b) A clear photocopy of my most recent utility bill

(phone, gas, water etc.) showing my name, address and
statement date (no older than 3 months)

c) A clear photocopy of the front and back of the following

card(s):

NB: Please hide/cover the middle numbers on the front side

of your card and the 3 digit CV2 code on the back strip of
your card. Please ensure that your name, the expiration date,

statement for each card listed above showing my name,
address, credit card number and statement date
