AFFIDAVIT OF DISCREPANCY

I, MARIA MAGDALENA, of legal age, single, Filipino and resident of 888 General Luna
St., Intramuros, Manila, under oath declare:

1. That I was born on May 28, 1963 in Aurora, Quezon to the spouses Angel Ramos Tolentino
and Cinderella Morales Tolentino;

2. That I was baptised on August 18, 1963 at San Pedro Parish in Aurora, Quezon with the
name MARIA MAGDALENA MORALES TOLENTINO as appearing on my
Baptismal Certificate. A copy of my Baptismal Certificate is hereto attached as ANNEX
“A”;

3. That since childhood, I am known by the name Maria Magdalena Morales Tolentino which
I have been using in all my personal records, transactions and communications. A copy of
my Transcript of Record, Driver’s License and Passport are hereto attached as ANNEXES
“B”, “C”, and “D”, respectively;

4. That the name appearing on my Certificate of Live Birth registered with the Civil Registry
of Aurora, Quezon and with the National Statistics Office is MA. MAGDALENA
MORALES TOLENTINO. A copy of my Certificate of Live Birth issued by the Civil
Registry of Aurora, Quezon and the National Statistics Office are hereto attached as
ANNEXES “E” and “F”, respectively;

5. That the word or name “MARIA” is commonly accepted to be abbreviated as “MA.” in

the Philippines;

6. That, however, when my daughter petitioned me to be an immigrant of France using the

name Maria Magdalena Morales Tolentino, the immigration authorities in the Embassy of
France in the Philippines requested us to have the discrepancy in my name corrected so
that they can start processing said petition of my daughter;

7. That since it will be easier to correct one document which is the Certificate of my Live
Birth, I am executing this two-page affidavit (including the jurat) to rectify my name on
said document and change the abbreviation “Ma.” to the full word or name “Maria”;

8. That this affidavit is further executed to attest to the truth of the foregoing facts and for all
intents this might be of legal worth;

IN WITNESS WHEREOF, I have hereunto affixed my signature this 21st day of November,
