AFFIDAVIT OF SUPPORT AND CONSENT TO TRAVEL

after having been duly sworn to in accordance with law, do hereby depose and state:

1. That I am the biological mother of __________ who were born on ________ in ___________;

2. That said children will travel to ____________ in order spend some time with their
grandmother ___________ from ________ to ___________;

3. That the children will be accompanied by their grandmother ________ on _________. They
will stay in the said country for a period of five (5) days. My children will be under the
custody of their grandmother upon arrival therein;

4. That I cannot personally travel with my children due to several workloads and busy
schedule;

5. That I am giving my full consent for the travel abroad of my children who will be
accompanied by their grandmother __________;

6. That I personally guarantee the financial support of my children in the course of their
travel and stay outside the Philippines and further guarantee that they will not in any
manner become a burden to the Philippine and _______ governments;

7. That I am executing this affidavit, to attest to the truth of the foregoing, for the
application for my children’s passport at the Department of Foreign Affairs and/or
travel clearance from the Department of Social Welfare and Development for their
travel abroad and for whatever legal purposes they may serve.
