We, GODOFREDO L. TAGUINOD and CATALINA V. RILLORAZA,

both of legal age, married and widow, respectively, Filipino citizens and residents
of Barangay Bonfal West, Bayombong, Nueva Vizcaya, after having been duly
sworn to in accordance with law, hereby depose and state:

1. We know personally DOMINGA D. RAMIREZ who was born to parents

Maximino Ramirez and Aurelia Dela Cruz on December 14,1924;

2. We attest the fact that DOMINGA D. RAMIREZ was born at Barangay

Bonfal West, Bayombong, Nueva Vizcaya;

3. We execute this affidavit to attest the foregoing statements and for whatever
legal intent and purpose it may serve.
