AFFIDAVIT OF REQUEST FOR THE

ISSUANCE OF NEW TITLE

I, ELSA BAYSA CORPUZ, of legal age, single, Filipino citizen and a

resident of Barangay Rang-Ayan, Paniqui, Tarlac, after having been duly
sworn to in accordance with law, do hereby depose and state - THAT:

1. RODRIGO BAYSA, SR. is the registered owner of parcel of land

located in Barangay Nipaco, Paniqui, Tarlac which is covered by
Transfer Certificate of Title (TCT) No. 377618 of the Registry of
Deeds of the Province of Tarlac with an area of TWO THOUSAND
FOUR HUNDRED THIRTY SIX (2,436) SQUARE METERS, more or
less;

2. By virtue of a Deed of Sale dated 22 March 2004, RODRIGO BAYSA,

SR. sold in favor of Sps. Edgar Peralta and Myra Ronquillo an
undivided 300 square meters portion of the above-described parcel of
land as annotated in TCT No. 377618 under Entry No. E-44-16513;

3. Resultantly, TCT No. 377618 was partially cancelled and, in lieu

thereof, TCT No. 384284 was issued covering the earlier sold 300
square meters portion of the above-described 2,436 square meters
wide parcel of land;

4. Subsequently, the remaining TWO THOUSAND ONE HUNDRED

THIRTY SIX (2,136) portion of the subject parcel of land was sold by
RODRIGO BAYSA, SR. in my favor pursuant to a Sale of
Unsegregated Portion which was later annotated in TCT No. 377618
under Entry No. 2018012518 dated 08 October 2018 upon
presentment of the Certificate Authorizing Registration issued by the
Bureau of Internal Revenue;

5. However, the issuance of the corresponding Transfer Certificate Title

for the remaining 2,136 square meters sold to me was put on hold
pending presentation of the approved plan and technical description
thereof;

6. Thus, in compliance with the aforesaid condition, hereto-attached is a

copy of the approved plan and technical description of the 2,136
square meters sold to me by RODRIGO BAYSA, SR.;

7. In view of the foregoing, the Register of Deeds of the Province of

Tarlac is most respectfully requested to totally cancel TCT No.
377618 and, in lieu thereof, issue a separate new title covering the
2,136 square meters I purchased from Mr. RODRIGO BAYSA, SR.;
 8. I am executing this Affidavit to attest to the truth of the foregoing
declarations and to support the issuance of a corresponding TCT for

the 2,136 square meters I bought from Mr. RODRIGO BAYSA, SR.
and, incidentally, the cancellation of TCT No. 377618.
