1. Total number of accomplished transactions (releases, payments, re-pricing, renewals etc.) as
account assistant for 2013 was 651.
2. Total number of accomplished transactions (releases, debit/credit memo, reports, etc.) as loans
and credit processor for 2011 and 2012 were 580 and 826, respectively.
3. Submits reports within the committed time schedule.
4. Ms. Claudel is occupying a validated position and her functions are necessary to Davao Lending
Center.

The foregoing data are supported by information contained in the employee’s 201 file,
performance management documents, auxiliary files, among others.
