Resumo: O objetivo deste trabalho foi avaliar a dose de insulina utilizada no tratamento do diabete melito gestacional
(DMG). Foram retrospectivamente incluídas 51 pacientes com diagnóstico de DMG, com gestação única, que
necessitaram de insulinoterapia. O período do estudo foi de janeiro de 2004 a dezembro de 2005. Foi utilizado
inicialmente 0,9 ui/kg/dia, dividido em quatro tomadas ao dia, sendo três doses iguais de insulina regular pré-
prandial e uma de NPH ao deitar; a dose foi ajustada conforme os resultados das glicemias capilares. Em 66,7%
das gestantes a dose total de insulina foi reduzida, utilizando uma média de 0,73 ui/kg/dia (DP = 0,15 ui) para o
controle glicêmico. A dose antes do café (AC) foi significantemente maior (p < 0,01), a dose ao deitar (AD)
foi significativamente menor que as outras (p < 0,01) e não foi encontrada diferença nas doses antes do almoço
(AA) e antes do jantar (AJ) (p = 0,83). A dose AC foi de 33%, de AA 25%, de AJ 25% e de AD 17% da dose
total. O peso médio dos recém-nascidos foi de 3.134 g (DP = 506 g), com 11,7% de GIG e 15,7% de hipoglicemia
neonatal, e não houve óbito perinatal. Concluiu-se, assim, que a dose mais adequada para essa população foi
de 0,7 ui/kg/dia.

Abstract: Objective: To evaluate the insulin dose for the management of gestational diabetes mellitus (GDM).
Materials and Methods: Fifty-one single-gestation patients diagnosed with GDM that needed insulin therapy
were included retrospectively in the study that occurred from January 2004 to December 2005. At first,
0,9ui/kg/day was used divided into four doses a day, with three equal doses of regular pre-prandial insulin and
one bedtime dose of NPH. The dose was adjusted according to the c����������������������������������������������
apillary glycemia measurement. Results: Total
insulin dose was reduced in 66.7% of patients, using an average of 0.73ui/kg/day (DP=0,15ui) for glucose level
control. The total dose before breakfast (BB) was significantly higher (p<0.01) and the dose at bedtime (BT)
was significantly smaller than other doses (p<0.01). No difference was found in the doses before lunch (BL) or
before dinner or supper (BD) (p=0.83). Doses were BB 33%, BL 25%, BD 25% and BT 17% of the total dose.
Average birth weight was 3,134g (DP=506g), with 11.7% LGA and 15.7% neonatal hypoglycemia. No perinatal
deaths were reported. Conclusion: The most adequate dose for this group was 0.7ui/kg/day.
