelial cells is still poorly understood. In this study,
we performed in vitro experiment using phenyl butyrate acid as ERS inhibitor to TNF-α -
induced-ATF6 expression in human vein derived endothelial cells. We measured ATF6
expression in endothelial cells as ERS biomarker and used phenyl butyrate acid (PBA) as
potent, selective ERS inhibitor to block its pathway. The early result shows that PBA
decrease translocated ATF6 expression in endothelial culture. From the result, it can be
concluded that PBA has the role in decrease endoplasmic reticu-lum stress in endothelial
cells.

Abstract
The World Health Organization (WHO) emphasizes the importance of education on
reproductive health in younger adolescents, the age-group of 10 to14 years or equal to Junior
High School-aged adolescents. The objectives were: 1) to analyze the influence of
counselling by using lecture method on the level of knowledge of adolescent reproductive
health in the Junior High School female students in urban and rural areas, and 2) to identify
social and economic factors and history of previous counselling, which probably influenced
the difference. A quasi-experiment two group pretest-posttest design with purposive
sampling, 78 students in urban and 80 students in the rural area, were analyzed. The result
was the counselling about adolescent reproductive health in female students of Junior High
School influenced the change in the level of knowledge, either in the rural area or urban area.

Length of Stay Diaphyseal Tibial Fractures in North Sumatera

Abstract
Diaphyseal tibial fracture is the most common long bone fractures. From distribution data in
Adam Malik General Hospital from January 2005 – March 2007 revealed that lower
extremity has the highest incidence rate (63,5%). Mostly the hospital shows ineffective
service as a reason to make the costwas increased. The purpose of this study is to know the
length of stay differences between close and open diaphyseal tibial fractures. Ninety-eight
patients with closed and open diaphyseal tibial fractures were retrospectively reviewed. Close
fracture has twenty-eight patients. Open fractures have seventy patients. The length of stay
was count by measured the differences between date came and dated out. In the close fracture
group, the mean time was 18.79 ± 14.37(p<0.05) and in the open fracture group the mean
time was 14.69 ± 9.82 (p<0.05) There were no statistically significant differences in the
length of stay between close and open fracture.

Abstract
MMP-9 enhances vascular permeability resulting edema and displacement of inflammatory
cells. Fluticasone furoate has a receptor affinity for glucocorticoid receptors. The objective is
to know the effect of fluticasone furoate on the expression of MMP-9 nasal polyps. The
research was with an experimental quasi-design of 16 nasal polyps. Examination of MMP-9
nasal polyp expression was performed with immunohistochemistry MMP-9 before
fluticasone furoate overexpression of 75% but after therapy 37.5%. There was a significant
decrease in MMP-9 expression after fluticasone furoate therapy.

Abstract
The safety of MSG on the fetus has not been confirmed yet. The objective of this research is
to determine the effect of tea of torbangun leaves on fetal development disorder of mice. The
present study is experimental with a Completely Randomized Design (CRD). Data obtained
were then analyzed by ANOVA, with p<0,05 as the level of significant. The comparison of
mean values ± SEM increase number of live fetuses group C1-T1
(4,60 ± 0,93; 11,40 ± 0,87), p=0,018. The comparison of mean values ± SEM decrease
number of embryo re-sorption percentage group C0-C1 (00,00 ± 00,00; 36,74 ± 15,13), group
C1-C2 (36,74 ± 15,13; 00,00 ± 0,00), group C1-T1 (36,74 ± 15,13; 00,00 ± 0,00), and group
C1-T2 (36,74 ± 15,13; 15,00 ± 22,36), p=0,020. This study concluded that T1 group is giving
the best effect on increasing life fetuses and decreasing embryo re-sorption percentage.

Association Between Nutritional and Serum Zinc Among

University of Sumatera Utara Hospital between July and November 2016. Samples were
children aged six months to 5 years without liver function disturbances, kidney diseases,
obesi-ty or overweight, and zinc supplementation. Anthropometric, serum zinc, and albumin
level measurements were held along with demographic data collection. Fifty-one samples
were obtained and divided into normal nutrition status, moderate malnutrition, and severe
malnutrition groups equally. There are significant associa-tions between malnutrition and
serum zinc and also malnutrition and albumin. There is a significant correlation between
serum zinc and albumin level.

Abstract
Multiple myelomas (MM) is neoplastic plasma-cell disorder, considered to originate from B
cell, with resultant monoclonal protein secretion that characterizes its type. A case of 56 years
old female felt progressive bone pain and made her difficult to walk.She has diagnosed with
MM and candidate for autologous stem cell transplantation candidates (ASCT), she got
induction chemotherapy in Medan with bortezomib 1,3mg/m2 and dexamethasone
40mg/dose for four cycles with reduction of plasma cells about 1,5%.Then She had run
ASCT in Surabaya within a month mobilization with G-CSF and the high dose of
cyclophosphamide after that they counted CD 34, it was followed by apheresis stem cell then
ablation with the high dose of melphalane.Have a rest time for three days that had been
planned for thawing and infusion of the fares result.The last result of BMP has shown
decreasing of plasma cell about 1%, patient in stable condition and can do daily activity.

Abstract
Diabetes and Tuberculosis, two chronic diseases which have a major impact on the
population health in developing countries, contribute to the increase in morbidity and
mortality worldwide. Diabetes presented with hyperglycemia due to insufficiency or
resistance of insulin has been associated with increased risk of infection. DM type 2 has a 3-
fold increased risk for the development of active pulmonary tuberculosis, greater severity of
tuberculosis, with a greater odds of cavitation, increased bacillary loads, delayed sputum
conversion, treatment failure, relapse, and death. Poor glucose control reflects by HbA1C and
body mass index has been suggested as risk factors in diabetic with tuberculosis.

Abstract
Exercise can be done continuously and intermittent interval with various intensity. The aim of
this study was to determine the chronic effect of various type of exercise on fasting blood
glucose (FBG) level and insulin resistance in DM type 2 model. Insulin resistance was
determined by calculating HOMA-IR. Data were analyzed with the paired t-test. The results
showed that all group could decline FBG level and HOMA-IR significantly (p <0,05) after
eight weeks exercise. But we found there as not statistically significant for insulin resistance
post-test in MICT group. The lowest FBG and insulin resistance level were shown in SIT
group. Continous training or interval training of various type can reduce FBG and insulin
resistance as the chronic effect in DM type2 rat model. Slow interval training model was the
best model to decreased both parameter in the recent study.

Endoscopic Endonasal Surgical Extirpation of Benign and
Malignant Tumours

Abstract
Endoscopic sinus surgery was first applied to manage acute inflammatory sinus disease in the
early eighties. Based on a then newly discovered physiological principle of nasal mucosal
ciliary flow, it produced resounding success rates. The phenomenal commercial viability of
this technique spawned increasingly better instrumentation and enabled surgeons the
visualisation and special surgical tools to expand this approach to treat tumours of the nasal
and para-nasal cavity. The surgical management of benign tumours e.g. inverted papilloma
via an endoscopic approach is now well established. The surgical precision made possible by
endoscopy has now been applied to treat malignant tumours. In tandem with improving
imaging modalities and chemotherapy and radiotherapy protocols, nasal and para-nasal
malignant tumours will be more precisely assessed and be managed more effectively and
with less morbidity than with open techniques.

Abstract
Increased fatherhood has been associated with an increased risk of spontaneous abortion,
infant mortality, premature birth, congenital malformations, childhood cancer, epilepsy,
autism, bipolar and schizophrenia in offspring. The biological mechanisms behind this
relationship are not known but may be associated with de novo mutations and epigenetic
changes in male cells with short and long-term health consequences for their offspring. Males
are responsible for most of the new mutations in the gene group, to the permanent distribution
of reproductive cells and the number of mutations increases significantly with age. The aim
of the study to determine the correlation between paternal age and onset of schizophrenia.
This study was correlation analysis with cross-sectional approach. The subject is 92
schizophrenia patient. Analysis of he data using statistical test that is chi-square. The result
showed there was the significant correlation between paternal age with the schizophrenic
aged onset of 0.001 (p <0.05).

Abstract
In type 2 diabetes mellitus, there are changes in hemostasis components, including
overexpression of PAI-1. By these facts, the authors are interested in conducting the study of
PAI-1 level in diabetic patients with and without foot ulcer. The sample of this research was
collected cross-sectionally on 20 type 2 diabetic patients with the foot ulcer and 20 without
foot ulcer. Blood samples were taken to measure serum PAI-1 level, complete blood count,
and hemostasis screening test. Statistical analysis showed there are significant differences in
hemoglobin, trombosit, Fibrinogen and D-dimer levels in the ulcer and non-ulcer groups but
no significant difference in PAI-1 level. No higher PAI – 1 scale was found in diabetic foot
ulcer group than those without diabetic foot ulcer.There was no significant difference
between PAI-1 level and grade of diabetic foot ulcer.

Abstract
The ratio of trans-3′- hydroxicotinine (3′-OH)/ cotinine (COT) referred as nicotine
metabolized ratio (NMR). Faster metabolizers of nicotine increased the risk for lung cancer.
The aim of this study was to identify the association between urinary nicotine metabolic ratio
with lung cancer patients.Consecutive sampling and case-control study were applied. All
samples were men and smoker or ex-smoker. We investigated NMR by using Liquid
Chromatography tandem-Mass Spectrometry (LC-MS/MS) technique. Statistical analysis was
performed by Chi-Square analysis using SPSS. This study involved 46 samples consisting of
23 cases of lung cancer and 23 healthy controls.There was the significant association between
urinary NMR levels and lung cancer incidence (p= 0,00). Fast metabolizer was 0.115 times
more susceptibility lung cancer incidence than slow metabolizer group (95% confidence
interval (CI): 0.03-0.5). There was the significant association between urinary NMR levels
with lung cancer incidence among smokers.

TIMI Flow between Streptokinase and Alteplase in Successful

Abstract
STEMI with onset≤12 hours, necessary to take Primary Percutaneous Coronary Intervention
(pPCI), but if there is no facility, another therapy is fibrinolytic, to improve blood flow in the
coronary artery and myocardi-al function, thus reducing infarction expansion. This is a
retrospective study in STEMI patients onset≤12 hours, had performed successful fibrinolytic
with Streptokinase (SK) and Alteplase (tPA) from January 2015 to August 2017. TIMI flow
was assessed by coronary angiography. There were 54 patients who had per-formed
successful fibrinolytic therapy with SK and tPA; each group had 27 patients. In the SK group,
there were nine patients (33.3%) with TIMI flow 2 and 18 patients (66.7%) with TIMI flow
3. While in the tPA group there were 11 patients (40.7%) with TIMI flow 2, 16 patients
(59.3%) with TIMI flow 3, (p=0.573). There is no difference in TIMI flow between SK and
tPA on STEMI patients after success fibrinolytic thera-py.

L858R EGFR Mutant Expressions in Triple Negative,

Abstract
The presence of EGFR mutation L858R conferring sensitivity to Tyrosine Kinase Inhibitor in
primary breast cancer has been unclear. Anti L858R EGFR mutant protein specific antibody
was used to screen EGFR muta-tions in 174 formalin fixed paraffin embedded (FFPE)
specimens. Immunohistochemistry revealed strong re-actions to the L858R antibody in 16 out
of 174 specimens. The proportion of positive staining was highest in HER2 (11 of 16),
followed by the triple negative (2 of 55) and luminal (3 of 103) subtypes. However, upon
DNA isolation and analyses, there were six L858R mutations, four L861Q mutations, and
remaining six no mutations. HER2 had the highest proportion of confirmed EGFR L858R
and L861Q mutations (31%; n=16), followed by the triple negative (3.6%; n=55), and
Luminal (1.9%; n=103) cancer subtypes. While EGFR muta-tion was rare, HER2 subtype
had the highest frequency of TKI sensitizing EGFR mutations.

Acetyl salicylic acid resistance and inhibition to platelet
aggregation

Abstract
The adverse side-effects of ASA are mainly gastro-intestinal bleeding. The objective is to
determine if 50 mg ASA is as effective as 100 mg use in normal healthy subjects. The 18
normal subjects had the normal platelet count of mean 301 ± 72.3 x109/L. Significant
(P=<0.001) platelet inhibition was seen in 15 subjects ingesting 50 mg ASA (mean 50.3
±21.9 %) and 100 mg ASA (mean 75.7 ±19.3%). Further significantly enhanced (P=<0.001)
inhibition was seen in 100mg ASA compared to 50 mg. Three subjects were non-responders
to 50 mg ASA (16.7%) and two to 100 mg (11.1%). The study showed that 100 mg ASA
dose is better suited for an-tiplatelet therapy compared to 50 mg. Significantly greater platelet
inhibition was seen with 100 mg ASA with no gastro-intestinal bleeding or other side-effects
reported. ASA resistance was seen in 16.7% of subjects ingesting 50 mg ASA and 11.1% in
100 mg dose.

Infant Feeding Practices and Anemia in 9-Month-Old Infants

Abstract
The incidence of anemia in infants is still high and nutritional deficiencies is suggested as the
main factor due to tue inappropriate feeding practices. To evaluate the relationship between
feeding practices and anemia in 9-month-old infants. A cross-sectional study was conducted
in 6 Puskesmas in Medan from August - October 2016. Chi-square test and multivariat
analysis were used in the statistical analysis. The incidence of anemia was 66.7%. No
significant association between anemia and exclusive breastfeeding (P=0.214, 95 CI).
Variables associated with anemia were starting complementary feeding at < 6-month-old
(P=0.032, PR=2.830, CI 95%), using steamed rice as initial complementary food (CF)
(P=0.021, PR=12 318, 95% CI) and CF not containing animal protein (P=0.013, PR=14 160,
95% CI). Anemia in 9-month-old infants was associated with infant’s age when
complementary feeding was first started, type of CF first given, and animal protein content in
the CF.

Abstract
Domestic Violence is a social problem associated with an increased risk of mental illness in
women. To investigate the association between education level, length of exposure and
economic level with the level of depression among woman experiencing domestic violence.
This study was an observational study with cross-sectional approach. Participants were 82
women experiencing domestic violence seeking support from PPT Bhayangkara Hospital and
Psychiatry Clinic of Pirngadi Hospital Medan. Association between education level and level
od depression (p=0,01), length of exposure and level of depression (p=0,01), and economic
level and level of depression (p=0,01). Factors significantly associated with a higher level of
depression among women experiencing domestic violence were low educational level (OR=
3,754, p=0,022), economic level (OR=4,436, p= 0,01) and length of exposure (OR=4,939, p=
0,018). Awareness needs to be raised through Mental Health Service and government in the
prevention of depression through psychosocial interventions for women experiencing
domestic violence.

Abstract
Skeletal Relating Events (SRE) defined by pathologic fractures, spinal cord compression,
surgery for prevention and treatment of pathologic fractures or spinal cord compression, or
radiation for bone pain. The aims of this study were to determine the incidence of SRE in
Non-Small Cell Lung Cancer (NSCLC) patients who metastasize bone. This is a descriptive
study. A total of 38 patients in this study consisting of 29 (76.3%) men and 9 (23.7%)
women, 31 cases (81,58%) Adenocarcinoma, and 7 cases (18,42%) Squamous Cell
Carcinoma. The incidence of SRE was found in 27 (71,05%) patients who included 21,05%
pathologic fractures, 55,26% spinal compression, 13,16% under bone radiation and only
2.63% undergoing bone surgery. SRE is common in Bone Metastasis NSCLC patients. The
most common SRE is spinal compression.

Abstract
Endothelial progenitor cell (EPC) can differentiate into mature endothelial cells as well as
play a crucial role in the protection and improvement of vascular endothelium. Based on
multivariate regression analysis, there was a strong correlation between the number of
circulating EPC and the subjects' combined Framingham risk fac-tor score. In hyperglycemia,
endothelial damage could occur over time may cause both micro- and macroangi-opathic
complications. The number of EPC was reduced and was inversely related to levels of
glycated hemo-globin (HbA1c) and type 2 diabetes mellitus. Additionally, in
hypercholesterolemic patients had lower EPC score compared to control subjects.

Abstract
To determine an association between matrix metalloproteinase 2 (MMP 2) and tissue
inhibitor matrix metallo-proteinase 2 (TIMP 2) and bone destruction in Atticoantral Type
chronic suppurative otitis media (CSOM) pa-tients at Haji Adam Malik General Hospital.
This analytical cross-sectional study included cholesteatoma from Atticoantral Type CSOM
taken and made into paraffin blocks. These tissues were immunohistochemically ex-amined
by assessing MMP 2 and TIMP 2 immunoreactivity at the anatomical pathology installation.
MMP 2 overexpression (36 sample, 90%), however, samples that overexpressed and did not
overexpress TIMP 2 were relatively even, 21 (52.5%) and 19 (47.5%), respectively. MMP 2
expression was not significantly associated with TIMP 2 (p=0.342). MMP 2 and TIMP 2
expressions were not significantly associated, and TIMP 2 with bone destruction were not
significantly associated in Atticoantral Type CSOM.

Effect of Zinc Supplementation on Linear Growth Velocity of
Children With Short Stature

Abstract
Short stature is still a burden in many parts of the world which can be a result of long-
standing malnutrition. By increasing the availability of zinc, zinc supplementation can
facilitate growth in children. This study aims to determine the effect of zinc supplementation
on linear growth velocity in children with short stature. An open label randomized placebo-
controlled trial was conducted in Singkuang Village, Mandailing Natal Regency, North
Sumatera from March to September 2016. Children aged 7-10 years were screened for short
stature. Subjects were divided into two groups, the intervention group received zinc
supplementation of 10 mg/day, six days a week, and the control group received placebo.
After 6 months of zinc supplementation, there was no statistically significant increase in
height (p=0.493) and linear growth velocity (p=0.365). Conclusion: There is no significant
increase in height and linear growth velocity after zinc supplementation for 6 months in short
stature children.

Identification of HPV Type 6 and 11 in Skin Tag with PCR

Abstract
Infection of human papilloma virus ( HPV ) is suspected as one of the causes of skin tag
lesion. To identify HPV type 6 and 11 in skin tag lesion by PCR technique. This study is a
descriptive study with cross sectional design involving 30 skin tags lesion. Skin tag lesion
will performed by PCR examination to identify HPV type 6 and 11. Furthermore the
collected data then processed and presented descriptively. In this study we found 8 (26,7%)
skin tag lesions identified with HPV type 6 and 6 (20%) skin tag lesions identified with HPV
type 11. Totally from 30 lesions of skin tags were examination with PCR technique, 14
(46,7%) lesions identified with HPV type 6 and 11.

Parotid gland tumor is the most common found salivary gland malignancy. Although most
cases are reported to be benign, in developing countries, malignant cases are also commonly
found. This case report presented cases of benign, malignant, and recurrent parotid gland
tumor in Haji Adam Malik Hospital. Parotidectomy remained the cornerstone of the therapy.
Radiotherapy and chemotherapy could be promising in advanced cases.

Ascariasis Incidence in Children who Received Single and
Repeated Educational Lecture

Abstract
WHO recommended albendazole 400 mg to eradication ascariasis. Despite effective
treatment options, re-infection following treatment alone is inevitable. A case-control study
was conducted to investigate the difference of incidence ascariasis in children who received
education on sanitation and personal hygiene once with recurrent in Singkuang village, South
Tapanuli, North Sumatera from April to October 2016. Subject were elementary school
children selected by cluster sampling technique. Statistical analysis was performed by Chi-
square and Fisher exact test. Of 170 elementary school children were divided into two
groups. 85 subjects in the control group. The incidence of Ascariasis was found to be 24% in
the control group and 3% in the case group. Children who received recurrent education
tended to be 8.4 times more likely not to occur ascariasis. There were significant differences
in the incidence of both group (P=0.001).

Abstract
Detention is one of the problems that has not received serious attention in the course of
reform of the criminal justice system in Indonesia. Losing freedom for a person can cause a
decrease in dignity and self-esteem so that stress appears to the individual. Also, the changes
that occur in the life of a prisoner who is full of pres-sure and loss can increase the tendency
of depression.

Relationship Between Habitual Snoring and Primary Enuresis
in Children

Abstract
Obstructive sleep disorder breathing is assumed to be associated with primary enuresis in
children. Prolonged enuresis may cause developmental disorders, poor school performance,
and emotional disorders. A cross-sectional study was conducted in Muara Batang Gadis
district, North Sumatera in April 2016. The samples of the study were children aged 5-14-
year-old. Sleep Disturbance Scale for Children (SDSC) questionnaire was used for measuring
the symptoms of sleep disorder breathing and International Relationship Child and Ado-
lescent Psychiatry and Allied Professions (IACAPAP) questionnaire for the presence of
primary enuresis. Da-ta were analyzed using chi-square test and logistic regression. The P
value of <0.05 was considered signifi-cant. 110 children, 27 (24.5%) snored more than three
nights per week (habitual snorers), and 18 (16.4%) had primary enuresis. The habitual snorers
with the history of primary enuresis outnumbered non-habitual snorers (9.1% versus 15.4%)
with the P-value <0.05. There is a significant relationship between habitual snoring and
primary enuresis.

Proportion of Ameloblastoma's Subtypes Based on Its
Location, Size, and Imaging

Abstract
Ameloblastoma is the most common odontogenic tumor of odontogenic epithelial origin. The
objective of this study is to investigate if there was any dominant subtype at a specific
location, size, and radiological imag-ing.This is the descriptive study with cross-sectional
approach. The samples of this study were determined by histopathological subtype then
correlated with its location, size and radiological imaging from the medical record. Based on
data from 25 samples in this study showed in mandible most frequent is a follicular pattern,
based on the size, small size (<5 cm) was dominated by plexiform pattern while large size
(>5 cm) was by the follicular pattern. The multilocular imaging was dominated by follicular
pattern while the unilocular has the follicular and plexiform design in the same portion. The
follicular pattern was the most frequently found in the mandible, a large size of the tumor and
multilocular radiological imaging.

Abstract
Presence of BCR-ABL1 transcript is essential in establishing Chronic Myeloid Leukemia
(CML). In the absence of BCR-ABL1, JAK2 V617F mutation testing is performed to support
the diagnosis of polycythemia vera (PV), essential thrombocythemia (ET), and primary
myelofibrosis (MF). We wanted to test the feasibility of multiplex detection of BCR-ABL1
transcript variants and JAK2 V617F mutation simultaneously using Reverse Dot-blot
Hybridization (RDB) method. Limit of detection (LOD) or analytical sensitivity of RDB
method using cDNA specimens was 0.5% and 6.25% in detecting BCR-ABL1 and JAK2
mutant transcripts, respectively. Diagnostic specificity and sensitivity to detect BCR ABL1
and JAK2 were 100% and 85.7%; 92% and 100%, respectively. RDB also detected BCR-
ABL1 transcripts in 22% of JAK2 V617F mutation positive samples (N=36). RT-PCR RDB is
a promising qualitative multiplexing method to detect simultaneous BCR-ABL1 and JAK2
mutant transcripts.

Diagnostic for TTNA Using a Thoracic Ultrasound Guidance
for Diagnosing Lung Cancer

Abstract
Transthoracic Needle Aspiration (TTNA) for diagnosing thoracic lesions are usually
performed with image guidance, including thoracic computed tomography scan and thoracic
ultrasound. The aim of this study is to describe the characteristics of samples, and diagnostic
accuracy of TTNA using US guidance compare to CT guidance. Image-guided TTNA were
performed in 91 consecutive patients suspected of lung cancer on clinically and radiological
work-up. Image guidance was selected randomly. The final diagnosis was based on cytology
analysis or clinical follow-up. For thoracic ultrasound guided TTNA, the accuracy,
sensitivity, specificity, were 87.8%, 91.6%, and 77.7%, respectively. For thoracic CT-guided
TTNA the accuracy, sensitivity, specificity, were 86.84%, 78.3%, and 100%. Thoracic US-
guided TTNA had a high sensitivity and specificity in lesions that abut the chest wall

Abstract
A common polymorphism, Arg257Cys of CYP2A13 gene, is associated with lung cancer in
several populations. The aim of this study was to analyze the relationship between this
polymorphism and lung cancer in several ethnics of Indonesia population. Consecutive
sampling and case-control study wsere applied. The PCR-RFLP assay was employed to
genotype. The chi-square test with p<0.05 considered as significant. Of all 50 subjects, 26
(13%) individuals were heterozygotes, and 37 (74%) were homozygote for the 257Cys allele.
The frequency distributions among ethnics were Acehnese 2 (5.4%) homozygote, Malay 2
(5.4%) homozygote, Chinese 1 (7.7%) heterozygote, 20 (54%) homozygote and 6 (46.1%)
heterozygotes in Batak, 13 (35.1%) homozygote and 6 (46.1%) heterozygotes in Javanese.
The CYP2A13 Arg257Cys variant represents a common polymorphism in Batak and
Javanese ethnics. There is no association between Arg257Cys polymorphism of CYP2A13
and lung cancer.

Presence of Helicobacter pylori of Chronic Tonsillitis tissue in
Medan in 2013-2017

Abstract
Helicobacter pylori (H.pylori) is a well-known pathogen which is recognized as the most
frequent infection of individuals. Helicobacter pylori play important role in the etiology of
gastritis, gastric and duodenal ulcers. We investigated the presence of Helicobacter pylori as
a reservoir in the tonsillectomy specimens. To determine the presence of Helicobacter pylori
of chronic tonsillitis in tonsillectomy specimens. We Collected 43 cases of chronic tonsilitis
were examined in tonsillectomy specimens for which available tissue for routine staining
could be retrieved histopathology chronic tonsillitis. The colonization of the helicobacter
pylori has been eval-uated with hematoxylin & Eosin and Giemsa stains under the light
microscope. With H&E staining, Helico-bacter pylori have been detected in 23 cases
(53,48%) of the tonsillectomy specimens in total and with Giemsa staining Helicobacter
pylori has been detected in 29 cases (67,74%) of the tonsillectomy specimens in total.

Abstract

Microsomal epoxide hydrolase 1 (EPHX1) plays an important role in both activation and the
detoxification of polycyclic aromatic hydrocarbons (PAH) and aromatic amines.
Polymorphism EPHX1 His139Arg in susceptibility to lung cancer has been reported with
inconsistent outcomes. The aim of this study was to analyze the relationship between this
polymorphism and lung cancer in smokers. Consecutive sampling and case-control study
were applied. Genotyping was performed by the PCR-RFLP assay. The chi-square test with
p<0.05 considered as significant. Of all 84 subjects, in case and control groups, wild-type
variant His139His were 34 (81%) and 30 (71.4%), heterozygote variant His139Arg were 8
(19%) and 12 (28.6%), there was no homozygote variant Arg139Arg identified. (p=0.36).
The EPHX1 His139His represents a common polymorphism in both of subject groups. There
is no association between His139Arg polymorphism of EPHX1 and lung cancer.

Effectiveness of Vitamin A in Albendazole Against
Reinfection of Ascaris Lumbricoides

Abstract
WHO recommends the supplementation of 200,000 IU vitamin A into 400 mg albendazole to
cut down its re-infection. A Randomized Controlled Trial was conducted in SD N 384
Sikapas, North Sumatera from April - June 2016. Inclusion criteria were children with
ascariasis. Sample divided into intervention and control group. Kato-Katz was used to
identify egg worms and faecal egg count intensity (FECs). P value < 0.05 was consider
significant. After three months follow up, there were 51 and 56 samples in intervention and
control group with average age 9.6 years (SD 1.99) and 9.7 years (SD 2.20). Prevalence of
ascariasis was 75.5%. There was no significant difference in Ascaris lumbricoides reinfection
but instead of in FECs intensity. The NNT was 22.31. The addition of vitamin A in
albendazole was not suggested according to this study.

Risk factor of Femoral Diaphysis Nonunion Fractures at Haji

Abstract
Nonunion incidents estimated between 5-10% of all fractures. In recent literature, femoral is
more commonly nonunion with incidence 54.68%. The aim of this study was to know the risk
factor of femoral diaphysis nonunion.This research using descriptive cross-sectional of
medical record with femoral diaphysis nonunion in January 2015 - December 2016 including
age, sex, an incidence of fracture, history of treatment, alcohol consumption, and smoking.
Of 30 patients, 17 men (57%) and 13 women (43%). Most ages 18-30 years=15 patients
(50%), 31-45 years=8 patients (27%), 46-60 years=3 patients (10%), and >60 years=4
patients (13% ); 25 patients (83%) did alternative treatment and 5 patients (17%) went to the
hospital; Open fractures 19 patients (63%) and closed fractures 11 patients (37%); 16
alcoholics (53%) and 14 nonalcoholics (47%); 20 smokers (67%) and 10 non-smokers (33%).
In conclusion, nonunion high in the society that doing alternative treatment.

Management of Palpebral Epidermoid Cyst with Tenzel
semicircular Flap Procedure

Abstract
Epidermal cyst is a benign cyst that develops out of the ectodermal tissue. The epidermal cyst
may have no symptoms. The definitive diagnosis is made after excisional biopsy based on
microscopic appearance. A 49-year-old woman presented with a painless left lower eyelid
mass. Examination revealed a firm, globular mass that was fixed to the tarsal plate of the
lower eyelid. Excisional biopsy performed and revealed epidermal cyst. As with epidermal
cyst in other location, surgical excision is the treatment of choice. Tenzel semicircular flap
procedure is selected for eyelid reconstruction in this case. Histopathologic sections
demonstrated a cyst line by squamous epithelium containing keratin, consistent with the
diagnosis of an epidermal cyst. Epidermal cyst of the lower eyelid in moderate size was
successfully managed by surgical excision with tenzel semicircular flap procedure and give
the best outcome for the palpebral epidermal cyst especially in moderate size.

Influence of Smoking With Proximal Femur Bone Density in

Abstract
Singh index is the classification of a trabecular pattern on proximal femur which is observed
with x-ray. Smoking has been found since 1976 that can decrease bone density. This is a
cross-sectional study performed at Haji Adam Malik Medan Hospital. Samples were divided
into two groups, smoking and non-smoking groups with 43 persons in each group. Then the
Singh index value was evaluated from the pelvic x-ray in all samples. There was a significant
correlation between smoking & the Singh index value (p=0,002). The duration of smoking
directly affects the Singh index (p=0,001). But there was no correlation between the cigarette
type (p=0,932) & the frequency of smoking (p=0,259) with the Singh index. It has been
found that smoking has a significant correlation with the Singh index value, where the
smoking group has a lower Singh index value compared to the non-smoking group.

Abstract
Across the world, the prevalence of childhood overweight and obesity is high. The influence
of several fac-tors, including genetic, cultural, and environmental factors such as sedentary
activity in children has been ex-tensively described. This study aims to determine the
association between sedentary activities and parental obesity with obesity in children. A case-
control study was carried out at ten elementary schools in Medan. The data about sedentary
life style and parental obesity were obtained by giving questionnaires to parents or guardians.
Data were analyzed by chi-square, p<0.05 was considered as statistical significant. There
were 212 children, aged 6-12 years, were recruited, 105 children with obesity as case group
and 107 normal weight children as the control group. This study found a significant
association between parental obesity and seden-tary activities with obesity in children
(p<0,01). It is important to do prevention efforts to prevent obesity in children.

Abstract
Diagnosis and treatment of childhood cancer is a challenge and a source of stress for children
and their moth-ers. Blood cancer is currently the leading cause of cancer death among
children under 15 in the United States. While the number of cancer victims in young children
continues to grow. The results show that despite the fact that many mothers adapt well to the
diagnosis of childhood cancer, there are still a large number of par-ents who struggle with
fatigue, anxiety, post-traumatic symptoms and stress after treatment of childhood can-cer.

Abstract
To analyze the association of p53 and muscle invasiveness in bladder cancer. The samples of
this study are the patients with bladder cancer. Samples then classified into Non-Muscle
Invasive Bladder Cancer (NMIBC) and Muscle Invasive Bladder Cancer (MIBC). All
samples then underwent immunohistochemis-try assay for p53, then analyzed group. p53 was
expressed in 29 (96.7%) patients of MIBC and 23 (76.7%) patients of NMIBC. There was a
significant difference in the expression of p53 between both groups (OR 8.8; 95% CI: 1.012-
76.96). Expression of p53 was significantly associated with muscle invasiveness of blad-der
cancer. Prospective study needed to determine its potential in predicting muscle invasiveness
of blad-der cancer.

The Role of Simplified Selvester Score as Predictor of
Successful Fibrinolytic in STEMI

Abstract
Fibrinolytic is one of the modalities of treatment for STEMI patients. The successful of
Fibrinolytic has been associated with better clinical outcomes. We sought to explore whether
the simplified Selvester Score electrocardiographically could predict successful fibrinolytic
therapy. 53 STEMI patients who received fibrinolytic therapy with streptokinase were
enrolled this retrospective study. The Simplified Selvester Score was estimated on the first
admission ECG. STEMI patients were divided into two categories; successful or failed
fibrinolytic by ECG. The optimal Cutt-off simplified Selvester Score was analyzed. Patients
with successful fibrinolytic have lower Simplified Selvester Score (2,3+1,6vs5,1±1,7;
p=0,001). In multivariate analysis, Simplified Selvester Score <3,5 remained significantly
associated for successful fibrinolytic and appeared to be the strong predictor for successful
fibrinolytic (p=0,004: OR=6,8;95%CI:1,55 – 37,05). The Simplified Selvester Score ≤ 3,5
can be used as a predictor of successful fibrinolytic in STEMI patients.

Comparison of Thyroid Hormones Level Between Severe and
Mild-Moderate Malnutrition

Abstract
Malnutrition is one of major health problems in developing countries and contributes to
children mortality in the world.Thyroid hormones hold the crucial role in metabolism which
used for growth and matura-tion.Objective: To understand the differences in thyroid
hormones level between severe and mild-moderate malnutrition.Methods: This was a cross-
sectional study from April-May 2016 in Adam Malik General Hospi-tal, Medan.Subjects are
1-5 years old with severe and mild-moderate malnutrition.Children with hypothy-
roid/hyperthyroid/took medicine that could affect thyroid hormones level were excluded.Data
were analyzed using independent T and Mann Whitney U Test with 95% CI. P value<0.05
were considered statistically sig-nificant.Result: Level of T3 was different between severe
and mild-moderate malnutrition.Level if T3 was lower in severe malnutrition than Mild-
moderate malnutrition (p <0.001, 95% CI 0.29 – 1.00) but the T4 level was not lower (p
0.065, 95% CI -0.32 – 3.00). Conclusion: There was a significant difference in T3 level be-
tween severe and mild-moderate malnutrition, where the T3 level was lower in severe
malnutrition.

Relation of Ferritin and Nutritional to Pulmonary Dysfunction
in Thalassemia Major

Abstract
Thalassemia major is a chronic disease requiring regular transfusions that may result in iron
loading, iron excess complication, such as heart, liver, endocrine organs, and lungs diseases.
Restrictive pulmonary dysfunction is known as the dominantly observed. Pulmonary function
test was conducted in 50 children aged 6-18 years with TM with no history of a chronic
respiratory disease. Assessment of nutritional status based on the anthropometric examination
(BW/H and MUAC/A). Among the 50 children, 18 (36%) had restrictive lung function. The
mean ferritin levels of the restrictive pulmonary dysfunction group (2096.6 ug/L) was lower
than the normal pulmonary function group (2209.6 ug/L). Nine of 18 children were mildly
malnourished in the restrictive pulmonary dysfunction group (OR=6.890; CI95% = 1.152-
41.340). There is no significant correlation between serum ferritin level and restrictive
pulmonary dysfunction in children with TM. Mild malnutrition increases the risk of
restrictive pulmonary dysfunction occurrence in pediatric TM.

Abstract
The structural changes underlying permanent noise-induced hearing loss (NIHL) and smoke
include loss of the sensory hair cells damage to their stereocilia and supporting tissues within
the cochlear lateral wall through cell death pathway. 1% -3% of all oxygen consumed by the
body will be converted into superoxide. All sources of superoxide, are produce the
superoxide dismutase allour to neutralize free radical superoxide in the cells. One molecule of
SOD take two molecules of superoxide. In humans, SOD exists in three forms and functions
in various locations throughout the body. We divided randomly thirty-three Rattus norvegicus
into groups (n=6). Group 1: control; Group 2: noise 50-80 dB; Group 3: smoke 3-6
cigarettes/day ; Group 4: noise 85-110 dB; Group 5: 7-12 cigarettes/day. All samples were
immunohistochemistrycally examined for the expressions of SOD in cochlear. The results
showed that significant differences for the expressions of SOD (p < 0.001) between groups,
significant differences for the expressions of (p < 0.005) in groups.

Abstract
Synthetic analgesic drugs have fatal side effects. This study aims to prove the effect of
Bangun-bangun leaves on the decrease of excitatory pain. The experimental animals used
were 24 mice divided into four groups. The results were analyzed using SPSS and ANOVA
then post hoc Tukey analysis. The decrease of excitatory pain in all treatment groups (I, II, III
and IV) was significantly different, whereas P=0.000. This study concluded that the Bangun-
bangun leaves both in the form of water extract and ethanol extract proved efficacious as a
pain reliever in the mice after induction of acetic acid. Even the group-IV (EDBB) has the
best efficacy as analgesic compared to the group-II (positive control/paracetamol) and group-
III (DBB) in decreasing the writhing response and migration of leukocyte to the inflamed
tissue.

Abstract
Tumor necrosis factor alpha (TNFα) is a regulator of the inflammatory process and is
considered important as an endogenous tumor promoter. TNFα Gene -308 G/A
polymorphism is involved in gene expression and has been linked to malignancy. The aim of
this study was to analyze the relationship between this polymorphism and lung cancer in
smokers. Consecutive sampling and case-control study wsere applied. Genotyping was
performed by the PCR-RFLP assay. The chi-square test with p<0.05 considered as
significant. Seventy-four subjects enrolled in this study. The homozygote allele (GG) 89,2%
of case group and 97,3% healthy group. The allele variant of TNFa -308 was found more in
the lung cancer patient (10,8%) compared with healthy groups (2,7%). There was no
association between TNFα gene -308 G/A polymorphism with lung cancer in Adam Malik
General Hospital.
