Nurse

Nursing is a profession within the health care sector focused on the care
of individuals, families, and communities so they may attain, maintain, or
recover optimal health and quality of life.

Nurses may be differentiated from other health care providers by their


approach to patient care, training, and scope of practice.

Nurses practice in a wide diversity of practice areas with a different scope


and practice and level of prescriber authority in each. Many nurses provide
care within the ordering scope of physicians, and this traditional role has
come to shape the historic public image of nurses as care providers.
However , nurses are premitted by most juridictions to practice independently
in avariety of settings depending on training level. In the post war period,
nurse education has undergone a process of diversification towards and
advanced and specialized credential, and many of the traditional regulations
and provider role are changing .

The american Nurses Association (ANA) state nursing is the protection,


promotion, and optimization of health and abilities, prevention of ilness and
injury, alleviation of suffering throught the diagnosis and treatment of human
response, and advocacy in the care of individuals , families, communities, and
populations.

“ACCEPTING A NEW LONG STAY PATIENT IN THE WARD”

A. Confersation
Kian is taken by the Emergency Room (ER) nurse to the VIP ward. The nurse
who is in charge in the VIP ward is welcoming them.
ER Nurse : Assalamu’alaikum, I have a new patient here, his name is Kian,
he is with his mother Mrs. Helen he suffers from tyhpoid.
VIP Nurse : Wa’alaikumsalam, alright, hallo Kian and Mrs. Helen, I’m Nurse
Kalila, welcome in VIP ward, let me take you to room 5. All of
the facilities have been ready here. Hopefully you’ll feel
comfortable.
ER Nurse : This is the medical record and please take care of him well.
VIP Nurse : Sure, ... let me see. His name is Kian ?
ER Nurse : Yes that’s right. Well, i have to leave to my room Nurse. Mrs.

Maya is our mutual patient with diabetes mellitus and chronic kidney disease.
She is on continous cycling peritoneal dialysis (CCPD) using an automated
cycler through the night resulting in a large dextrose load during this time, and

may require insulin change to reflect that change in schedule. In a effort to


more efficientlymanage her health care, I would appreaciate the two of us
developing a strategy to jointly care for her glycemic control. The K/DOQI
guidelines recommend HbA1c be monitored quarterly, although it has not yet
established a target value. Due to recent studies associating intense control
with higher mortality without obvious decrease in morbidity, the goal should be
no lower than 6. Values of 7 to 8 may be acceptable. In stable patients
achieving goal HbA1c may be monitored semiannually. To avoid duplication
of services, we would like to coordinate this monitoring with you. Following
your periodic evaluations of this paient, I would very much appreciate the
following:
 A (phone call/fax/mailing with number preffered) with your findings and the
immediate plans that you feel need to be focused on.
 Notification when medication are added or changed
 Copies of any laboratory data obtained
As we co-manage the care of this patient in the future, I would like to provide
