I, OLIVER P. MATAMMU JR., of legal age, Filipino, single and a resident of Purok 5
Kalye 11 Old Pampang, Angeles City, after having been duly sworn to in accordance with law,
hereby voluntary depose and say:

1. That I bought a motorcycle from the Rusi Company located at Sta. Cruz, Porac,
Pampanga;

2. That said motorcycle is more particularly described as follows:

4. That recently, I have attached a sidecar to said motorcycle unit, which attachment
was not included in the registration;

5. That I am executing this affidavit to change the Body Type of said motorcycle to
Motorcycle with Sidecar and attest to the truth of the foregoing and in support of my request
to change the entry in the Body Type section to Motorcycle with Sidecar in the motorcycle’s
OR/CR.
