Beg to state that my daughter Mohana Das Gupta, Student of

Class IV, Roll # B-3 (Shapla) has been suffering from serious
Allergy (URTICARIA) from Oct 24, 2006. She was admitted in to
hospital on Oct 25 and discharged on Oct 30, 2006 (Photocopy
enclosed). She is still under treatment.

You are informed earlier that she has suffered from serious Lung
Disease and got treatment from Apollo, Chennai, India. She is
still taking medicine.

So, she can not attend school for next few days.

This is for your kind information and necessary action.
