MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

The PROVINCIAL LOCAL GOVERNMENT UNIT of Surigao del Sur, a local government unit created and
existing under the laws of the Republic of the Philippines with postal address at Provincial Capitol
Building, Telaje, Tandag City, Surigao del Sur, represented by Governor Vicente T. Pimentel, hereinafter
referred to as PLGU;

- and -

The PHILIPPINE CIVIC CRIME MONITORING GROUP, a non-stock non-profit organization organized and
existing under the laws of the Republic of the Philippines with postal address at Pagbahanan 3,
Poblacion, Marihatag, Surigao del Sur, represented by its Provincial Director David S. Agopitac, Jr.,
hereinafter referred to as PCCMG;

- WITNESSETH -

WHEREAS, Republic Act No. 7160, otherwise known as the Local Government Code of 1991 has
mandated the local government units to be the providers of the basic social, economic and agricultural
services;

WHEREAS, the PLGU of Surigao del Sur has prioritized a program of assistance to non-stock non-profit
organization that cater to crime prevention and monitoring in the community particularly within the
province;

WHEREAS, PCCMG is organized to assist the PNP subject to existing laws in coordination with the DILG,
AFP, LTO and other government agencies;

NOW, THEREFORE, FOR AND IN CONSIDERATION of the foregoing premises and consideration, the
parties hereto agree as follows:

ARTICLE I – GENERAL PROVISION

This Memorandum of Agreement shall cover the activity entitled: HOSPITAL SECURITY ASSISTANCE
located at PAGBAHANAN 2, POBLACION, MARIHATAG, SURIGAO DEL SUR;

ARTICLE II – RESPONSIBILITIES OF THE PCCMG

1. Conduct monitoring and evaluation on the progress of the work through its duly
appointed/designated inspection office and ascertain that the project was implemented in
accordance with the approved POW, plans and specifications;

ARTICLE III – RESPONSIBILITIES AND DUTIES OF THE PCCMG

1. Managed and disburse the funds in accordance with government accounting and auditing rules
and regulations;
2. Observe applicable provision and stipulations of Republic Act No. 9184 in the implementation of
projects/programs;
3. Undertake the implementation of the project in accordance with approved POW and plans;
4. Prioritized employment of displaced local workers;
5. Submit to the PLGU digital photos/videos before, during and after implementation;
6. Keep the books of accounts open for examination by PLGU through authorized representatives;
7. Implement the project within the duration of ONE (1) YEAR as stipulated in the program of
works;
 ARTICLE IV – RESPONSIBIITIES AND DUTIES OF PLGU

1. Provide Hospital Assistance to PCCMG members on duty at the hospital;

2. Provide PHILHEALTH membership;
3. Provide medicine assistance for PCCMG members on duty;
4. Provide food for those on duty; and
5. Provide coffee for those on duty (night shift and graveyard duty).

ARTICLE V – EFFECTIVITY AND AMENDMENT

This AGREEMENT shall take effect on the date of signing hereof by both parties shall be effective
immediately.
