AFFIDAVIT OF LOSS

I, MILAGROS N. ARAGON, of legal age, Filipino citizen, and a

resident of Brgy. Tuao South, Bagabag, Nueva Vizcaya, after being sworn
to in accordance with law, hereby depose and state that:

1. I am the Attorney-In-Fact of the heirs of the registered owner of a

parcel of land covered by Transfer Certificate of Title No. T-58681
registered in the name of Domingo Aragon;

2. The said parcel of land is identified as Lot 466-Y, of the subdivision

plan (LRC) Psd-231537 being a portion of Lot 466, with an area of
FIVE THOUSAND TWO (5,002), SQUARE METERS, more or
less, located in the Barrio of Tuao North, Municipality of Bagabag,
Nueva Vizcaya, Island of Luzon;

3. Sometime in the January 2016, I went to the Municipal Assessor’s

Office to inquire on the real property tax payments of the heirs of
Domingo Aragon and brought with me the Owners Duplicate
Certificate of Transfer Certificate of Title No. T-58658 which I
placed inside a long brown envelope together with other documents;

6. That on February 27, 2017, I blottered the incident at the Office of

the Punong Barangay of Dupax del Norte, Nueva Vizcaya and then
at the Dupax del Norte Police Station on February 28, 2017;

7. That I attest to the truth of the foregoing statements to the best of my

own personal knowledge and based on authentic documents or
records. I am executing this Affidavit for all lawful intents and
purposes it may serve.

IN WITNESS whereof, I have hereunto set my hand on this Affidavit

of Loss this 3rd day of March 2017 at Bayombong, Nueva Vizcaya.
