Fitness for Life

Instructor: Mrs. Holmes
Email: mholmes@saltlakespa.org Phone: (801) 466-6700 ext. 106

Course Description:

Fitness for Life is an individualized, concept-based, one-semester course designed to give students the
knowledge and skills necessary to self-assess, create, conduct, evaluate, and redesign personal fitness
programs. The course is a combination of classroom and activity-based learning activities with a focus on
proper nutrition and the mastery of skills and concepts necessary for students to become accomplished
monitors of their personal lifetime fitness.

Course Objectives:

By the end of high school, students will be college and career ready, as demonstrated by the ability to
plan and implement different types of personal fitness programs, demonstrate competency in lifetime
activities, describe key concepts associated with successful participation in physical activity, model
responsible behavior while engaged in physical activity, and fill a need for self-expression, challenge,
social interaction and enjoyment.

Dress Policies and Hygiene:

All students are required to dress appropriately for Physical Education EVERY CLASS.

o Inappropriate PE attire: Skirts, dresses, boots, flip flops, sandals, and sheer or revealing clothing
is not appropriate. Students whose clothing or footwear limit them from participation will have
to sit out and will lose participation points for the day.
o Hygiene: Please be aware there are no shower facilities at our school. Showers need to be
happening on a very regular basis at home and remember that deodorant!!

Lockers:

Each student at SPA has been assigned a locker. It will be expected that backpacks, jackets, food and
especially cellphones will be stored in these lockers during class. If we are going outside in colder
weather, students will be allowed to get a jacket. There will be no other exceptions to this rule. Paper
and pens/pencils will be provided when we are working on paper assignments. Students will be excused
5 minutes prior to the bell in order to change clothes/get what they need from their lockers for the next
class period. If students need assistance in locating their assigned locker or getting a lock, they can
speak with Michelle in the office. It will be expected that students are using their locker by the second
day of class.

Makeup Credit: (Up to 4 per term allowed)

Students with excused absences will have the opportunity to make up their points by completing 60
minutes of physical activity at home for each absence and completing a 300 word (about 3
paragraphs) write up of an article either relating to exercise or nutrition. This should also include a
signature from a parent/guardian. These are due within 2 weeks of each absence.

Personal Fitness Log:

As the endpoint of this class is to help students put in place good fitness habits, a fitness log will be
required. A single subject spiral bound notebook will be fine. Students should plan on updating this log
at least once a week. The logs will be required to be brought into class at midterm and at the end of the
term for grading. Prior notice will be given.

Evaluation:

Each day is worth just 5 points in the following manner:

 On-time Attendance (10%) 25 points (1 per day)

 Appropriate Dress (10%) 25 points (1 per day)
 Participation (30%) 75 points (3 per day)
 Personal Fitness Log
o Midterm check (20%) 50 points
o End of term check (20%) 50 points
 Assignments (10%) 25 points

Participation:

 It is expected that students will participate fully each class period in whatever activity we are
doing. If students are unable to participate on a given day, an alternative written assignment
will be provided for half credit. If a student has a legitimate medical reason, a note from a
medical provider will allow them to do the alternate assignment for full credit. Meetings with
the student, guardian and teacher can be arranged as needed.
 If parents need to get ahold of their student during class, please contact the office. Students will
not be allowed to “check” their text messages during class. It is advisable that transportation
arrangements be taken care of prior to the end of the student’s lunch break.

What to Bring:

 A water bottle should be brought to class. There is a bottle-filler across the hall making use very
convenient.
 Appropriate clothing and footwear

Language and Behavior:

 Profanity, negativity and bullying will not be tolerated. Our class is a positive learning
environment for all and each student is expected to add to not detract from the class. Students
may be asked to sit out of an activity if behavior persists and participation points may be lost for
that day.
 Classroom rules will be posted in the front of the room and must be followed in all locations
used for PE.

Injuries:

Physical Activity can result in minor to severe personal injury. As a participant in this class you
acknowledge the risks associated with physical activity and will not hold Salt Lake School for the
Performing Arts, its affiliates, or the instructor, Margaret Holmes, liable for any injury sustained through
participation in this course. Based on the severity of the injury, students may require a note from a
medical provider before they can begin or resume participation in the class.

Americans with Disabilities Act:

In compliance with the Americans with Disabilities Act, students in need of accommodations may
contact the instructor for alternate arrangements.

Bus Use:

Occasionally we may use the SPA school bus to go to a nearby park as part of PE. Signing below will give
your student permission to participate.
