JUDICIAL AFFIDAVIT

I, CARDO DALISAY, Filipino, of legal age, married and with residence

and postal address at #34 Balimbing St., General Santos City, after having been
sworn in accordance with law, depose and say that:

Preliminary Statement:

The person examining me is Atty. Princess Rosshien L. Hortal with address

at 2/F GSC Investment Action Center Bldg., General Santos City. The examination
is held at the same address. I am answering her questions fully conscious that I do
so under oath and may face criminal liability for false testimony and perjury.

Questions and Answers:

1. Please state your name and other personal circumstances for the record.
A: I am Cardo Dalisay, of legal age, married to Aleana Dalisay and with
residence and postal address at #34 Balimbing St., General Santos City,
and currently employed as Principal I in Walang Pasok Elementary
School.

2. Are you the same CARDO DALISAY, the petitioner in this case?
A: Yes, sir. I am CARDO DALISAY, the petitioner in this case.

3. As a petitioner, under what circumstances have you acquired the facts by

which now you are testifying?
A: Through my personal knowledge of the facts, Ma’am.

4. What is the subject of your petition?

A: The petition is about the re-issuance of the TCT No. 1 in the name of
Cardo Dalisay.

5. Why did you file the instant petition?

A: My original copy of TCT No.1 was lost.

6. You said your original TCT No.1 was lost, how did you lose it?
A: While on board a bus from Koronadal City to General Santos City, last
August 10, 2018, I fell asleep until the bus arrived at the terminal. I hurriedly
got up because I was the only passenger left and so I forgot to bring with the
envelope which contains the original copy of TCT No.1.

7. After losing the original copy of TCT No.1, what did you do afterwards?
A: I executed an Affidavit of Loss.
 8. I am now showing you a copy of Affidavit of Loss. Is this the same
Affidavit of Loss you are referring to?
A: Yes, Ma’am. That is the same Affidavit of Loss.

A copy of the BL signed Application Form is hereto and made an integral

part hereof and marked as Exhibit “B.”

9. Do you affirm and confirm the truthfulness and veracity of all the
statements you stated in this judicial affidavit?
A. Yes, Ma’am.
