JOINT AFFIDAVIT

KNOW ALL MEN BY THESE PRESENTS:

WE, DENNIS AMAR ADARLO and REGINA LYN LIBRES

BALBASTRO, Filipinos, of legal age, with residence and postal address
at Brgy. Tiguisan, Bansud, Oriental Mindoro, after having been duly
sworn to hereby state, THAT:

1. We are the biological parents of minor MARIA DENISE LIBRES

ADARLO;

2. Said minor child was born on August 29, 2018 in Brgy.

Tiguisan, Bansud, Oriental Mindoro;

3. From the moment of the birth of the said child we had very big
difficulties in looking after the minor because we are not in the
condition to take care of her due to our very difficult economic
situation, due to the lack of a stable employment and the
circumstance that our family is already composed of 5 sons.
For these reasons our family unit is made subject of a case
study by the Social Workers of _______. Due to the above
described situation MARIA DENISE or “Mia” would end
neglected and abandoned;

4. Her paternal aunt, LORIE ADARLO BONDI and her husband

DAVIDE BONDI are our relatives that we really love and
appreciate and we value them as serious and good people.
They live in Via Pogdora n. 16, 41043 Casinalbo di Formigine,
Italy, in a comfortable and healthy house; and they have
expressed their availability to the fact that Maria join them at
their present place of residence in order to take care of her
granting her a good upkeep and maintenance considering that
they both have a job, and are able to take care of her

5. Lorie Adarlo Bondi has been the actual physical custodian of

the minor child maria Denise since she was an infant up to this
date, taking care of all the baby’s needs;

 6. We voluntarily assign to said Ms. Lorie Adarlo and her husband
all the rights to manage the baby and take care of her
protecting her rights and do not object whatsoever to her
leaving with them in Italy;

7. Davide Bondi and Lorie Adarlo Bondi have expressed their

availability that our daughter Maria Denise join their family in
Via Pogdora n. 16, 41043 Casinalbo di Formigine, Italy to stay
with them and enable her to have a good upkeep and
maintenance;

8. Wherefore we declare solemnly and conscientiously to confirm

that we do not have any objections and we fully consent to
Maria Denise residing in Italy in the home and with the family
of Davide Bondi and Lorie Adarlo Bondi.

9. We formally ask to the competent authorities to give execution

to the above

10. We are executing this Affidavit to attest to the veracity of the

foregoing statements and to prove that LORIE ADARLO BONDI
and/or DAVIDE BONDI are free of any liability whatsoever in
their taking custody and care, as well as bringing the minor
Maria Denise Adarlo with them in their travels as they deem
best under the circumstances.
