Defendant.

The undersigned has reason to believe that the Defendant committed the following offense(s):

COUNT 1: POSSESSION OF FORGED WRITING OR DEVICE FOR WRITING, a third


degree felony, in that the defendant, with intent to defraud, knowingly possessed any writing that
was a forgery, or with intent to defraud knowingly possessed any device for making any writing
that was a forgery, in violation of Section 76-6-502, Utah Code Annotated, 1953, as amended.

COUNT 2: POSSESSION OR USE OF A CONTROLLED SUBSTANCE (MARIJUANA),


a class B misdemeanor, in that the Defendant did knowingly and intentionally possess or use a
controlled substance analog or a controlled substance in violation of Sections 58-37-8(2)(a)(i)
and 58-37-8(2)(d), Utah Code Annotated, 1953 as amended.

COUNT 3: CONTRIBUTING TO THE DELINQUENCY OF A MINOR, a class B


misdemeanor, in that the defendant committed an act or engaged in conduct which he knew or
should have known would have the effect of causing or encouraging a minor to commit an act
which would be a misdemeanor or infraction criminal violation of any federal or state statute or
any county or municipal ordinance if committed by an adult, in violation of Section 76-10-2301,
Utah Code Annotated, 1953, as amended.
COUNT 4: CONTRIBUTING TO THE DELINQUENCY OF A MINOR, a class B
misdemeanor, in that the defendant committed an act or engaged in conduct which he knew or
should have known would have the effect of causing or encouraging a minor to commit an act
which would be a misdemeanor or infraction criminal violation of any federal or state statute or
any county or municipal ordinance if committed by an adult, in violation of Section 76-10-2301,
Utah Code Annotated, 1953, as amended.

COUNT 5: POSSESSION OF DRUG PARAPHERNALIA, a class B misdemeanor, in that


the defendant used, or possessed with intent to use, drug paraphernalia to plant, propagate,
cultivate, grow, harvest, manufacture, compound, convert, produce, process, prepare, test,
analyze, pack, repack, store, contain, conceal, inject, ingest, inhale or otherwise introduce a
controlled substance into the human body, in violation of Section 58-37a-5(1), Utah Code
Annotated, 1953, as amended.
