Dear Mr./Ms. [Name],

We are pleased to confirm our offer of employment effective ________, under the
following terms and conditions:

1. Position: You shall be hired as _____________, and your duties will be


as follows:

c. and such other duties and responsibilities that may be assigned to you
by management from time to time.

2. Employment Status: You shall undergo a period of probation for six (6)
months during which time your performance on the job and overall
compliance to Company policies and regulations shall be reviewed prior
to confirmation of regular employment.

3. Compensation:
a. Your basic salary is Php per month payable every 15th and

30 of each month. This includes payment of all legal holidays and
special holidays.
b. Your monthly compensation is subject to withholding tax.
c. You are entitled to a 13th month pay equivalent to one-twelfth (1/12) of
your basic salary within a calendar year or a proportionate amount
thereof in compliance with PD# 851.

4. Contributions:
a. You are required to contribute to the benefits provided under existing
laws such as the Social Security Act and the Philippine Health
Insurance Act, among others, deductions of which are properly
reflected in your payslip.

5. Benefits: You will be entitled to the following benefits:


a. Holiday pay
b. Overtime/Night premium pay
c. Service Incentive Leave of five (5) days after one (1) year of
employment
d. Other benefits subject to confirmation of regular employment and one
year of service to the Company
e. Benefits duly provided under the law

6. Working Hours: Regular work schedule shall be five (5) days a week,
eights (8) hours a day exclusive of one (1) hour meal break. However,
changes in work schedules shall be made following operational
requirements and in accordance with labor laws. Work in excess of the
regular work hours will be compensated similarly.

7. Other Arrangements:
a. This letter when signed by you shall serve as our employment
contract which shall automatically expire on ______________, unless
otherwise terminated earlier. Should you pass the period of probation
your regular appointment shall be confirmed in writing starting
_____________. Any increase in salary will be dependent upon your
performance review and no adjustment is promised in connection with
your regular employment status.
b. Your appointment is subject to the submission of and compliance with
all pre-employment requirements.

c. The Company shall provide you a copy of the Employee’s Manual and
the Code of Conduct during the orientation. Details of Company
benefits, policies, guidelines, rules and regulations shall likewise be
discussed in the orientation.
d. While employed with the Company, you shall comply strictly with our
policy on “conflict of interest” and not become employed or connected
with any other company, partnership or entity.
e. In the event that your services is terminated for whatever reason, you
are to resist and/or reject employment by or any kind of relationship
with any of the competitors of the Company for a period of at least six
(6) months.
f. Should you desire to resign from the Company, you are required to
give thirty (30) days written notice to enable the Company to look for
and hire your replacement and for you to turn over any accountability
to the Company. Failure on your part to comply with this requirement
shall constrain the Company to hold you liable for any and all
damages that may arise because of your untimely resignation.
g. All information that you may gain access to in the course of your
employment with the Company shall be held in strict confidence.
