II. DESCRIPTION OF THE CLIENT

The client is admitted at NCMH, 2 years ago for murdering someone and cutting his own hand. The
client is wearing the usual hospital uniform during the interaction. He stands approximately 5’3, has a
small body physique, fair skin, has a scar on his left head, only has one hand/amputated arm. He seems
joyful and cooperative.

III. DESCRIPTION OF THE ENVIRONMENT

During the orientation phase, we talked in the receiving area of the pavilion 4. It’s around 9:35 in the
morning. It was well ventilated, we are seated face to face with at least 3 feet.

IV. OBJECTIVES

ORIENTATION PHASE:

At the end of the Interaction with the client the student will be able to:

 Build trust and rapport by demonstrating trust

 Establish therapeutic environment and privacy
 Learn about the client’s back ground
 Assess the client’s knowledge about reality
WORKING PHASE

At the end of the 15 minutes interaction between the client and the student, the student will be able to:

 Assist the client to express his thoughts and feelings

 Have a good rapport and trust
 The student will have a better understanding about the client

TERMINATION PHASE

At the end of the 15 minutes interaction between the client and the student, the student will be able to:

 Encourage the client to discuss about his feelings regarding the activities that have done.
 Prepare the client for termination and separation
 Determine the client’s plan upon his discharge

VI. HISTORY

The client was brought to the NCMH , 2 years because of the reason that he murdered someone by cutting
its neck to death. According to him, the reason why he did it was he had some auditory hallucinations
telling him to kill someone. And after doing it, he also cut his own left hand, he had a murder case and
after some mental evaluation he was proven and diagnosed as a schizophrenic person.

VII. REFLECTION

Time flies really fast. It feels like we just started our clinical duty yesterday, but in reality, we only have 2
days left here at the National Center for Mental Health. I guess that people were right when they said that
you will never notice the amount of time you have spent on something when you enjoy doing it especially
if there is learning involved. During my stay here, I have never thought that I will learn more from our
patients than the the patients will learn from us. I can still remember the first activity that we had about
self awareness, one of the questions we have there was “ what do you expect during your stay here” and
I answered “ I expect to learn more and become more aware about the mental health and to share, know
and hear a story“ and just like what I expected, I heard stories, different stories about these different
people with different status when it comes to mental health. And just by hearing these stories I can
already say that these people are fragile, they are like a book, in every chapter you will discover and learn
something. Right, during my stay here I have learned so much about mental health, on how we could help
to stop the stigma about them, and how we could promote what their real advocacy is. And that is to
spread awareness. How we could extend our hands to someone who needs help, how a simple talk could
mean something for someone. As a student nurse, I found out that I matter, what I have learned here in
the area matter because who knows, your words might have save someone who is drowning in the sea of
depression. We, in our own little way can help someone drown in depression to see the beauty and the
brighter side of life.
