Ringkasan Riwayat Penyakit

 Pasien rujukan dari RS Damanhuri Barabai dengan diagnosis Guillen Barre Sindrom. Dirawat
di RSUD Barabai selama 4 hari. Di RSUD Barabai mendapatkan terapi:
1. Inj. Ceftriaxon 3x1 gr (H.4; 7x)
2. Salbutamol 3x2 mg P.O (H.4; 11x)
3. Asetilsitein 3x1 tab P.O (H.3; 7x)
4. Azitromisin capsul 1x1 P.O (H.2; 2x)
5. Nebuliser NS 3 ml 2x/hari (H.2; 3x)
 Pasien dirujuk ke RS Ulin sabtu malam (15/09/2018) dan masuk ke bangsal anak lantai 2 divisi
neurologi senin (17/09/2018) dengan diagnosis mielitis tranversa.
 Pasien mengeluh lemah pada tangan dan kaki yang muncul 11 hari SMRS. Lemah dimulai pada
tangan kanan, tangan kiri, lalu kemudian kedua kaki. Lemah ini muncul ketika pasien sehabis
olahraga di sekolah, lalu pulang ke rumah dan setelah mengganti baju pasien merasa tidak bias
menggunakan tangannya.
 Sebelum merasakan lemah pasien ada mengeluh demam tinggi 1-2 hari. Demam dirasakan
muncul tiba-tiba dan turun dengan pemberian paracetamol.
 Pasien juga ada mengeluh batuk berdahak yang dirasakan 1 minggu sebelum munculnya lemah
pada tangan dan kaki.
 Pasien kemudian dirawat di RS Barabai, dan pada hari pertama perawatan, pasien mengeluhkan
nyeri tenggorkan dan sakit menelan.
 Pasien tidak ada mengeluhkan gangguan BAK dan BAB.
 Keluhan sekarang: pasien masih merasakan lemah pada kedua tangan dan kaki, batuk berdahak,
dan nyeri tenggorokan.

 Kesadaran : Composmentis
 GCS : E4V5M6
 Mata : Reflek cahaya (+/+), pupil isokor, diameter (2 mm/2 mm)
 Leher : Kaku kuduk dan rangsang meningeal (-)
 Lesi nervus kranialis : (-)
 Ekstrimitas : Lateralisasi (-), spastis (-), klonus (-), tonus agak menurun pada
kedua kaki

Summary of Disease History

 This patient was referred from Damanhuri General Hospital, Barabai with a diagnosis of Guillen
Barre Syndrome. Treated at Barabai Hospital for 4 days. In Barabai Hospital, they get therapy:
1. Inj. Ceftriaxon 3x1 gr (H.4; 7x)
2. Salbutamol 3x2 mg P.O (H.4; 11x)
3. Acetylsitein 3x1 tab P.O (H.3; 7x)
4. Azithromycin capsules 1x1 P.O (H.2; 2x)
5. NS Nebuliser 3 ml 2x/day (H.2; 3x)
 The patient was referred to Ulin Hospital on Saturday night (15/09/2018) and entered the 2nd
child ward in the neurology division of Monday (17/09/2018) with a diagnosis of tranverse
myelitis.
 The patient chief complain is weakness in the hands and feet that appear 11 days before
admission. Weakness starts in the right hand, left hand, then then both legs. This weakness
appears when the patient after exercise at school, then returns home and after changing clothes
the patient feels unable to use his hands.
 Before feeling weak, the patient complains of 1-2 days of high fever. Fever was felt suddenly
and dropped with paracetamol.
 Patients also complain of cough that is felt 1 week before the appearance of weakness in the
hands and feet.
 The patient is then treated at Barabai Hospital, and on the first day of treatment, the patient
complains of sore throat and swallowing pain.
 The patient not complains of loss of bowel or bladder control.
 Current complaints: patients still feel weakness in both hands and feet, cough, and sore throat.
