You are eligible for REAL ID

Bring the following documents with you to a DMV office:

Out of State REAL ID or Enhanced Document


Must be current or not expired for more than two years.
U.S. Social Security Card
Federal or NY State W-2
Must show name and current NY State address. Must be
mailed to your residence address within one year of your
DMV office visit.
Health Insurance Card WITH YOUR ADDRESS
PRE-PRINTED
Must show pre-printed name and current NY State
address. Must show current proof of health insurance
coverage.
U.S. Passport
Your Passport must be valid (not expired). Be sure to
check the expiration date.
Your Passport must show your full middle name, if
applicable. If your full middle name DOES NOT appear
on your Passport, you must bring in your U.S. Birth
Certificate.

Also:
Complete the Application for Permit, Driver License or Non-Driver ID Card
Standard Driver License (class D, DJ, and N) $65 or $75*
Motorcycle (class M and MJ) $70 or $80*
Taxi, Livery, Limousine (class E) $95 or $105*
Commercial Driver License (class A, B and C) $127 or $137*
* Residents of these counties must pay the Metropolitan Commuter Transportation District (MCTD) fee
($10): Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Dutchess,
Nassau, Orange, Putnam, Rockland, Suffolk and Westchester.

Fees depend on your age when you apply and where you live. Exact fees cannot be calculated until you
apply. Visit driver license and learner permit fees for more information. Payment can be made by cash,
credit, debit card, check or money order made out to Commissioner of Motor Vehicles.

Please note:

Your documents must


Be original or certified. We cannot accept photocopies, damaged or illegible documents.
Have the same first and last name (If your name changed, bring an original or certified
copy of your marriage certificate, divorce decree, or court issued name change.
Documents must be issued in the U.S.).
At least one document must have your full middle name (if applicable).
Be printed off if your proof of residency was sent to you electronically.
You must be clearly recognizable in any photo document.
Bring your glasses or contact lenses (if needed) to pass your vision test for license renewal or
original permit.
Your proofs of residence must show your current address (cannot be a P.O. Box).
DMV cannot accept two proofs from the same financial institution (bank statement, credit
card statement, ATM card, credit card or cancelled check).
