AMENDED PETITION

PETITIONERS, by counsel, to this Honorable Court, respectfully

aver that:

1. Petitioners Quirino U. Telow, Jr. and Mirasol S. Yap are both

Filipino citizens, of legal age, in possession of full civil capacity and legal
rights, married to each other, and residents of Poblacion, Dangcagan,
Bukidnon;

2. Petitioners were married on 25 July 2013, as shown in the Marriage

Certificate attached as Annex “A”, but have no issue on their own nor
adopted children ever since;

3. Petitioners are of good moral character, have not been convicted

of any crime involving moral turpitude, emotionally and psychologically
capable of caring for children, more than sixteen (16) years older than
the adoptee, and are in a position to support and care for their future
children in keeping with the means of the family;

4. On the other hand, adoptee Quinn Albert Yap was born on 1 April
2017, as shown in his Birth Certificate attached as Annex “B”, and is

already in the custody of the petitioners as the child has been voluntarily
committed to them by the child’s biological mother with consent of the
biological father;

5. The child’s biological mother, Maribeth Salon Yap, is unwed and

a sister of co-petitioner Mirasol Salon Yap. Attached are the Birth
Certificates of Maribeth and Mirasol S. Yap marked as Annex “C” and
“D”, respectively, to prove consanguinity;

6. The biological parents of the child gave their consent to the

adoption as reflected in their affidavits attached as Annex “E” and “F”,
respectively;

7. Adoptee is not disqualified by law to be adopted and has no estate

to speak of;

8. It is the desire of petitioners that the first name, middle name, and
surname by which the adoptee is to be known and registered in the Civil
Registry, if the petition is granted, would be QUINN ALBERT YAP
TELOW as it is but proper to carry the surname of c0-petitioner,
Quirino U. Telow, Jr.;

9. Attached are the Home and Child Study Reports marked as

Annexes “G”, “H”, “I”, and “J”;

10. Also attached are the Judicial Affidavits of petitioner spouses

marked as Annex “K” and “L”, respectively, with corresponding annexes.

Prayer

WHEREFORE, premises considered, it is most respectfully prayed of

this Honorable Court that judgment be rendered:

1. Declaring the child, Quinn Albert Yap, to all legal intents and
purposes, be the adopted son of petitioners Quirino Uy Telow, Jr. and
Mirasol Salon Yap;

2. Adoptee’s full name be changed, known and registered in the Civil

Registry as Quinn Albert Yap Telow;

3. Other reliefs and remedies that this Honorable Court may deem
just, proper and equitable in the premises.

citizens, all of legal age, and residents of Poblacion, Dangcagan,
Bukidnon, after duly sworn to in accordance with law, hereby depose
and say:

1. That we have caused the preparation of the foregoing Petition for

Adoption of Quinn Albert Yap with Change of Name to Quinn Albert
Yap Telow and we have read and understood the contents thereof; and
that the allegations therein stated are true and correct of our own
personal knowledge;

2. That we have not filed nor shall we file and prosecute before any
administrative or judicial body any other action or proceeding involving
the same issues or acts contained herein; and

3. That should we learn that a similar action or proceeding has been

filed or pending before any other tribunal or agency, we will notify this
Honorable Court within five (5) days from such notice.
