JOINT AFFIDAVIT OF TWO (2) DISINTERESTED

PERSONS
That WE, LALAINE R. CARBON, of legal age, married, Filipino
and a resident of Bgy. Tiniguiban, Puerto Princesa City, Philippines
and LEILANIE A. TIMBANCAYA, likewise of legal age, married and
a resident of Bgy. Tiniguiban, Puerto Princesa City, Philippines,
after having been duly sworn to in accordance with law, hereby
depose and say:

1. That we personally know and are familiar with DEXTER

ANGELO SANTIAGO LAMBAN, his family being our close
friendsand he is likewise our neighbor;

2. That we likewise know that the said DEXTER ANGELO

SANTIAGO LAMBAN is the minor child of SPO4 EDDIE PILAR
LAMBAN (now deceased);

3. That in the Insurance Policy of Public Safety Mutual Benefit

Fund Inc. (PSMBFI) where the said SPO4 EDDIE PILAR
LAMBAN was a member, the latter erroneously entered the
name “DEXTER S. LAMBAN” instead of “DEXTER ANGELO
SANTIAGO LAMBAN” as one of the beneficiaries;

4. That the true and correct name of DEXTER ANGELO

SANTIAGO LAMBAN is DEXTER ANGELO SANTIAGO
LAMBANand not DEXTER S. LAMBAN;

5. The names “DEXTER S. LAMBAN” and “DEXTER ANGELO

SANTIAGO LAMBAN” refer to one and the same person;

6. That we are executing this JOINT AFFIDAVIT to attest to the

foregoing and to use the same for whatever legal purpose it
may serve them best.
