JUDICIAL AFFIDAVIT

I, SHERYL ROSALES, is a Filipino, of legal age, married, and resident

of No. 56 Hunters ROTC Avenue, New Saint Francis Village, Brgy. San Juan,
Cainta, Rizal, states under oath as follows:

The person interrogating me is _________ with office at _________.

The interrogation is being held at the Max's Restaurant, Robinson's Cainta,
Rizal, in the English language that I am very familiar of. I am answering his
questions fully conscious that I do so under oath and may face criminal
liability for false testimony and perjury.

OFFER OF TESTIMONY

The testimony of witness, DR. SHERYL ROSALES is being offered to

prove the injuries sustained by the complainant as a result of the defendant's
beating. She will prove/corroborate all the material allegations embodied in
the complaint, and will identify several documents in relation thereto;

1. Please tell the honorable court your name, age and address.
I am Sheryl Rosales, Filipino, 32 years old and a resident of No. 56 Hunters
ROTC Avenue, New Saint Francis Village, Brgy. San Juan, Cainta, Rizal.
2. What is your relation with the parties in this case?
I am the doctor of Ms. Angela Rosales whom she consulted at the time
of beating.
3. Can you tell us anything significant that happened at the night of ____
, if any?
That night, I was in the ER and Ms. Angela went there to seek first aid for her
injuries.
4. What kind of injuries were you able to see?
I saw bruises located at her right arm. But she also complained of her ankle
and head injuries.
5. What happened after that?
She was subjected to an X-ray examination. And I gave her the necessary
treatment for her injuries.
6. What were your findings?
Upon thorough examination, it was found that she had fracture on the lateral
malleolus of the right ankle, moderate trauma on the right side of the skull and
contusions on the right side of the arm.
7. Can you please identify this document for the court?
Yes. That is the medico legal report that I issued to Ms. Angela Rosales at the
night of the beating.
