• SEMUA LAPISAN/TAHAPAN PENGEMASAN : dengan label sitostatika yang jelas,
termasuk bungkus terluar saat distribusi
• Wadah terlindung cahaya matahari sesuai sifat obat
• Apoteker memastikan maksud wadah yang digunakan jika berupa gelas. Apakah
dimungkinkan dengan wadah yang lain
• Apoteker melakukan pengambilan jika kemasan bentuk blister
• Dispensing : lengkapi instruksi dan label sitostatika + hal yang perlu diperhatikan
• Peracikan : tidak boleh digerus
• Apoteker menetapkan :
• Penempatan dalam penyimpanan tidak boleh bercampur dengan obat non
sitostatika
• Aman
• Instruksi : Tidak mudah dijangkau anak (pasien di rumah)
Prosedur - dispensing
1. Baca ulang resep : nama obat, kekuatan, bentuk sediaan, jumlah
2. Cuci tangan
3. Pakai masker dan sarng tangan
4. Ambil sejumlah obat dengan pelan, seminimal mungkin menghasilkan debu
5. Dilarang hanya membaca nama dan jumlah obat dari kemasan
6. Baca ulang nama, kekuatan dan cek jumlah obat
7. Masukkan dalam plastic, dengan nama menghadap luar sehingga mudah dibaca
8. Beri label HIGH ALERT, sitostatika
9. Lindungi dengan wadah plastic dengan ukuran lebih besar dari wadah plastic
pertama
10.Pastikan label mudah terbaca
11.Pastikan nama obat mudah terbaca, agar mudah untuk cek ulang tanpa
mengeluarkan obat
12.Lepas APD

7. Baca identitas pasien
8. Baca instruksi pengobatan pemberian obat
9. Baca obat
10. Dobel cek dengan sejawat senior
11. Ambil tablet
12. Hitung ulang jumlah tablet
13. Masukkan air lebih kurang 20 ml
14. Masukkan tablet dalam wadah berisi pelarut
15. Tunggu tablet mulai hancur
16. Aduk pelan pelan satu arah
17. Berikan pasien
18. Sisa wadah diberi air-pelan-pelan dan diminumkan, diulang sampai obat bersih
19. Wadah, pengaduk dan APD masukkan dalam tempat sampah dan ikat kuat
20.Cuci tangan

Any unused medication should be returned to the
pharmacy for appropriate disposal
