AFFIDAVIT

I S SEKAR S/O S SUBBARAYUDU, Indian Hindu , aged about 17 years, residing at Drno 5-5-381,
Sanjaigandhi nagar, Tirupati Urban, Tirupati, chittoor district do hereby solemnly affirm and state
as follows.

1. I am the deponent herein.

2. I state that i was born on 18-05-1999, Presently I residing at Door no 5-5-381, Sanjaiganhi
nagar,Tirupati Urban, Tirupati, Chittoor Dist, Andhra Pradesh, sonce 10 years. This affidavit is
proof of my support of my residence. Hence this affidavit.

3. This affidavit is required to be produced before the concerned authorities, so as to prove we

are residing at the above said address since10 years and to obtain Aadhar card in favour of S
SEKAR, S/O S SUBBARAYUDU.

4. The above said facts are true and correct to the best of my Knowledge, belief and information.
