I am Isagane S. Fundal resident of Barangay Li-ong Roxas City, Teacher-I of
Dumolog National High School, Roxas City.

I would like to request for transfer from Dumolog National High School to Don Jose
Acevedo Integrated School (DJAIS ) at Li-ong , Roxas City.
The undersigned is requesting for transfer due to the following reason

1. I am a resident of Brgy. Li-ong Roxas city.

2. I can render better services to my school and students since I am near to them .
3. My transfer will save time, money, efforts, etc. on my part.
4. I had already served at Dumolog National High School for almost Eight
years.
5. I am teaching T. L.E. -8 and MAPEH-8. EsP-7.
6. I earned four NC-II such as carpentry ,masonry driving, and automotive servicing.
7. My wife undergone in her shoulder dislocation operation , due to accident.

I am hoping that this request Should be granted.
