8 Wellsite Permit to Work System Procedures


1.0 GENERAL

1.1 PURPOSE
The Wellsite Permit to Work System Procedures (“Procedures”)
are a planning and work control system designed to assist all
personnel working on Australian onshore wellsites, to carry out
work in accordance with safe working practices. The Procedures
are to be used in conjunction with all applicable legislative and
regulatory requirements and are subject to those requirements.

1.2 SCOPE
The Procedures apply to work carried out by Operators and
associated Wellsite Contractors (as defined in Section 10
Glossary):
• On Australian onshore wellsites that have been handed
over from the Operator’s Production group, or
• Any other associated activities as conducted by the Wellsite
Contractor.
It is the responsibility of all personnel to ensure these Procedures
are carried out.
These Procedures do not apply to:
• Operations where the permit to work system of the
Operator takes precedence.
• Seismic operations where they do not impact other
operations or installations.
• Construction activities in a fenced off area separate from
producing plant and free of hydrocarbons.
• Sites managed and operated by third parties where the
Operators and/or Wellsite Contractors personnel have no
direct operational control.

It is however expected that the above activities will be conducted
under a similar planning and work control system.
Where it is unclear whether or not the Procedures apply, the
matter shall be referred to the Operator for a decision.

1.3 DEFINITIONS
WELLSITE PERMIT TO WORK - sets out the work to be done, the
precautions to be taken, and specifies all Work Conditions.
WELLSITE PERMIT AUTHORITY - the person authorised to issue
WELLSITE PERMITS TO WORK.
OPERATOR REPRESENTATIVE - the person authorised to act on
behalf of the Operator relating to the management of wellsite
activities.
WELLSITE PERMIT HOLDER - the person who receives the
WELLSITE PERMIT TO WORK.
WELLSITE WORK PARTY - the people whose work is covered by
the WELLSITE PERMIT TO WORK.

1.4 RESPONSIBILITIES
It is essential that the WELLSITE PERMIT AUTHORITY, the
OPERATOR REPRESENTATIVE, the WELLSITE PERMIT HOLDER and
all members of the WELLSITE WORK PARTY clearly understand
their responsibilities.
For rig related activities, the WELLSITE PERMIT AUTHORITY will
be the Rig Manager/Toolpusher or his designate.
For other activities or circumstances responsibilities must be
assigned at a meeting between the appropriate Operator
personnel and senior representatives of all Wellsite Contractors
expected to participate in the activity before that activity
commences (before the Wellsite Permit to Work is issued).

WELLSITE PERMIT AUTHORITY
• With the WELLSITE PERMIT HOLDER, shares the
responsibility for filling in Section A - SCOPE on the
WELLSITE PERMIT TO WORK.
• Defines and carries out Prestart work precautions (Section
B - PRESTART on the WELLSITE PERMIT TO WORK).
• Defines ongoing Work Conditions (Section C - ONGOING
on the WELLSITE PERMIT TO WORK).
• Ensures that the WELLSITE PERMIT HOLDER and all
members of the WELLSITE WORK PARTY understand all of
the WELLSITE PERMIT TO WORK terms and conditions.
• With the WELLSITE PERMIT HOLDER, completes Section D -
ISSUE/CLOSE OUT on the WELLSITE PERMIT TO WORK.
OPERATOR REPRESENTATIVE
• The OPERATOR REPRESENTATIVE must also ensure that all
the responsibilities as listed for the WELLSITE PERMIT
AUTHORITY and WELLSITE PERMIT HOLDER have been
satisfied and the WELLSITE PERMIT TO WORK activities do
not interact with future activities.
• Where an OPERATOR REPRESENTATIVE has not been
assigned to a well activity, the WELLSITE PERMIT AUTHORITY
must be briefed by the Operator’s wellsite group Manager/
Superintendent/ Senior Supervisor on the requirements as
they pertain to the WELLSITE PERMIT TO WORK.
• The OPERATOR REPRESENTATIVE must be satisfied that all
Wellsite Contractors have appropriate procedures prepared
and in place, JSAs performed, and be satisfied that Wellsite
Contractor personnel are appropriately trained and/or have
a good understanding of these Procedures and JSAs.
WELLSITE PERMIT HOLDER
• With the WELLSITE PERMIT AUTHORITY, shares the
responsibility for filling in Section A on the WELLSITE
PERMIT TO WORK.

• Ensures the WELLSITE WORK PARTY understands all the
WELLSITE PERMIT TO WORK terms and conditions.
• Ensures the WELLSITE WORK PARTY abides by the WELLSITE
PERMIT TO WORK terms and conditions.
• With the WELLSITE PERMIT AUTHORITY, completes Section
D on the WELLSITE PERMIT TO WORK.
• Responsible for completing the Additional Protective
Measures Checklist on the back of the WELLSITE PERMIT
TO WORK before the WELLSITE PERMIT TO WORK is issued.
• Satisfy themselves that the Work Precautions have been
carried out.
WELLSITE WORK PARTY
• Must understand all the WELLSITE PERMIT TO WORK terms
and conditions.
• Each member of the Work Party signs the Work Party
Acceptance of the WELLSITE PERMIT TO WORK.
• Abide by all WELLSITE PERMIT TO WORK terms and
conditions.

1.5 DEVIATION
Any deviation from the Procedures must be authorised in
writing.
A deviation from the Procedures for a particular task or job under
one permit requires the approval of the applicable WELLSITE
PERMIT AUTHORITY’s management in consultation with the
respective Operator, after an appropriate Risk Hazard Analysis
has been performed.
Standing deviations require the approval of the applicable
WELLSITE PERMIT AUTHORITY’s management after an
appropriate Risk Hazard Analysis has been performed. Standing
deviations, once approved, must be effectively communicated
to all parties potentially affected by the standing deviation and
a copy must be kept on file at each wellsite location.

Copies of all standing deviations must be provided to the Wellsite
Permit to Work System Steering Committee within one month,
for review.

1.6 REVIEW OF PROCEDURES


These Procedures are subject to review by the Wellsite Permit
to Work System Steering Committee and must be reviewed every
two years in their entirety. Changes can be initiated via the
Wellsite Permit to Work Change Request Form (refer to Appendix
C) and submitted to the Wellsite Permit to Work Steering
Committee (“Committee”) for consideration.
1.6.1 Wellsite Permit to Work Steering Committee Charter
Committee Membership
The Committee shall be composed of representatives from
Australian onshore Operators and Wellsite Contractors. The
Committee will appoint a chair.
Should a Committee representative be unable to attend a
meeting, a proxy should be appointed and attend that meeting.
The Committee shall conduct reviews of the Wellsite Permit to
Work System and meet as nominated by the chairman.
Minutes of meetings are to be circulated to the appropriate
management of the Committee members and other Australian
onshore Operators and Wellsite Contractors as deemed
appropriate.
Responsibility
The actions and decisions of the Committee are subject to review
by the respective management of the Committee
representatives.
Committee Role and Duties
The role of the Committee is to oversee the maintenance and
development of the Wellsite Permit to Work System Procedures,
specifically the following:

• Focal point for the review of existing procedures and
introduction of new procedures as required, for example
by OH&S legislation, or new equipment.
• To maintain a register of Operators and Wellsite Contractors
implementing and using the System.
• To maintain consistent interpretation and implementation
of the Procedures.
• Ensure that all new and amended Procedures are
disseminated to all concerned.
• Oversee maintenance of the WELLSITE PERMIT TO WORK
forms used.
• Oversee the process of training and competency testing of
new and existing WELLSITE PERMIT AUTHORITIES and
OPERATOR REPRESENTATIVES.
• Stay abreast of current and pending changes to work
management systems.
• Audit the use of the Procedures at various locations and
steward compliance.
• From time to time the Committee shall appoint teams to
work on specific issues.
• Seek feedback on the usage of the Procedures from the
relevant Operators and Wellsite Contractors and other
persons involved in the implementation of the Procedures.

2. WELLSITE PERMIT TO WORK SYSTEM
PROCEDURES

2.1 OBJECTIVES
The objectives of the Procedures are to ensure that:
1. People, environment, equipment and product are
protected.
2. Safe working practices are followed whilst work is being
carried out.
3. The work site is inspected and all necessary precautions
have been considered and implemented before work
commences.
4. The work is performed only on the specified equipment.
5. The location of people and the type of work being carried
out are always known should an emergency occur.
6. The person(s) carrying out the work understands the
conditions under which work may be carried out.
7. The equipment to be worked on has been correctly
prepared for the work to be carried out.
8. Correct protective measures are available and are being
used.
9. The work being done is carefully monitored.
10. The work site is left in a clean and safe condition upon
completion of the work.
11. The details of non-completed work are documented and
communicated to the WELLSITE PERMIT AUTHORITY and
OPERATOR REPRESENTATIVE as appropriate.
12. Equipment left unserviceable is correctly tagged and
identified to incoming shifts.

2.2 ISSUING AUTHORITY
A WELLSITE PERMIT AUTHORITY must be available for each
wellsite. The WELLSITE PERMIT AUTHORITY issues WELLSITE
PERMITS TO WORK and is aware of all WELLSITE PERMITS TO
WORK issued within their area of responsibility.
For each wellsite there is only one WELLSITE PERMIT AUTHORITY
responsible for work activities on that wellsite.

2.3 OPERATOR REPRESENTATIVE


The OPERATOR REPRESENTATIVE is authorised to act on behalf
of the Operator with regard to the management of the wellsite
activities. They must countersign the WELLSITE PERMIT TO
WORK, or if impracticable refer to Section 1.4.

2.4 WELLSITE PERMIT AUTHORITY QUALIFICATIONS


To qualify as a WELLSITE PERMIT AUTHORITY, an individual must
meet the following criteria:
• Understand the intent of government, Operator and
contractor regulations, standards and safe working
practices related to the petroleum industry.
• Have work experience in various oil or gas operating areas.
• Have a sound understanding of wellsite activities.
• Complete competency requirements in the following (or
equivalent) training:
• Gas Testing and Monitoring.
• Equipment Isolations.
• Confined Space.
• Wellsite Permit to Work Issue.
• Work Safety.
• Explosive and Radioactive Work.
• Pressure Systems Work.

• Know how to conduct Risk Hazard Analysis (including
Stepback or equivalent)
• Know how to perform a Job Safety Analysis (JSA) and be
able to determine when a JSA is required, including a
Confined Space Risk Assessment.
• Be issued with a WELLSITE PERMIT AUTHORITY Certificate,
Card or other suitable documentation by the respective
manager in the individual’s name, and stating the
categories of work for which permits may be issued. Such
a certificate is valid for a maximum of two years, and may
be re-endorsed upon expiry subject to successful completion
of follow up training or competency assessment.

2.5 OPERATOR REPRESENTATIVE QUALIFICATIONS


To qualify as an OPERATOR REPRESENTATIVE, an individual must
meet the following criteria:
• Understand the intent of government, Operator and
contractor regulations, standards and safe working
practices related to the petroleum industry.
• Have work experience in various oil or gas operating areas.
• Have a sound understanding of wellsite activities.
• Complete competency requirements in the following (or
equivalent) training:
• Gas Testing and Monitoring.
• Equipment Isolations.
• Confined Space.
• Wellsite Permit to Work Issue Guidelines.
• Work Safety.
• Explosive/Radioactive Work.
• Pressure Systems Work.

2.6 WELLSITE PERMIT HOLDER AND WELLSITE WORK PARTY


QUALIFICATIONS
To qualify as a WELLSITE PERMIT HOLDER, an individual must
meet the following criteria:
• Attended both Operator and contractor safety induction
sessions relevant to that site within the prescribed
timeframe.
• Demonstrated an understanding of the WELLSITE PERMIT
HOLDER responsibilities under the Wellsite Permit to Work
System to the WELLSITE PERMIT AUTHORITY.
• Demonstrated an understanding of the content of the
applicable site specific safety regulations to the WELLSITE
PERMIT AUTHORITY.
• Demonstrated an understanding to the WELLSITE PERMIT
AUTHORITY of the use of Job Safety Analysis and when it
applies.
To qualify as a member of the WELLSITE WORK PARTY, an
individual must meet the following criteria:
• Attended both Operator and contractor safety induction
sessions relevant to that site within the prescribed
timeframe.

• Demonstrated an understanding of the WELLSITE WORK
PARTY responsibilities under the Wellsite Permit to Work
System to the WELLSITE PERMIT AUTHORITY.
• Demonstrated an understanding of the content of the
applicable site specific safety regulations to the WELLSITE
PERMIT AUTHORITY.
Note: The demonstration of understanding for the WELLSITE
PERMIT HOLDER and WELLSITE WORK PARTY may be achieved
through the successful completion of competency assessed
training that covers the above requirements.

2.7 CATEGORIES OF WORK


WELLSITE PERMITS TO WORK must be issued for all categories
of work defined as COLD WORK, HOT WORK, PRESSURE
SYSTEMS WORK, ELECTRICAL WORK, EXPLOSIVE/RADIOACTIVE
WORK, and CONFINED SPACE ENTRY. Non-manual activities such
as visiting a site do not require a WELLSITE PERMIT TO WORK,
however, the presence of all persons in operational areas must
be cleared by the WELLSITE PERMIT AUTHORITY.
Categories of work are described in Section 3. For each category
of work there are Work Precautions that must be taken.

2.8 WORK PRECAUTIONS


The WELLSITE PERMIT AUTHORITY is responsible for all
preparatory work and reinstatement work which is necessary
to ensure safe working conditions. Such work is known as Work
Precautions. These Work Precautions may continue while work
is in progress (eg. continuous gas monitoring).
All Work Precautions taken are recorded on the WELLSITE
PERMIT TO WORK. The WELLSITE PERMIT HOLDER and members
of the WELLSITE WORK PARTY should satisfy themselves that
all appropriate Work Precautions are understood and
implemented. In particular, they should check isolations and

lockouts and must install their own locks and tags where
required (refer to tagging procedures in Section 6).
2.8.1 Work Precautions Carried Out by Another Person
The WELLSITE PERMIT AUTHORITY must be satisfied that if
another person is to carry out Work Precautions that the person
is competent to perform the Work Precautions. This competency
may be demonstrated by training. The table below shows
examples of appropriate training:

The WELLSITE PERMIT AUTHORITY must specify on the WELLSITE
PERMIT TO WORK (Section C - ONGOING) who is responsible for
verifying Work Precautions (Work Precautions Verifier) before
work commences.

2.9 WORK CONDITIONS


The WELLSITE PERMIT HOLDER is responsible for ensuring that
all instructions given by the WELLSITE PERMIT AUTHORITY are
carried out and that all protective measures stipulated by the

WELLSITE PERMIT AUTHORITY (Section C - ONGOING) are being
used or are in place. Such instructions and protective measures
are known as Work Conditions.
A WELLSITE PERMIT TO WORK will be withdrawn immediately
if Work Conditions are not being strictly complied with.
The WELLSITE PERMIT HOLDER agrees to ensure that the
WELLSITE WORK PARTY understands and abides by the Work
Conditions by signing the WELLSITE PERMIT TO WORK in the
space provided.
The names of all members of the WELLSITE WORK PARTY shall
be listed on the WELLSITE PERMIT TO WORK.
All members of the WELLSITE WORK PARTY are required to sign
the WELLSITE PERMIT TO WORK.
If the WELLSITE PERMIT HOLDER or any member of the WELLSITE
WORK PARTY is uncertain about any aspect of a Work Condition,
clarification must be sought from the WELLSITE PERMIT
AUTHORITY before proceeding with the work.

3. WORK DESCRIPTIONS

3.1 COLD WORK PERMIT


A COLD WORK PERMIT is required for work carried out that is
non-standard, will not generate any source of ignition, such as
flame, spark or temperature sufficient to ignite any flammable
material and is not covered by any other permits.
Non-standard work is any work that has a health and safety risk
and is not regularly performed. A COLD WORK PERMIT is
required for any non-standard hazardous operation, task or
activity, maintenance and/or repair work that meets the
following criteria:
• One off activity that has the potential to injure personnel
or damage equipment.
• Any hazardous activity that personnel are not familiar with.
• Any hazardous activity where no procedure exists.
• Any deviation or modification to existing procedures and/
or JSA.
Non-standard work includes, but is not limited to:
• Isolation, override or disablement of rig safety systems, such
as fire and gas detection systems, communication systems,
life saving and rescue equipment, fire fighting equipment,
and emergency devices.
• Non-standard operations or maintenance of draw works,
rotary, etc.
• Any deviation from procedure.
• Any work at heights, not currently covered by a standard
work procedure.
• Non-standard maintenance in the mast during normal
operations.
• Scaffold erection on rig or camp site.
• Work in unguarded/unprotected area.

• Work with chemicals for which personnel are unfamiliar.
• Inspection of mast, standpipe, derrick, or substructure
fixtures during normal operations.
Note: The requirement for a COLD WORK PERMIT may be
superseded by the use of an Approved Standard Procedure or
Approved JSA (Job Safety Analysis). It is expected that the
requirement for the COLD WORK PERMIT will drive the
development and establishment of new Standard Procedures
and JSAs. As such draft versions may be used in conjunction
with WELLSITE PERMIT TO WORK, to give personnel an
opportunity to develop, trial, review and improve the procedures
prior to seeking approval.
3.1.1 Excavation
Prior to the commencement of work by the Wellsite Contractor
an agreement must be made between the Operator and Wellsite
Contractor as to the area in which and restrictions under which
the Wellsite Contractor is allowed to carry out excavation
activities. Inside this area and subject to the restrictions, the
WELLSITE PERMIT AUTHORITY may issue permits for excavation,
however, outside of this area the Operator’s permit system must
be used for such activities and may require the issue of an
Excavation Permit by the Operator.
Excavation is the digging/penetration into any ground surface
and the permit provides checks on hazards such as buried cables,
buried flowlines and suitability of the excavation, and includes
such work as:
• hand excavation,
• mechanical excavation,
• grading wellsites.

3.2 HOT WORK PERMIT


A HOT WORK PERMIT is required for work which could generate
fire, naked flame, heat, spark or other source of ignition, and is
located anywhere on the wellsite other than within an approved

maintenance or welding workshop or designated smoking area,
provided that workshop or area is not within 45 metres of the
well or other source of flammable vapour.
HOT WORK includes, but is not limited to:
• welding, soldering, hot riveting,
• burning, flame cutting, flame heating,
• grinding, shot blasting,
• concrete chipping,
• use of electrical hand tools, power driven cutters,
• use of internal combustion engines,
• lighting a fire of any kind (all zones) including operations
such as drill stem tests, air drilling and live workovers,
• use of any equipment which may provide a source of
ignition,
• use of non-intrinsically safe electrical devices,
• use of cameras with batteries,
• transfer of hydrocarbon fluids to or from road tankers (with
a flash point <20°C)
• use of x-ray generating equipment,
• potential to generate static electricity.

3.3 PRESSURE SYSTEMS WORK PERMIT


A PRESSURE SYSTEMS WORK PERMIT is required for any work
involving equipment which usually is under pressure, may be
under pressure, or any work involving pressurised operations.
PRESSURE SYSTEMS WORK includes, but is not limited to:
• work on the accumulator unit and any associated pressure
lines or pressure equipment,
• work on the blowout preventors and any associated
pressure lines or pressure equipment,

• work on wellhead equipment,
• pressure cleaning of tubulars,
• any pressure tests,
• leak off and formation integrity tests,
• fracture stimulation,
• chemical injection into wellheads for inhibition,
• work on pumps and any associated pressure lines or
pressure equipment,
• work on any hydraulic systems,
• work on any air pressure systems.

3.4 ELECTRICAL WORK PERMIT


An ELECTRICAL WORK PERMIT is required for work on any
electrical circuits or equipment. This work should be carried out
by a person holding formal electrical qualifications.
ELECTRICAL WORK includes, but is not limited to:
• work on generators,
• work on electrical panels,
• work on silicon controlled rectifier units,
• work on electric motors,
• work on live electrical circuits which are not in electrical
substations,
and includes repair, maintenance, testing and connection.

3.5 EXPLOSIVE/RADIOACTIVE PERMIT


An EXPLOSIVE/RADIOACTIVE PERMIT is required whenever
explosives or radioactive substances are to be used or handled.
EXPLOSIVE/RADIOACTIVE WORK includes, but is not limited to:
• perforating,
• well logging using radioactive substances,

• back off tools,
• flow stream meters which are radioactive.

3.6 CONFINED SPACE ENTRY WORK PERMIT


A CONFINED SPACE ENTRY WORK PERMIT is required whenever
entry into a confined space is required as defined below.
A confined space is a space of any volume which:
• is not intended as regular workplace,
• has restricted means of entry and exit,
• may have inadequate ventilation and/or atmosphere which
is either contaminated or oxygen deficient,
• and is at atmospheric pressure during occupancy.
CONFINED SPACE ENTRY is the entry of a person’s upper body/
head into a confined space. Examples of confined spaces include
but are not limited to:
• storage tanks, mud tanks and other tank like compartments
usually having only a manway for entry,
• open-topped spaces of more than 1.5 metres in depth such
as pits and well cellars, which are not subject to good
natural ventilation,
• pipes, tunnels, shafts, ducts and similar structures,
• fabricated metal enclosures such as inlet and exhaust
plenums.
CONFINED SPACE ENTRY PERMIT only authorises entry into a
confined space. Any work done inside a confined space is subject
to the normal safety requirements for such work, and the
appropriate WELLSITE PERMIT TO WORK.

3.7 WORK NOT REQUIRING A WELLSITE PERMIT TO WORK
WELLSITE PERMITS TO WORK are not required in the following
instances:
• Routine operations (work that is regularly performed and
has no peculiar health or safety risk).
• Minor maintenance work carried out by trained competent
personnel as agreed with the WELLSITE PERMIT AUTHORITY
provided that an appropriate Risk Hazard Analysis (eg. a
Job Safety Analysis or Stepback) has been performed.
• In the event of initiating the Wellsite Emergency Response
Plan and only as directed by the Toolpusher/Rig Manager
or Operator Representative.
• HOT WORK carried out in an approved maintenance
workshop.
• COLD WORK or HOT WORK carried out in camp areas
provided that an appropriate Risk Hazard Analysis has been
performed.
• Routine rig up or rig down of third party equipment if
approved procedures are followed and not specifically
covered by any of the permits.
• Surveillance activities performed at a wellsite by Petroleum
Engineering once the well has been handed over via an
approved well handover procedure.
Site specific exceptions for work not requiring a WELLSITE
PERMIT TO WORK must be approved as per Section 1.5.
Any work which may be unclear as to whether a WELLSITE
PERMIT TO WORK is required must be referred to the WELLSITE
PERMIT AUTHORITY.

4. WELLSITE PERMITS TO WORK

4.1 GENERAL
A WELLSITE PERMIT TO WORK is a document that sets out the
work to be done and the Work Precautions to be taken. It
specifies Work Conditions and provides a record of all hazards
that have been considered in advance. It assists in defining Work
Precautions. It does not, in itself, make the job safe, but is
dependent for its effectiveness on the persons involved
complying with all the requirements of the Procedures.
Special care should be taken to ensure those personnel and
contractors who carry out specific tasks at wellsites are briefed
in the Procedures. Contractor employees and representatives
may be unaware of the nature of any special risks inherent in
wellsite activities and operations.

4.2 WELLSITE PERMIT TO WORK DURATION


A WELLSITE PERMIT TO WORK is valid only for the remainder of
the WELLSITE WORK PARTY’s shift in which it was issued or an
earlier nominated expiry time. Should it be necessary to continue
the work beyond the nominated expiry time, work shall cease
and a new Permit shall be issued before recommencement of
work.
If work is suspended, or delayed due to shift change, the Permit
shall become invalid and a new Permit shall be issued before
work can recommence.
If for any reason the WELLSITE PERMIT HOLDER becomes
unavailable during an activity, the Permit becomes invalid, work
shall cease and a new Permit shall be issued before
recommencement of work.
When work is interrupted, the WELLSITE PERMIT HOLDER must
advise the WELLSITE PERMIT AUTHORITY of the delay when it
first occurs and then immediately prior to work recommencing.

The WELLSITE PERMIT AUTHORITY is to be notified as soon as
the work is complete or terminated.
All WELLSITE PERMITS TO WORK must be returned to the
WELLSITE PERMIT AUTHORITY prior to the end of the shift unless
an alternative time has been specifically agreed between the
WELLSITE PERMIT HOLDER and the WELLSITE PERMIT
AUTHORITY. The requirement for the WELLSITE PERMIT
AUTHORITY to monitor work outside the shift should be
reviewed on a case by case basis.
If work is completed before the end of the shift, then the
WELLSITE PERMIT TO WORK must be returned to the WELLSITE
PERMIT AUTHORITY at the time work is completed.
4.2.1 Emergency Siren
Sounding of the emergency siren automatically voids all
WELLSITE PERMITS TO WORK unless it is part of routine siren
testing. All persons must assemble at the muster location or as
specified in the applicable site emergency plans. The WELLSITE
PERMIT HOLDER is to take their WELLSITE PERMIT TO WORK
with them to the muster point.
WELLSITE PERMITS TO WORK must be re-issued by the WELLSITE
PERMIT AUTHORITY before work resumes.

4.3 WELLSITE PERMIT AUTHORISATION LEVELS


The nominated WELLSITE PERMIT AUTHORITY should be
authorised to issue the following WELLSITE PERMITS TO WORK:
• COLD WORK PERMIT.
• HOT WORK PERMIT.
• PRESSURE SYSTEM WORK PERMIT.
• ELECTRICAL WORK PERMIT.
• EXPLOSIVE/RADIOACTIVE PERMIT.
• CONFINED SPACE ENTRY PERMIT.

4.4 ISSUING WELLSITE PERMIT TO WORK PERMITS
4.4.1 Restrictions, Conflicting Work, and Suspensions
It is essential that separate work activities do not conflict with
each other. The WELLSITE PERMIT AUTHORITY, OPERATOR
REPRESENTATIVE, WELLSITE PERMIT HOLDER, and WELLSITE
WORK PARTY must be aware of potential interaction with other
work activity, and should check when a permit is prepared
whether it impinges on one already issued or any other work
not requiring a permit.
Close liaison may be required, especially when conflicting permits
involve different responsible persons. Consideration must be
given to the number and type of permits issued within any work
area, and whether there are sufficient resources to properly
manage the work.
Restriction on the number of active permits and amount of work
may be required to ensure resources can manage any problem
scenarios. Any emergency situation automatically voids all
permits, and they must be re-issued before work resumes.
4.4.2 Permit Issue
Step 1 - Notification
The WELLSITE PERMIT AUTHORITY and OPERATOR
REPRESENTATIVE will in many instances require advance
notification of work to be done. The notification time will
depend on the preparatory work required and the effect of the
work on other activities.
Advance notification ensures that:
a. The WELLSITE PERMIT AUTHORITY and OPERATOR
REPRESENTATIVE are actually available on site.
b. The work can be scheduled to meet operational
requirements.
c. There are minimal delays for the WELLSITE WORK PARTY.
d. The job can be assessed in advance.

Step 2 - Site Inspection and Safety Briefing
In most instances a site inspection and safety briefing is required.
This is to ensure that:
a. The WELLSITE PERMIT AUTHORITY, OPERATOR
REPRESENTATIVE, WELLSITE PERMIT HOLDER and WELLSITE
WORK PARTY are referring to the same plant or equipment.
b. Control Points in the job are identified.
c. Work Precautions that need to be taken become more
obvious.
d. The existing state of the plant and equipment is assessed.
e. Work Conditions (including any specific instructions) can
be properly explained.
f. The tools and equipment to be used by the WELLSITE WORK
PARTY can be inspected.
g. The effect of the work on other activities can be better
assessed.
h. Personnel and vehicle access and egress for the job is
assessed.
Whenever a site inspection is not practical, then the site
inspection and Work Precautions may need to be carried out as
per Section 2.8.1. It is essential that preparatory work and
reinstatement work be fully specified in the Work Conditions.
Step 3 - Preparing the Permit (and carrying out
preparatory work)
Each WELLSITE PERMIT TO WORK consists of 3 copies.
Copy 1 - WELLSITE PERMIT AUTHORITY copy. This copy remains
with the WELLSITE PERMIT AUTHORITY and gives the WELLSITE
PERMIT AUTHORITY the overall information required to
maintain control over all permitted work in any given area.
Copy 2 - OPERATOR REPRESENTATIVE copy. This copy is given to
the OPERATOR REPRESENTATIVE and gives the OPERATOR
REPRESENTATIVE the overall information required to monitor
work in any given area.

Copy 3 - WELLSITE PERMIT HOLDER copy. This copy remains in
the possession of the WELLSITE PERMIT HOLDER on the job site
and gives the WELLSITE PERMIT HOLDER all of the information
required to carry out the job safely. This copy is returned to the
WELLSITE PERMIT AUTHORITY or nominated person at the
termination of the permit.
Features of the WELLSITE PERMIT TO WORK
The WELLSITE PERMITS TO WORK are divided into 4 sections on
the front page and also has information on the back of Copy 3.
The Permit number is printed at the top of the front page and is
unique.
The category of work must be checked at the top of the front
page (tick the appropriate check box). All the Work Precautions
Checklist (Section B - PRESTART) for that category must be
completed before the Permit is valid.
SECTION A - SCOPE (Front Page)
Section A of the WELLSITE PERMIT TO WORK is completed by
both the WELLSITE PERMIT AUTHORITY and the WELLSITE
PERMIT HOLDER (it must be checked by the WELLSITE PERMIT
AUTHORITY).
Included in Section A is:
• Date of issue.
• Name of WELLSITE PERMIT HOLDER.
• Company name of the WELLSITE PERMIT HOLDER.
• Specific location of work.
• Specific work description.
• List of associated documents.
• WELLSITE WORK PARTY (names of all personnel covered
by the WELLSITE PERMIT TO WORK).
• Description of tools and equipment to be used.

SECTION B - PRESTART (Front Page)
Section B of the WELLSITE PERMIT TO WORK is completed by
the WELLSITE PERMIT AUTHORITY. It consists of questions which
are framed as to constitute a ‘checklist’ of precautions which
must be considered by the WELLSITE PERMIT AUTHORITY and it
also has a section in which the WELLSITE PERMIT AUTHORITY
can record details of isolations and pre-start gas test results.
This section is designed so that the WELLSITE PERMIT AUTHORITY
can stipulate whether a precaution is required (yes/no) and be
initialled once the precaution has been carried out.
SECTION C - ONGOING (Front Page)
Section C is defined by the WELLSITE PERMIT AUTHORITY and
must be complied with by the WELLSITE PERMIT HOLDER and
all members of the WELLSITE WORK PARTY.
It consists of a list of Work Conditions that must be reviewed
and if considered mandatory must be marked as being required.
Any additional protective measures agreed between the
WELLSITE PERMIT AUTHORITY and the WELLSITE PERMIT
HOLDER must be transferred to this section (other details and
conditions) from the back page.
Any specific conditions, precautions or requirements will be
stipulated as “Other Details or Conditions”. For example,
“Circulate hole clean and pull out of hole to shoe” may be a
condition required prior to conducting any HOT WORK around
the wellhead or mudtanks after penetrating a potential
hydrocarbon bearing zone.
If the WELLSITE PERMIT AUTHORITY nominates a person to be
responsible for the Work Precautions their name must be
recorded as indicated (Work Precautions Verifier).
SECTION D - ISSUE/CLOSE OUT (Front Page)
Section D of the WELLSITE PERMIT TO WORK is to be completed
by the WELLSITE PERMIT AUTHORITY and signed by the
appropriate parties where indicated. Section D consists of the
following areas:

Time of permit issue and expiry time and date if relevant to be
filled in by the WELLSITE PERMIT AUTHORITY.
• Acceptance/authorisation
Wellsite Permit Holder Acceptance
Record of the WELLSITE PERMIT HOLDER’S name and
signature accepting all terms and conditions under which
work can be carried out.
Wellsite Permit Authority Approval
Record of the WELLSITE PERMIT AUTHORITY’s name and
signature approving the work to be performed under the
conditions set out in the permit. The permit is not approved
until all work precautions are completed and Work
Conditions have been met.
Operator Representative Authorisation
Where an OPERATOR REPRESENTATIVE has been assigned,
this records the name and signature authorising the work
to be performed under the conditions set out in the permit.
• Work Complete
The WELLSITE WORK PERMIT is signed off when the work
is complete or the time allocated on the permit has expired
or earlier terminated (eg. upon emergency siren sounding),
site clean up, all locks removed, and/or all tags are removed.
All three copies need to be collected and signed by the
WELLSITE PERMIT AUTHORITY, OPERATOR REPRESENTATIVE
(if any) and the WELLSITE PERMIT HOLDER in the
appropriate location.
Back Page
The following is contained on the back of Copy 3:
• Emergency response siren details.
• Additional protective measures checklist which is agreed
upon between WELLSITE PERMIT AUTHORITY and the

WELLSITE PERMIT HOLDER. Those agreed must be
transferred to Section C of the WELLSITE PERMIT TO WORK.
• The WELLSITE WORK PARTY is to sign the back of the permit
form. This signature confirms the understanding of the
permit form, and acceptance of the Work Conditions.
• Details of Uncompleted Work Carried Over (filled in by
WELLSITE PERMIT HOLDER just before the permit is
returned to the WELLSITE PERMIT AUTHORITY)
• If a CONFINED SPACE ENTRY PERMIT is required then the
Confined Space Entry Person(s) and Confined Space Standby
Person(s) must enter their name and sign the back of the
permit form where indicated. The Confined Space Entry
Log must also be completed.
Step 4 - Monitoring Work
The WELLSITE PERMIT AUTHORITY monitors the work carried
out by the WELLSITE WORK PARTY. The WELLSITE PERMIT TO
WORK is withdrawn at any time that noncompliance of any Work
Conditions is observed.
It is also necessary to check that the Work Precautions that have
been taken are adequate.
If any person observes work being carried out that appears
unsafe then work must cease immediately. The WELLSITE PERMIT
HOLDER and the WELLSITE PERMIT AUTHORITY must review the
Work Conditions before work recommences. The WELLSITE
PERMIT HOLDER’S supervisor must be informed. If another
contractor is involved, the OPERATOR REPRESENTATIVE must also
be informed.
The WELLSITE PERMIT AUTHORITY must approve any change in
the WELLSITE WORK PARTY. A change in WELLSITE WORK PARTY
must be clearly identified on the front of the WELLSITE PERMIT
TO WORK and countersigned by the WELLSITE PERMIT HOLDER
on his copy. The new member of the WELLSITE WORK PARTY
must also sign the permit.

If there is any change in WELLSITE PERMIT HOLDER during a
job, the permit becomes invalid, work shall cease and a new
permit shall be issued before recommencement of work.
Any change in WELLSITE PERMIT AUTHORITY or the OPERATOR
REPRESENTATIVE during a job must be acknowledged by the
new WELLSITE PERMIT AUTHORITY or the OPERATOR
REPRESENTATIVE countersigning the permit, having checked all
Work Precautions , and the WELLSITE PERMIT HOLDER notified.
Step 5 - Clearing the Permit
The WELLSITE PERMIT HOLDER is responsible for leaving the
work area clean and tidy, and where appropriate, for overseeing
the removal of locks and personal danger tags used for the
equipment lockouts when the job is complete and it is safe to
do so. (See Section 6.)
If the job identified on the WELLSITE PERMIT TO WORK is
complete, the WELLSITE PERMIT HOLDER indicates YES the job
is complete in the Job Complete section, and returns their copy
of the WELLSITE PERMIT TO WORK to the WELLSITE PERMIT
AUTHORITY and OPERATOR REPRESENTATIVE (if any). The
WELLSITE PERMIT AUTHORITY, upon receipt of the WELLSITE
PERMIT HOLDER’s copy carries out any reinstatement work
required to return equipment to its operating state. The
WELLSITE PERMIT AUTHORITY, OPERATOR REPRESENTATIVE (if
any) and WELLSITE PERMIT HOLDER can then sign off all copies
of the WELLSITE PERMIT TO WORK as YES under the Job
Complete section.
If the job identified on the WELLSITE PERMIT TO WORK is not
complete, the WELLSITE PERMIT HOLDER indicates that NO the
job is not complete in the Job Complete section, then fills out
the section on the WELLSITE PERMIT TO WORK Details of
Uncompleted Work Carried Over, and returns their copy of the
WELLSITE PERMIT TO WORK to the WELLSITE PERMIT
AUTHORITY and OPERATOR REPRESENTATIVE (if any). The
WELLSITE PERMIT AUTHORITY, upon receipt of the WELLSITE

PERMIT HOLDER’s copy carries out any work required to ensure
that the equipment and area remains in a safe state. All copies
of the WELLSITE PERMIT TO WORK are then signed off as NO
under the Job Complete section by the WELLSITE PERMIT
AUTHORITY, OPERATOR REPRESENTATIVE (if any) and WELLSITE
PERMIT HOLDER and other personnel are notified of the
uncompleted work as required.

4.5 SPECIAL CASES


4.5.1 Mixing of Hot and Cold Work Permits
The impact of one work task on another must be thoroughly
investigated prior to the issue of a permit.
Work conditions for a COLD WORK PERMIT must not be assumed
when there is HOT WORK being carried out in the area.
Work under a COLD WORK PERMIT and HOT WORK are not to
be covered under the same WELLSITE PERMIT TO WORK.
Separate WELLSITE PERMITS TO WORK must be issued.
4.5.2 Use of Mechanical Earthmoving Machinery
The use of gas, diesel, petrol or electrically powered earthmoving
machinery is HOT WORK. When this equipment is used
specifically for excavation, a HOT WORK PERMIT is required.

4.6 RETENTION OF PERMITS


All completed WELLSITE PERMITS TO WORK should be retained
at the office of issue for a period of three months except those
involving Confined Space Entry (refer to Section 8.1).
If an Accident/Incident Report is associated with any work
covered by a WELLSITE PERMIT TO WORK then a copy of the
WELLSITE PERMIT TO WORK must be attached to the Report.

5. Work Precautions - SPECIFIC INSTRUCTIONS
AND PROTECTIVE MEASURES

5.1 GENERAL
All specific instructions given to the WELLSITE PERMIT HOLDER
and WELLSITE WORK PARTY are summarised and listed on the
WELLSITE PERMIT TO WORK. Such instructions will vary
according to the work being done, the wellsite, the category of
work, and the experience and training of the WELLSITE PERMIT
HOLDER and WELLSITE WORK PARTY.
These instructions are in addition to those covered by protective
measures.
Protective measures that need to be taken by the WELLSITE
WORK PARTY are also listed on the WELLSITE PERMIT TO WORK.
These measures are in addition to any lockouts, gas tests, and
CONFINED SPACE ENTRY procedures.

5.2 PROTECTIVE MEASURES FOR ALL WORK


Procedure
Written document providing details of the steps, practices or
series of actions to be done in order to accomplish the activity
in a safe manner. This may include documented and approved
JSAs, Job Sheets and Hazard Sheets.
Eye Protection
Must be worn.
Hearing Protection
Must be worn whenever the noise level in the work area is
greater than 85 dB(A). Some work areas require hearing
protection to be worn at all times.
Goggles and Face Masks
Must be used whenever dangerous chemicals or extreme dust
conditions are present. Grinding operations require goggles and
full face mask to be worn.

Gloves
Must be worn unless the wearing of the gloves impairs the ability
of the person to perform the task. Gloves must be appropriate
for the job.
Protective Clothing
Full body clothing including long sleeve shirts and trousers to
be worn at all wellsites. Additional protective clothing must be
worn whenever harmful products are being handled.
Respirators
Must be worn whenever airborne toxic materials and/or dusts
are present, unless breathing apparatus is being worn.
Housekeeping
A good standard of housekeeping must be maintained at all
times.
Hard Hats
Must be worn at all times unless the Work Conditions specify
otherwise.
Safety Footwear
Must be worn at all times unless the Work Conditions specify
otherwise.
Intrinsically Safe Lights
May be used when there is insufficient natural light.
Intrinsically Safe Hand Tools
Intrinsically safe or non-sparking hand tools must be used where
practicable.
Lifting Gear
Must be certified and in good condition. Loads must be attached
to lifting gear using the correct equipment (eg. slings, shackles
etc). Loads must not be lifted over areas where persons are
working and over unprotected high pressure pipework unless a
Risk Hazard Analysis has been performed.

Ladders (or approved mobile platforms)
Must be used to gain access to high places. Where possible non
conductive ladders should be used for electrical work. Ladders
must also be secured to prevent slipping.
Scaffolding
May be erected whenever work is carried out above ground
level. Scaffolding must be erected by competent personnel.
Barriers
Must be used to prevent people entering unsafe environments.
Safety Harness
Must be used when there is a danger of falling from high
exposed positions, and when working above ground level
without proper railings.
Safety Briefing
All persons working on a job with the WELLSITE PERMIT HOLDER
must be briefed on the Work Conditions before the job
commences.
No Smoking
Smoking is not allowed at any production or process facility or
within 50 metres of any wellhead or hydrocarbon transport
facilities.
Emergency Procedures
As they apply to each work site must be explained to the WORK
PARTY.
Access and Egress
Safe access to the job site must be provided. This may, for
example, entail provision of scaffolding. It may also require the
temporary removal of handrails or fences. Ease of egress in the
event of an emergency must also be considered.
Gas Tests
Prestart gas test readings must be recorded prior to setting up a
continuous gas monitor.

Continuous Gas Monitoring
Continuous gas monitors must be set up whenever initial Lower
Explosive Limit (LEL) readings in the work area could be affected
by possible gas leak sources and changes in wind direction.
Continuous gas monitors shall have latching, visual and audible
alarms.
Area Free from Combustibles
A good standard of housekeeping requires that wood, grass,
paper, plastic, rags, and soil soaked with oil must not be present
in the work area.
Mechanical Lockout/Tagout
A positive/physical isolation of the energy source must be
achieved.
Electrical Lockout/Tagout
A positive/physical isolation of the energy source must be
achieved.
Pipes Marked
Where work is performed the pipe, tanks or vessels must be
clearly marked or identified and indicate the nature of the
substance.
Warning Signs
Must be used to warn personnel of the potentially hazardous
activity under way.
JSAs
A Job Safety Analysis (JSA) (including risk assessment) should be
considered for all permits prior to permit issue.

5.3 SPECIFIC HOT WORK PROTECTIVE MEASURES


Protective Screens
Must be used around the work area to protect others from
sparks, welding splatter and welding flashes. Such areas may
also be roped off with notices as follows:

Fire Watcher
A person (nominated the Fire Watcher) must be in attendance
whenever the work being carried out requires the use of a naked
flame or causes materials to be excessively heated or burned.
The assigned Fire Watcher:
a. Must be familiar with the nature of the job being done.
b. Must be alert for any fire outbreak or hazard and will take
immediate action to combat any outbreak that occurs.
c. Does not allow HOT WORK to proceed outside the area
specified in the permit.
d. Must ensure all persons in the area are protected from
welding flashes.
e. Must ensure fire extinguishers and/or fire hose is ready for
immediate use, and be trained in the use of such
equipment.
Drains Covered
Open drains within 15 metres of the work area must be sealed,
preferably with an expandible mechanical plug. Alternatively,
they may be covered and sanded.
Any potential source of flammable vapour, such as a sample
point, vent or drain situated within 15 metres of the work area
must be rendered safe by isolating the source. If there is a relief
valve outlet in the vicinity, it may be necessary to take equipment
out of operation.
Fire Extinguisher
A spare fully charged fire extinguisher, additional to those
permanently installed, must be positioned near to the work area.
Fire Hose
If available, a fire hose may be laid out ready for use.
Full Spark Containment
When HOT WORK is performed in an area where there is the
possibility of gas leaks from pipe fittings (eg. valve glands and

flanges) full spark enclosure must be used, The enclosure must
be so constructed as to contain all sparks.
HOT WORK in a Confined Space
All the conditions concerning HOT WORK must be complied with
in addition to those set out in Section 8 of these Procedures.
Power Driven Machinery or Motorised Equipment
All power driven machinery (including internal combustion
engines) must be located more than 15 metres from sources of
flammable vapours. They should also be placed upwind of
flammable vapour sources.

5.4 SPECIFIC PRESSURE SYSTEM WORK PROTECTIVE


MEASURES
Personnel Certification
Only those who are approved for pressure system work will
undertake such work.
Double Block and Lock
The pressure side of the work location is to be blocked off by at
least two valves or blind flanges, each individually closed, locked
and tagged out.
Energy Dissipation
Dissipate, restrain or contain any stored energy that may give
rise to a hazard, including accumulators.
Verify
Physically verify that all stored energy has been dissipated.

5.5 SPECIFIC ELECTRICAL WORK PROTECTIVE MEASURES


Personnel Certification
Only those approved for electrical work will undertake such
work.
Energy Dissipation
Ensure the discharge of electric capacitors and static electricity.

Electrical Insulation
Use insulation material (eg. rubber mats) and PPE as appropriate.

5.6 SPECIFIC EXPLOSIVE/RADIOACTIVE WORK PROTECTIVE


MEASURES
Identification
Identify details of explosive or radioactive source.
Personnel Certification
Only those who are approved and certified for explosive/
radioactive work are to undertake such work.
Storage
Adequate means of storage is to be provided for explosives and
radioactive sources.
Radio Transmission Sources
Within the danger zone, all such services are to be isolated and
tagged out.
Welding Unit
Within the danger zone, all such devices are to be isolated and
tagged out.
Overhead Power
Within the danger zone, all such lines are to be isolated and
tagged out.

5.7 SPECIFIC CONFINED SPACE ENTRY PROTECTIVE MEASURES

6. MECHANICAL, ELECTRICAL OR FLUID POWER
ISOLATIONS

6.1 GENERAL
Lockouts using a padlock (and chain if necessary) must be carried
out to ensure plant and equipment are isolated from all sources
of mechanical, electric or fluid power so that the plant and
equipment cannot operate. Mechanical power covers all
machinery that has a direct or indirect source of power (fluid,
electrical, pneumatic) as well as having moving parts and stored
energy systems that must be immobilised by physical means
where possible. Electrical power covers low voltage (less than
1,000 volts) electrical isolations. Fluid power includes the supply
of compressed air (gas, liquids and steam).
The WELLSITE PERMIT AUTHORITY must decide what lockouts
are required to ensure safety of personnel while work is being
carried out.
These lockouts are recorded in the appropriate section of the
Work Precautions Record.

6.2 LOCK AND PERSONAL DANGER TAG


The WELLSITE PERMIT AUTHORITY must ensure that the
WELLSITE PERMIT HOLDER places a padlock and personal danger
tag on each lockout identified in the Work Precautions Checklist.
If more than one WELLSITE PERMIT HOLDER is to work on the
locked out equipment, then separate locks and tags must be
used. Where appropriate, members of the WELLSITE WORK
PARTY may also attach their own padlock and danger tag on
each lockout. Scissor locks are available for this purpose.
For mechanical power lockouts the machinery must be isolated
from its source of power (electrical, fluid). Where possible
machinery should be immobilised by a physical means eg. pin in
a fly wheel. Stored or potential energy systems such as springs,

overhead hazards and pressure reservoirs also need to be
considered.
For electrical power lockouts, the isolation switch at the motor
control panel must be padlocked in the OFF position. In some
cases a padlock cannot be used and an alternate safe means
must be found (eg. removal of starter keys, removal of fuses,
disconnection of conductor at terminal, or removal of battery
leads).
For fluid power lockouts, the fluid supply line must be blinded,
or an isolating valve locked shut using chain and padlock.
6.2.1 Use of Padlocks
Padlocks are used to isolate electrical lockout bars and switches.
Padlocks and chains are used to lockout valves and other
equipment.
6.2.2 Use of Danger Tags and Out of Service Tags
Refer to appropriate Danger Tag and Out of Service Tag
Regulations for further information on the use of these tags.
Upon completion of the job, the WELLSITE PERMIT HOLDER must
remove their padlock. When the person who placed a padlock
or danger tag is for some reason prevented from removing their
lock and tag, the person’s supervisor may remove it. Before doing
so, the supervisor must make every reasonable effort to ascertain
the whereabouts of the person, ensure that no dangerous
condition exists, and obtain the approval of the WELLSITE
PERMIT AUTHORITY.

6.3 PROVING MECHANICAL ISOLATION


To ensure the correct motor has been isolated, the WELLSITE
PERMIT HOLDER must press the start button at the stop/start
station. Providing the motor does not start, a danger tag must
be affixed to the stop/start station. Where possible machinery
should also be immobilised by a physical means, eg. pin in a
flywheel. To ensure this physical means cannot be removed, the

WELLSITE PERMIT HOLDER must place a padlock, and a danger
tag on the affected parts of the machinery.

6.4 PROVING ELECTRICAL ISOLATION


In many instances electrically driven equipment has a local stop/
start station and a remote motor control panel (usually in the
motor control centre). Electrical isolation and locking of the
isolation switch is carried out at the motor control panel.
To ensure that the correct motor has been isolated, the start
button at the stop/start station is pressed. Provided the motor
does not start, a danger tag is affixed to the stop/start station.
The WELLSITE PERMIT HOLDER must place a padlock, and a
danger tag on the main isolator prior to commencing work.

6.5 PROVING FLUID POWER ISOLATION


Where valves are used as a means of fluid power isolation, they
must be locked shut using a padlock and chain and have a danger
tag affixed.
To ensure that the valve does not leak, the supply line
downstream of the valve must be depressured by opening vent,
bleed or drain valves or physically disconnected. As these valves
are usually small bore, measures must be taken to ensure that
they are not blocked.

7 GAS TESTING

7.1 GAS TESTS


Gas tests must be carried out to ensure that:
1. HOT WORK areas are free of flammable gas mixtures, and
below the maximum allowable 5% Lower Explosive Limit
(LEL) limit (refer to Section 8.5.1).
2. All work areas are preferably free of toxic gases, or levels
are within acceptable Threshold Limit Value (TLV) limits
(refer to Section 8.5.2).
3. Conditions of CONFINED SPACE ENTRY meet regulatory
requirements (refer to Sections 8.5.1 and 8.5.3).
The WELLSITE PERMIT AUTHORITY must ensure that the
appropriate gas tests are carried out. Only personnel competent
in the use of gas testing and monitoring equipment are to carry
out these tests.
Details of gas testing must be entered on the WELLSITE PERMIT
TO WORK. Record all readings obtained and the initials of the
person carrying out the gas testing.
The WELLSITE PERMIT AUTHORITY must ensure that when a
continuous gas monitor is set up, that the WELLSITE PERMIT
HOLDER and WELLSITE WORK PARTY understands that all work
must cease immediately the alarm sounds.
7.1.1 Testing for Flammable Gases in a Work Area
The concentration of flammable gases must be less than 5% of
the Lower Explosive Limit (LEL) before any work can proceed.
The presence of any gas should be double checked. Work is best
done with a zero LEL target.
• Where practical the LEL should be less than 5% to begin
work or to enter a confined space.
• If LEL is 5-10%, work may continue provided continuous
type gas monitoring equipment is in place.

• If LEL exceeds 10%, work must immediately cease and
persons must be evacuated from a confined space.
7.1.2 Testing for Toxic Gases in a Work Area
The Worksafe Australia exposure standards (extracted from
NOHSC:1003 (1995)) are as follows:
Hydrogen sulphide : 10 ppm maximum
Carbon dioxide : 5,000 ppm maximum
Carbon monoxide : 30 ppm maximum
These levels are for a maximum daily exposure of 8 hours for a
5 day working week without respiratory equipment. Data for
other potential toxic gases must be sourced and tests completed
to ensure levels are satisfactory before personnel are exposed.
There may be additional toxic gases or substances present.
Additional gas testing for toxic substances as per AS2865 should
be considered after a review of the vessel service. Consult with
Safety Advisers to review the proposed Work Precautions and
Work Conditions.
7.1.3 Testing for Oxygen in a Work Area
Oxygen content must be between 19.5% and 23.5%.

7.2 SERVICING AND CALIBRATION OF GAS TESTING


INSTRUMENTS
All gas testing instruments must be maintained in good working
order and regularly calibrated as per the manufacturer’s
recommendations. A record of inspections and calibrations must
be kept for each instrument.
Each instrument must carry a label identifying the instrument,
show period of validity, date of last calibration, and initials of
the issuing officer.

8. Work Precautions - CONFINED SPACE ENTRY

8.1 GENERAL
Before a person may enter a confined space, certain safety
precautions must be taken. The extent of these precautions will
depend upon a Confined Space Risk Assessment which must be
undertaken by the WELLSITE PERMIT AUTHORITY and WELLSITE
WORK PARTY prior to a CONFINED SPACE ENTRY PERMIT being
issued.
The completed Confined Space Risk Assessment shall be attached
to the CONFINED SPACE ENTRY PERMIT and must be retained
by the office of issue for a period of 5 years from the time of
their validity on completion of the work.
Only people who have been trained in Confined Space Entry
may enter confined spaces. Training records must be kept for
the term of the employee’s employment.
A current Confined Space/Standby Person Certificate must be
sighted by the WELLSITE PERMIT AUTHORITY before
authorisation for that person to enter a confined space is
approved.

8.2 RISK ASSESSMENT


Prior to the issue of the CONFINED SPACE ENTRY PERMIT a risk
assessment must be carried out by the WELLSITE PERMIT
AUTHORITY and the WELLSITE WORK PARTY which meets the
requirements of Australian Standard AS2865-1995.
The assessment shall be in writing and take into account all, but
not limited to the following:
• The name of the confined space.
• The work required to be done, including whether it is
necessary to enter the confined space.
• The range of methods by which the work can be done.

• The hazards involved and the associated risks.
• The actual method selected and plant proposed.
• Emergency and rescue procedures.
The WELLSITE PERMIT AUTHORITY shall ensure that the
assessment is revised whenever there is evidence to indicate that
it is no longer valid.
Each Operator and/or Wellsite Contractor shall have a formal
Confined Space Risk Assessment System. An example of a
Confined Space Risk Assessment form is attached in Appendix
E.

8.3 HAZARD IDENTIFICATION


Without limitation, the following additional hazards specific to
confined spaces shall at least be considered:
• Possible oxygen deficiency due to oxidation, combustion,
dilution, absorption and physical activity.
• Oxygen excess due to leaking hoses.
• Airborne and/or surface contaminants.
• Moving equipment.
• Uncontrolled introduction of fluids, ie. steam, nitrogen, etc.
• Suffocation/engulfment by solids.
• Electrocution.
• Noise.
• Temperature.
• Radiation.
• Explosion or fire.
• Slips, trips, and falls.
• Manual handling.

8.4 RISK FACTORS
Factors that shall be considered during the Confined Space Risk
Assessment are:
• Geographic remoteness from support services.
• Arrangements for rescue, first aid, and resuscitation.
• Number of persons occupying the space.
• Number of Standby Person(s) required.
• Proposed work procedures.
• Soundness and security of the confined space structure.
• Identity and nature of the substance previously occupying
the confined space.
• Confined space preparation procedures.
• Atmospheric testing procedures and parameters.
• Status of fitness and training of persons proposing to enter
the confined space.
• Work instruction briefing.
• Personal protective equipment.
• Signage.
• Work in adjacent areas.
• Cleaning of the confined space prior to and during entry.
• Mobile equipment location and use.
• Necessity for HOT WORK.

8.5 CONTROL MEASURES


For all hazards identified during the Confined Space Risk
Assessment, the assessment team shall determine appropriate
control measures to eliminate or minimise the risk associated
with such hazards using the following hierarchy:
• Elimination.

• Substitution.
• Isolation.
• Engineering controls.
• Administrative controls.
• Use of personal protective equipment.

8.6 STANDBY PERSON PROVISION


Standby Person(s) shall be provided wherever the risk assessment
indicates that:
• There may not be a safe oxygen level.
• Atmospheric contaminants are present or may be present
in concentrations above the exposure standards.
• There may be a risk of fire or explosion.
• There may be a risk of entrapment or engulfment.
• The work to be performed may generate risk to health and
safety.
• Equipment or conditions outside the confined space require
control or monitoring to ensure the health and safety of
persons in the confined space, (for example, ventilation,
respirator supply, vehicles and weather).
• There may be other risks to the health or safety of persons
entering the confined space.

8.7 STANDBY PERSON QUALIFICATIONS


To undertake the duties of a Confined Space Standby Person,
the following qualifications must be met:
• Hold a current Confined Space Standby Person Card which
certifies competency in:
1. Communication techniques via oral (English),
portable radio, and signal lines.
2. First aid including cardio pulmonary resuscitation.

3. Emergency and rescue procedures.
4. Self contained breathing apparatus.
• Must be clean shaven.
• Must be competent in the use of equipment used in
confined space entries, ie. air supply compressors,
ventilation fans, etc.
• Must understand the WELLSITE PERMIT TO WORK system
as it applies to CONFINED SPACE ENTRY.

8.8 STANDBY PERSON DUTIES


The duties of the Standby Person are:
• To remain on the outside of and in close proximity to the
confined space.
• To be positioned so as to be capable of being in continuous
communication with and to observe those inside the
confined space if practical.
• To control the entry of authorised persons only into the
confined space.
• To log persons into and out of the confined space on the
Confined Space Log Card or Log Book.
• To monitor the area about the confined space to ensure
that those inside the confined space are not placed at risk
due to other work activity or circumstances.
• Warn persons inside the confined space in case of imminent
or immediate danger.
• To be familiar with and ensure that the Confined Space
Risk Assessment conditions are complied with.
• Initiate the emergency procedures in the case of an
emergency.
• Monitor the functional operation of any safety apparatus
that maybe in use for the CONFINED SPACE ENTRY, ie.
ventilation fans, air supply compressors, etc.

9. Work Precautions - MOTORISED EQUIPMENT/
VEHICLE ENTRY

9.1 MOTORISED EQUIPMENT ENTRY


Gas, diesel or petrol powered motorised equipment such as
cranes, welding sets, cement mixers, compressors, soil excavators,
generating sets, and similar equipment may enter a wellsite as
a condition of a HOT WORK PERMIT, provided the work area
has been tested gas free, and the wellsite has been checked for
signs of abnormal conditions immediately prior to entry and
exit.
The equipment descriptions must be recorded in the Work
Conditions section of the WELLSITE PERMIT TO WORK.

9.2 VEHICLE ENTRY


9.2.1 Vehicle Entry to a Wellsite
Any vehicle used for the transport of personnel and materials
may be cleared by the WELLSITE PERMIT AUTHORITY to enter a
wellsite provided the vehicle remains at a distance of at least 30
metres away from the wellhead or other source of flammable
vapour.
Vehicle entry within 30 metres of a wellhead may be authorised
using a HOT WORK PERMIT. If the WELLSITE PERMIT AUTHORITY
has to enter within 30 metres with their vehicle, they do not
have to complete a WELLSITE PERMIT TO WORK, however they
must take the same precautions as if completing a HOT WORK
PERMIT.
The vehicle registration numbers or Operator unit numbers must
be recorded on the WELLSITE PERMIT TO WORK.

10. GLOSSARY
Confined Space
As defined by AS 2865 - Working in a Confined Space, a confined
space is a space of any volume which:
• Is not intended as a regular workplace.
• Has restricted means of entry and exit.
• May have inadequate ventilation and/or atmosphere which
is either contaminated or oxygen deficient.
• Is at atmospheric pressure during occupancy.
Confined Space Entry
The entry of a person’s upper body/head into a confined space.
Confined Space Standby Person
A competent person who observes or communicates with
persons inside a confined space and who raises the alarm in the
event of any danger.
Control Points
These are critical stages in a task or procedure. Work carried
out before this stage must be checked to ensure that it has been
carried out correctly and completely before the next stage is
started.
Excavation
The digging/penetration into the ground surface.
Fluid Power
Fluid power is any medium (except electricity) providing motive
power to equipment (including turbines, control valves) and
includes fuel gas, starting gas, hydraulic fluid, steam and
compressed air.
Gas Testing
The atmospheric testing of vessels, equipment and spaces in
general for flammability, toxicity and oxygen content.
Hot Work
Work which can generate fire, naked flame, heat or spark.

Intrinsically Safe
A general term in which any spark or thermal effect produced
normally or through fault conditions is incapable of causing
ignition.
Job Safety Analysis (JSA)
A planning and communication tool designed to involve the
WELLSITE WORK PARTY to ensure tasks can be completed safely
and efficiently by recognising hazards, identifying controls to
put in place to mitigate the risks associated with those hazards,
and the resources required.
Licence Area
The area comprised in the relevant petroleum exploration or
production licence.
Lockouts
Mechanical and low voltage electrical (less than 1,000 volts)
isolations of plant and equipment via the use of a padlock (and
chain if necessary). Such isolations protect from the sources of
electricity and fluid power. See also High Voltage Isolations.
Operator
The Oil and/or Gas Exploration/Production Company who holds
the petroleum exploration or production licence for the area
granted under the relevant Act and has implemented the
Wellsite Permit to Work System Procedures (as described in this
document).
Protective Measures
Safety requirements which must be met by the WELLSITE PERMIT
HOLDER. These safety requirements or measures are in addition
to any lockouts, process isolations, gas tests and high voltage
isolations.
Risk Hazard Analysis
An analysis done to ensure that the hazards associated with a
task are identified and controls are put in place to mitigate the
risks associated with those hazards. A Job Safety Analysis is a
form of Risk Hazard Analysis, as is Stepback.

on a well location where a Wellsite Contractor and/or their
equipment is situated or stored.

The company and/or personnel contracted to the Operator
(including subcontractors of the Wellsite Contractor) to provide
a service on or relating to oil and gas wells.
WELLSITE PERMIT AUTHORITY
A person who defines, carries out and/or verifies Work
Precautions, issues WELLSITE PERMITS TO WORK, and monitors
work carried out.
WELLSITE PERMIT HOLDER
A person who receives and accepts a WELLSITE PERMIT TO
WORK, carries out work described by it, and is responsible for
ensuring Work Conditions are followed.
WELLSITE PERMIT TO WORK
A work control document which assists in work being carried
out safely.
WELLSITE PERMIT TO WORK STEERING COMMITTEE
Representatives from various Australian onshore Operators and
Wellsite Contractors who oversee the maintenance and
development of the Wellsite Permit to Work System.
WELLSITE WORK PARTY
All members of a Wellsite Work Party whose work is covered by
a permit and who must abide by the Work Conditions of that

EMERGENCY SIREN
Sounding of an EMERGENCY SIREN automatically voids ALL work permits unless it is part of routine siren testing. All persons must assemble as
a safe location, the nominated point. The WELLSITE PERMIT HOLDER is to take their WELLSITE PERMIT TO WORK with them to the muster
point.
WELLSITE PERMIT TO WORK must be re-issued by the WELLSITE PERMIT AUTHORITY before work resumes.

Category
Wellsite Permit to Work Changes are those which are categorised as being:

Mandatory: Requested to satisfy safety, statutory, specification, or quality requirements.

Desirable: Is non mandatory and is viewed as an extension or addition to the current


Wellsite Permit to Work protective measures to allow work to be carried out
safely.

Procedure
Procedure for requesting and implementing changes:

1. In is incumbent upon all Permit personnel, particularly the Wellsite Permit Authorities,
OH&S Representatives and Supervisors/Rig Managers to discover, identify and recognise
variations and changes needed to allow work to be carried out safely.

2. After ascertaining that a Wellsite Permit to Work Change Request (WPWCR) is to be raised
to process a Wellsite Permit to Work Procedure change, the initiator of the proposal will
prepare a WPWCR form. Reasons for the request and a general statement covering the
changes considered necessary to resolve the deficiency must be given.

3. After being reviewed by the initiator’s Supervisor/ Rig Manager, the WPWCR is then passed
to the appropriate Operator’s/Contractors Management respectively for review.

4. The rejected and approved WPWCRs should then be passed to the Wellsite Permit to
Work Steering Committee Chairperson. After reviewing the WPWCR, the committee
chairperson will enter the change request on the next committee meeting agenda for
discussion and validation by the Wellsite Permit to Work Steering Committee.

5. After reviewing the WPWCR, the Wellsite Permit to Work Steering Committee, if they
consider the proposal justified, may have a detailed evaluation carried out by all affected
or specialist (eg safety officers) groups as appropriate.

6. The Wellsite Permit to Work Steering Committee Chairperson will co-ordinate all changes
of the affected procedures and associated documentation and distribution to the
appropriate organisations.

7. Wellsite Permit Authorities, through their company management and Supervisors/Rig


Managers, will be given notification of intended changes and amendments.
