LESSON LEARNED#13 – Fatal injury due to crushing during
handling of material

Accident summary:
During the loading of wastes from a caisson to a truck, by means an excavator equipped
with bucket, the operator of the vehicle has been crushed between the excavator and the
caisson.

Main cause of the accident:

 Operator wrong position, on the excavator stabilizer;
 Working procedure not followed;
 Lack of leadership during supervision in the field;
 Pre-job briefings inadequate.

Corrective actions taken by the enterprise:

 Introduction of new instruments for observation of the field behavior;
 review of operating modes relating to the organization of the activities and
identification of roles;
 awareness campaign on behavioral factors that cause accidents;
 updating of training programs for supervisors and workers.

LESSON LEARNED
It is unacceptable that the person in charge for the safety does not
guarantee the surveillance regarding the strict compliance with the
instructions received, and specifically the rule of not ever having to
pass within the range of action of the vehicle

The procedures, result of careful planning of the activities, and the

roles identified in them, must be complied with
