Affidavit of Loss

I, Mr. John Patrick B. Arazo, a fourth year student, taking up Bachelor of Science in Accountancy from the
College of Accountancy and Finance with student number 2014-00680-MN-0 do hereby depose and say:

That sometime on September 2018, I lost my identification card due to unforeseen events, and
ultimately my negligence; that earnest and diligent effort to locate the same was rendered futile, and
that the same was not confiscated by authorities for violation of a ny law or used for illegal purposes.

In view thereof, I am executing this affidavit to attest the truth and for whatever legal purpose it may
serve.

AFFIDAVIT OF LOSS

I, John Patrick B. Arazo of legal age, single, Filipino citizen, and a resident of the Philippines after being
sworn to accordance with the law depose and say:

1.) I am a holder of an identification card issued by the Polytechnic University of the Philippines.

2.) The foregoing identification card with I.D. No./Student No. 2014-00680-MN-0 was lost on September
of 2018 because of negligence.
3.) That despite diligent efforts ahs been exerted in order to find and locate the said identificatoin card;
the same could no longer be found and proved to be beyond recovery;

4.) That this affidavit was executed to attest the truth of the foregoing and for purposes of completing
my general clearance for graduation in the absence of my identification card in my favor and whatever
legal purposes it may serve best.

BY WITNESS THEREOF, I have been unto set my hand this 6th day of December, 2018 in the City of
Manila, National Capital Region.
