INSTRUCTIONS
1. This is only open for DLSU-M (Green & White 2011-2014, 2016) and DLSU-STC (Green & White 2016).

3. Fill out this Project Green & White Appointment Form with your complete details.
NOTE: For Representative Claiming, download and fill out the authorization letter on this link:

4. Choose between Personal or Representative claiming
5. Choose your two (2) desired appointment schedules.
a. This is the link for the distribution dates https://goo.gl/dTs9Vo
b. Your appointment schedules should be a week before. Be sure to submit your
accomplished form on or before the indicated deadline by 6PM.
c. Late accomplished appointment forms will be rejected and will be asked to choose a different
appointment schedule.
d. Claiming dates may change due to class suspension, staffers availability, etc. Be sure to
check the Project Green & White Facebook page from time to time.
6. Appointment forms should be saved in PDF format only.
7A. (Personal Claiming) Send your accomplished appointment form to

appointment form.

8. All requests will be processed during the weekend.
