WAIVER OF RIGHTS

KNOW ALL MEN BY THESE PRESENTS:

I, CRISTINA C. RAMOS, Filipino, of legal age, and a resident of Ilang-

Ilang, Kalibo, Aklan, Philippines, freely and spontaneously declare and make
manifest:

That I am the wife of the late LORETO ULAY NALAYOG, SR. who was a
Member of COCOLIFE and who died on February 12, 2015;

That as his legal spouse and beneficiary, I am entitled to his death

claims from the COCOLIFE Office, Iloilo City;

NOW THEREFORE, for and in consideration of the foregoing, I hereby

WAIVE, RENOUNCE and QUITCLAIM all my rights, interests and participation
in the above-said claims in favor of my daughter MILA N. NO-O.
