AFFIDAVIT

I, RYAN MARK EVA FABUL, Filipino, of legal age, single, and a resident of #55
RSG Condo Ermin Garcia St., Brgy. Pinagkaisahan, Quezon City, Philippines, after
being sworn in accordance with law, depose and state:

1. That I am a duly licensed driver with Driver's License No.N02-08-010625, which

is valid until the year 2023-01-21 and usually driving a motor vehicle, a 2009
model Subaru Impreza bearing a licensed plate number NVO-606, registered
under the name of LIU YICHIA, a resident of B2-D Alpha Victoria Homes, New
Manila, Quezon City;

2. That on March 31, 2018, at around 7:30 AM, I was about to left turn heading
towards to the direction of Edsa/Cubao coming from Don Alfredo Eguia St.,
when a passenger jitney, with plate number CTV-251 (registered
owner/operator: Ernie Bautista), driven by LORENCE CADAGAN BAUTISTA,
approaching in a fast pace, suddenly bumped into the right front side portion to
the vehicle I was driving, thereby causing damage to the same;

3. The I caused pecuniary damage for the repair of the vehicle, including serious
inconvenience due to reckless driving of Mr. LORENCE BAUTISTA;

4. At first, Mr. Lorence Bautista was coordinating with me and in fact verbally
agreed upon that he will pay the damage sustained by the vehicle at the most
convenient term, after the confrontation before the investigating officer, PO3
Joel Y. Aviso of Station 10, PNP who documented the incident in the Blotter of
the Traffic Accident Investigation Report;

5. Unfortunately, months have already passed, Mr. Bautista was no longer

responding to my calls and text messages, apparently, to evading his
obligation.

6. In view of the foregoing, I am executing this Affidavit to attest to the truth of the
foregoing and in support of charges for "RECKLESS IMPRUDENCE" resulting
in damaged of property against the driver of the said jitney or for whatever other
action that may be filed against the driver and/or registered owner of the said
vehicle and in support of claims against the insurer for the reimbursement of
the expenses incurred for the repair of the damage caused to the vehicle I was
driving.
